Quick answer
Three documentation formats for the outpatient contact, the crisis assessment and consultation with the care chain.
Here is a brief overview of how Hysio writes up a home visit or crisis contact. You read every report back before you share it with the care chain.
The SPV profile covers Outpatient Contact & Home Visit, Crisis Assessment and Multidisciplinary Meeting & Care Chain Handover. They focus respectively on one outpatient contact, a crisis contact that has already been professionally assessed and a handover to a care chain partner. Choose based on the purpose of your document. Hysio organises the information provided, while observations, professional interpretation, decisions and implementation stay with their original source. [3] [4] [5]
The contact report describes the request for help, the perspective of the person seeking help, the living environment and the network. Concrete observations are kept separate from your stated SPV interpretation. Interventions carried out, responses and follow-up agreements keep their own status. Hysio does not fill in a diagnosis, urgency or care goal. Earlier documents are only included as recognisable source context when you actually provide them. [3]
No. The name refers to the report of a crisis contact that has already been assessed by the SPV and any other healthcare professionals involved. Hysio records the reason, timeline, observations, stated assessment and agreements. The platform does not carry out triage or risk assessment and does not choose a legal route, admission or medication. Those decisions stay with the authorised people involved. [4]
This documentation format is intended for a handover about one person seeking help. Information from the multidisciplinary meeting (MDO) can be a source for it, but the result is not a stand-alone set of minutes of the meeting. The document describes the intended recipient, relevant information provided, explicit decisions and actions. Hysio does not itself decide which data may be shared and does not assume that anything has been sent or received. [5]
Describe which change the person seeking help wants and which goals were actually discussed. Keep your professional interpretation of this separate. Hysio does not turn a wish into an agreed care goal and does not choose an intervention to achieve that goal. State separately which support was provided and which follow-up was agreed, so a reader can follow the contact. [3]
State when reports, observations, consultations and interventions took place. Keep historical risk information separate from the current enquiry. Hysio must not automatically present an earlier judgement as the current assessment. Check that each decision sits with the right source and the right moment, and that a planned action does not wrongly appear as carried out in the summary. [4]
Provide the contributions that are relevant for this handover and name the speaker. Keep proposals, dissenting views and explicit decisions separate. Hysio must not assume a joint position because several participants were present. Also make clear which information goes to which recipient and which actions were agreed. Taking part in the multidisciplinary meeting does not in itself imply consent for an external handover. [5]
Note who gave the information and what it relates to. A report from a relative stays distinct from the perspective of the person seeking help and your own observation. Hysio does not automatically turn these into one statement. Only include relevant information and keep differences visible if they were not clarified during the contact. [3]
The subscription price for community psychiatric nursing is separate from the chosen care moment and the documentation format. For community psychiatric nursing within outpatient mental healthcare and the client's living environment, Hysio's Starter plan costs €20 per month, excl. VAT. Pro costs €40 and Unlimited €80 per month, excl. VAT; with yearly billing you get two months free. The community psychiatric nurse assesses the content for recovery-oriented goals in the living environment and decides what is adopted. [6]
For an outpatient contact or home visit, decide which information is needed and follow your organisation's agreements on processing and access. Information from relatives can be relevant, but does not have to end up in full in every report. Before sharing with another person involved, check the legal basis and the purpose. Hysio's privacy policy explains the processing roles and use of data. [8]
"Nurse" is a protected profession under article 3 of the Wet BIG (Dutch Individual Healthcare Professions Act). A valid BIG registration matters for using that title. The specific SPV expertise and any quality registrations are distinct from this. A contact report shows what was done with a person seeking help, but is not proof that the writer meets the registration requirements. [14]
The SPV works from the nursing profession with additional social psychiatric expertise. The Wet BIG describes the general area of expertise of the nurse; the SPV area of expertise works out the specific context in more detail. Keep that professional position distinct from the content of an individual report. A described action or assessment does not by itself prove which powers or registrations the writer holds. [11]
The field standard (veldnorm) lists two registrations for the community psychiatric nurse: a BIG registration as a nurse and registration in the Kwaliteitsregister V&V with the area of expertise Sociaal psychiatrisch verpleegkundige. That is a description of the professional position. A report of a home visit or outpatient contact is something other than proof of those registrations; so keep professional registration and client-related documentation apart. [15] [14]
The SPV area of expertise from 2023 refers to the professional profile of the bachelor-level (hbo) nurse, the code of conduct for nurses and carers and the SPV's own professional code. Those documents serve different functions: describing professional practice, guiding professional conduct and clarifying the specific SPV context. For a concrete question, use the appropriate document and check whether a newer version has since appeared. [11]
No. The Landelijk Kwaliteitsstatuut GGZ describes conditions and competencies for the lead practitioner role (regiebehandelaarschap), partly depending on the setting. The job title alone does not determine the role. If the SPV actually fulfils this role, it must be visible who handles alignment and coordination and how responsibilities have been agreed. A Hysio report does not assign this position on its own. [19]
The difference between the SPV and the nurse practitioner in mental healthcare stays recognisable in the documentation, with each role having its own source and professional contribution. A verpleegkundig specialist GGZ (nurse practitioner in mental healthcare) is a separate, legally regulated specialism under article 14 of the Wet BIG, with its own register and independent authority for some of the reserved procedures (voorbehouden handelingen). The SPV has a different area of expertise. Keep the roles apart in the documentation and write down for each healthcare professional which contribution was actually made. [14]
Make clear which information comes from the GP, the POH-GGZ or the SPV and which alignment actually took place. Keep a referral, consultation contribution and agreed next step apart. The involvement of a care chain partner does not automatically say who is responsible for each task. So state concrete agreements and open questions, without filling in roles or task acceptance. [11]
No. Population studies give context about a studied group and period, not a diagnosis or prediction for someone in your caseload. For any figure, always check the study population, the year and what exactly was measured. In the individual report, use the information from the contact and your assessment, without adding conclusions from general percentages. [11] [12]
When you write it yourself, you formulate the note directly. When you dictate, you first provide information that Hysio helps to organise; that also counts as voice-driven admin. In both cases the professional assessment stays with you. Compare whether the perspective of the person seeking help, network details and agreements come back correctly and how much correction is needed. [9]
You can use Hysio free for 14 days for a fictional outpatient conversation. See how information about the request for help, the living environment and agreements is organised. Check the platform's current options in advance and assess the text yourself. You can also use the documentation formats for community psychiatric nursing during the trial. [7]
The V&VN guideline Richtlijn Verpleegkundige en Verzorgende Verslaglegging (nursing and care documentation guideline) provides the general framework for recording the nursing process. In addition, use the SPV area of expertise for the specific professional context. A recognisable contact note must fit the record as a whole. Choosing a Hysio documentation format does not replace the guideline or the agreements on documentation within your organisation. [13]
The area of expertise describes specific SPV professional practice within the nursing profession. The 2023 version works out, among other things, the social psychiatric context and collaboration. The document helps with questions about the professional role; it is not an automatically completed client record. In a contact report, record what was actually discussed and done, and for professional questions check the current version. [11]
The nursing process must be traceable in the record. The V&VN guideline distinguishes five phases in it, so a single contact note does not make up the whole record. Make sure the report of an outpatient contact fits the goals, care provided and evaluation in the record. For a recovery-oriented goal, what was agreed with the client and what was discussed during the contact should stay visible. [13]
The Landelijk Kwaliteitsstatuut GGZ describes tasks, competencies and conditions around the lead practitioner role. Which role someone can fulfil depends, among other things, on the setting and the applicable conditions. So the job title SPV does not automatically carry every responsibility for the treatment plan. In the documentation, make clear who has which role and which agreements apply within the organisation. [19]
The field standard describes professional groups and quality criteria for work within mental healthcare (geneeskundige ggz) and forensic care. Training and registration, among other things, are taken into account. The document is about professional position and deployability, not about a fixed list of sentences for every SPV report. Check the applicable conditions separately and state in the record which contribution was actually made. [15]
Akwa GGZ supports the development and use of quality standards and associated tools for mental healthcare. That information can guide professional conduct and conversations about quality. It does not replace your judgement in an individual contact. Record the relevant care and agreements; a reference to a standard does not by itself prove that all components have been carried out. [17]
The SPV area of expertise stresses that collecting and providing information go together with attention to privacy. Make clear who data comes from and for which care goal it is needed. Do not automatically take over all personal information from relatives. Before sharing data, check the legal basis, recipient and agreed scope. A complete contact report is not automatically also the appropriate content to share. [11] [12]
An SPV may need to record information for outpatient care and for alignment with the general practitioner (GP) or care chain partners. When systems do not connect, the same information may have to be entered again. The V&VN documentation guideline names this as a bottleneck. Consistent documentation helps keep the content recognisable, but does not by itself solve the technical connection between those systems. So distinguish between the text and the way it is handed over. [13] [11]
Read the text in full and check the person seeking help, source roles, times and agreements. Only use the passage you have assessed and that is needed under the record agreements. Creating a report does not automatically place it in the record or with a care chain partner. Check the current integration and working method separately, and check what was actually transferred. [10] [9]
Check whether the report distinguishes between what the client says, information from a relative and your own observations. During a home visit or multidisciplinary meeting, different perspectives can exist side by side; they must not quietly merge into one conclusion. Also read back the agreements made, correct inaccurate wording and approve the text yourself. With voice-driven reporting, that final check is always your step. [9]
A community psychiatric nursing diagnosis is a professional outcome of clinical reasoning by the SPV within their own expertise. That is something other than a conclusion generated by Hysio. State the assessment explicitly if you want to record it, and distinguish it from observations and client reports. Hysio organises your statement, but does not formulate or confirm the diagnosis itself. [11]
No. Care demand classification (zorgvraagtypering) belongs to the professional and administrative agreements that apply to it; the lead practitioner (regiebehandelaar) is responsible for determining it. Hysio must not derive a type from complaints, risk information or a DSM classification. An explicitly provided type can be recorded recognisably with its source. Check rules on registration and data submission separately, including any applicable privacy policy. [16]
The SPV area of expertise describes open, respectful communication that fits the person you are speaking with. Language proficiency, understanding, cultural background and emotional resilience can differ. In the report, use understandable, concrete wording and keep the client's own expressions recognisable. A careful tone also means that an observation does not quietly turn into a judgement about the person. [11] [12]
Use concrete language that fits the understanding and situation of the person seeking help. Avoid unnecessary jargon and explain necessary concepts. Keep clear what was discussed and which agreement was made, without presenting your professional interpretation as a statement by the person seeking help. Also check whether the text is intended for this recipient and which information may be shared. [11] [12]
Describe separately what the person seeking help says, what a relative reports and what you observe. A difference between those sources does not have to disappear. Hysio does not pick a winner and does not derive its own risk assessment. Record your stated assessment and the contribution of other healthcare professionals separately, with the moment and status of any decisions. [4]
Describe the goals and agreements as stated by the person seeking help and the healthcare professionals involved. Where this has been agreed, state who takes on an action and within what timeframe. Hysio does not turn a preference into a joint decision. Also check which content the recipient needs and is permitted to receive; the whole contact report does not automatically need to be included. [5]
The outpatient report describes one conversation or home visit about the request for help, living environment, interventions and follow-up. The crisis report documents a crisis contact that has already been carried out professionally, including timeline and explicit assessment. Network information can appear in both. So choose based on the actual contact and do not use the crisis format only because a concern or risk factor was mentioned. [3] [4]
Only use the document as recognisably provided context and state the version or date when known. Hysio does not assume that an old crisis card is still current or that all steps have been carried out. State which information was confirmed during this contact and which action actually took place. The platform does not fill in missing warning signs or safety agreements. [4]
Compare the passage with the source and correct the error without adding new assumptions. Check whether the change also affects the summary, your assessment or a follow-up agreement. Keep uncertainty visible if a detail has not yet been confirmed. Read the full text again before you use or share it under the record agreements. [9]
Do not fill in a missing word, time or decision because it sounds likely. Verify the information with the right source. In a crisis contact, the difference between an intention and implementation can be important. With voice-driven reporting, you keep uncertainty visible and only adopt a confirmed assessment or agreement. [9]
For the SPV qualification, Vektis lists a BIG registration as a nurse and registration in the V&VN quality register for SPV. An AGB code serves administrative identification in healthcare and must be checked separately from client-related documentation. A good contact report does not prove that the registration requirements have been met. For an application, consult the current Vektis criteria. [20]
Zorginzicht lists the Landelijk Kwaliteitsstatuut GGZ 4.0 as the applicable quality standard. From 1 January 2026, version 3.0 no longer applies. So check which version underlies the agreements of your organisation. An older job description or research note does not determine the current division of roles. The SPV assesses together with the organisation which conditions apply to the specific setting. [19]
As an SPV, you account for continuing professional development in your own portfolio for the Kwaliteitsregister V&V. It concerns your professional development and the activities with which you substantiate it. That portfolio has a different purpose from the client record, in which the care provided is recorded. So a report of a care encounter does not by itself prove that the requirements for continuing professional development have been met. [12] [18]
A proposal stays linked to the person who puts it forward; a decision states what was explicitly agreed. Hysio does not turn several contributions into a consensus without a source. Also keep the next step separate: a decision is not yet an action carried out. At the handover, check which action owner and timeframe were actually named and which points are still open. [5]
Note what you actually carried out and which response the person seeking help reported or you observed. Keep your professional interpretation of this separate. Hysio can organise that information, but does not add a treatment effect or explanation. An intervention that was only discussed remains an intention and must not appear in the report as care provided. [3]
No. The crisis report contains only the explicitly provided assessments and decisions of the healthcare professionals involved. Hysio does not derive risk from behaviour or silence and does not choose an admission or legal route. So state who made a decision and at what moment. Check that a possibility that was discussed is not presented as a final decision or one carried out. [4]
For the handover, provide the relevant, checked information from the crisis contact. Keep current assessment, historical context, decisions and open actions recognisable. The crisis assessment and the handover each have their own purpose. Hysio does not independently determine the minimum content to share or the legal basis. You check the recipient and the agreements before the information is handed over. [4] [5]
State your professional interpretation explicitly and keep it separate from what the person seeking help or a relative says. Hysio must not fill in your judgement when you only provide observations. Conversely, a client report does not become a professional conclusion. When reading back, check whether uncertainty, context and the agreed next step keep the same meaning as during the contact. [3]
For comparable contacts, measure how much time recording, reading back, correcting and transferring take together. Also compare the accuracy of source roles, times, interpretation and agreements. A shorter first version is no gain when important differences between perspectives disappear. Use your own measurement and assess the quality of the full report before you conclude there is a benefit. [9]
First use a few fictional cases and agree who checks the text. Look at the privacy agreements and record-keeping practice before you start. Only expand voice-driven reporting once risk information and the perspectives of relatives come back correctly. AI reporting does not replace agreements on access and data sharing in your organisation. [9] [8]
Hysio creates the report. You remain responsible for what you save in the record.
