Quick answer
Four documentation formats for paediatric care, in which the child, the parents and your own observation each keep their own place.
This is how Hysio works in paediatric nursing. You read every report back before it goes into the child's record.
The profile covers an Intake, Care Encounter Report, Parent Conversation and Handover & Discharge. The intake describes the initial data collection; the care encounter report a single direct care moment. A separate conversation with the child and parents has its own documentation format, as does a shift or discharge handover. Choose the document purpose that truly fits your contact and keep the different people involved clearly apart. With voice-driven reporting, after the contact you describe what the child, the parent and you contributed; Hysio organises it and you check that each perspective is correct. [3] [4] [5] [6]
The intake organises the reason for care, the people involved, information about illness, safety, environment and development, nursing observations and the care plan discussed. Input from the child and from parents stays recognisable. A nursing analysis uses the information provided; any additional consideration is kept separate. Check those parts individually, so that a question for later does not end up in the record as an established finding. [3]
This documentation format describes a single direct care moment: what the child or a parent reports, what you observe, which care was provided and what was agreed. It does not turn the contact into a new intake or a separate parent conversation. Include times and completion status where relevant. Check in particular that a planned procedure is not presented as care already provided. [4]
The Parent Conversation describes a separate conversation with the child and parents or representatives. It sets out their perspectives, the explanation given, questions, concerns and agreements. It is not a daily log or formal handover. Hysio does not infer parental authority or consent from someone's presence. Check who said what and which question or agreement is still open. [5]
This format is intended for a handover between shifts or institutions, or for a handover at discharge to follow-up care. State which of the two purposes applies and who the recipient is. The report organises current care, technical care, instruction and open points. A document that has been drawn up does not prove that it was sent or received and does not automatically form a technically validated eOverdracht message. [6]
The Paediatric Nursing Intake fits an initial care encounter in which the development, functioning and coping capacity of the child and family are gathered. You organise what parents contribute, what you observe yourself and which next step was agreed. Missing information stays open and the paediatric nurse determines what is suitable for the record. [3]
Say what was actually done and distinguish it from a prescription, request or plan. For medication, keep the drug, dose, unit, route and time as far as they were provided. Hysio does not fill in any missing value and does not give medication advice. Check the result against the original information before you record an administration or procedure in the record. [4]
State who expresses a concern, wish or question. The child has their own perspective; statements by parents are not substituted for it. Differing accounts from parents also stay separate. Hysio must not turn separate contributions into joint agreement. Check whether the report reflects the conversation actually held, including disagreement or an unanswered question. [5]
For paediatric nursing, Starter costs €20 per month, Pro €40 and Unlimited €80, excl. VAT. If you choose annual billing, you pay two months less. The price does not differ per care moment or documentation format. The paediatric nurse assesses the source, observation and agreements before reviewed text goes into the record in line with local arrangements. [7]
Follow the privacy arrangements of the care organisation and decide which information is needed for the care encounter. Keep statements by the child and by a parent clearly apart. Check who is authorised to receive information or make decisions about data sharing; presence at the conversation is not enough for that. Hysio's Privacy Policy describes how the processing roles and data use are arranged. [9]
Nurse is a protected profession under article 3 of the Wet BIG (Dutch Individual Healthcare Professions Act). Registration and the conditions for authorised practice are separate from the quality of a written report. Hysio does not confirm the writer's registration or competence. Check professional requirements separately and describe in the record only the care, observations and agreements that actually belong to the contact concerned. [26]
No. Reserved procedures require the applicable statutory authority, a valid instruction where one is required and competence for the specific procedure. Supervision and intervention must be arranged where needed. The title paediatric nurse does not give a general exemption. A report describes what happened and does not retroactively put right a missing instruction or authority. [14]
The CZO training plan for paediatric nursing in extramural care includes a professional activity around observing, monitoring and supporting vital functions in a critically ill child who is responding sufficiently to the therapy provided. This describes part of the training. It is not a standalone treatment protocol and not a general statement that every paediatric nurse can provide all forms of acute care. [25]
In a nursing day care centre for children, daily childcare and nursing care come together. Coordination with the child, parents and others involved requires clear information about the care plan and how the day went. In the handover, make visible what you observed yourself, what a parent told you and which agreements apply to the next care moment. [19]
Paediatric nursing has its own area of expertise and CZO training routes, with attention to development and the context of the child and family. That goes beyond a different name above a general nursing report. For record keeping, the child's input, the role of parents and your own nursing observations must stay distinguishable. Check which expertise the specific care moment requires. [19] [25]
No. That an instruction was given does not automatically mean that a parent understood it or can carry it out independently. Record separately what was explained, what the parent repeated back or demonstrated and what is still open. Hysio must not infer understanding, agreement or competence from silence. Check those differences when you describe instruction in a handover. [6]
State the current care situation, relevant values and times, ongoing care, instructions and open agreements that were actually provided. Mention who receives the information and who picks up a follow-up action, as far as this has been agreed. Hysio does not add a new care instruction. Check that older information is not presented as current and that actions not yet carried out stay recognisably open. [6]
An intake gathers the initial nursing information about the child and family and the care plan discussed. The Parent Conversation records a single separate conversation, with perspectives, explanations, concerns and agreements. The presence of parents does not turn every intake into a Parent Conversation. Choose based on the purpose of the contact and check that the text does not add intake components that were not discussed. [3] [5]
When you write yourself, you phrase the note straight away. When you dictate, you first say what the child, a parent and you yourself contributed; after that you assess the organised text. Compare whether the three perspectives stay recognisable and whether care and agreements are reflected correctly. Include the time for corrections when you assess which way of working best supports your paediatric care encounter. [10]
The trial lasts two weeks. With a made-up conversation between child, parent and nurse, you can see whether the different perspectives stay clear and agreements are presented correctly. Also try a short care encounter report with medication. You can use all four documentation formats for paediatric nursing during the trial. [8]
For children younger than twelve, parents with parental authority or the guardian may view the medical record. From twelve to sixteen, the child's consent is needed, except for information parents need to help decide on a treatment. From sixteen, the young person in principle decides on this themselves. Check which situation applies; presence at a conversation is not general consent to access. [12]
A calamity is an unintended or unexpected event that relates to the quality of care and has led to death or seriously harmful consequences. The care provider reports an established calamity to the IGJ within three working days. An unfavourable outcome is therefore not automatically a calamity. The assessment and reporting procedure sit alongside the factual documentation of the care encounter. [18]
The Richtlijn Verpleegkundige en Verzorgende Verslaglegging (guideline on nursing and care documentation) from V&VN applies across the profession and was not written solely for paediatric nursing. The version consulted was authorised in March 2022. Use that basis together with the context of the child and family: a general documentation requirement does not automatically tell you which information is needed in a specific paediatric care encounter. That requires the assessment of the paediatric nurse. [20]
V&VN recommends an electronic care record and unambiguous language for data exchange. In addition, attention needs to be paid to an easy-to-use system and a report format that fits the work context. The care organisation makes choices about set-up and funding; nurses can advise on these. Using a digital record does not in itself make a report complete or correct in content. [20]
The set-up of a record and the nature of the contact can differ. A short progress note therefore does not need to have the same form as an intake or handover. Use your organisation's arrangements and keep relevant observations, care provided and follow-up recognisable. A fixed layout only helps when it clearly reflects the information actually available. [20] [21]
No. This intake organises the initial data collection and the care plan discussed, but does not establish a separate needs assessment or funding route. The paediatric nurse assesses the content in line with the applicable professional standards. A nursing analysis or additional question in the text is not a needs assessment decision. Check which assessment actually took place and do not adopt an intention as a final allocation. [3]
No. In this intake, the DD JGZ remains a separate record. A mention that the child health clinic is monitoring something does not give Hysio access to that information and is not a reason to fill in JGZ findings. Describe only the context provided, with its source. Check data exchange and access rights separately before information from another record is used. [3]
These terms are not interchangeable. When assessing a possible calamity, the quality of care and the severity of the consequences are among the factors that matter. First describe concretely what happened and follow the organisation's reporting and investigation route. A term in a care report must not quietly anticipate that assessment or replace the required report. [18]
First assess the text for the correct child, the contact moment and the separate contributions of parents. After that, use only the reviewed content in line with your organisation's record arrangements. Check the current integration options and do not assume automatic saving. After transferring, check that the correct note is attached to the correct contact and that no family information has been included unintentionally. [11] [10]
Review the report for the source of every statement: the child, a parent or your own observation. Check whether functioning, coping capacity and agreed next steps are presented correctly, without mixing up information from family members. Correct the text and approve it yourself before you use the report. A missing observation stays open until you can substantiate it yourself. [10]
Medische Kindzorg Samenwerking is a way of working for assessing needs for, organising and providing nursing care to sick children outside the hospital. It gives a coherent picture of the medical situation, safety, environment and development. The aim is appropriate, coherent paediatric care. A separate contact report supports the transfer of information, but does not replace the joint assessment, organisation and delivery of that care. [15]
Care for a child in their own environment also affects the parents and the daily family situation. That context helps to understand the care needs and possibilities. It does not mean that all family members have one voice or the same record. Record relevant contributions separately and keep visible what the child indicates, what parents report and what you observe. [15] [22] [23]
An independently authorised professional can have a reserved procedure carried out by someone who is competent to do so. The person giving the instruction must assess that competence and, where needed, give directions and arrange supervision or intervention. The person receiving the instruction follows the instruction and the directions given. An instruction and the actual performance are different facts; do not record them in the report as one self-evident event. [14]
No. The care encounter report describes the direct care moment. A handover focuses on a recipient and on current care, responsibilities and open points at the transition. The contact note can contain relevant information for this, but does not prove a handover. Check which information still applies, what was actually shared and which next steps the recipient has confirmed. [4] [6]
Note which explanation you actually gave and which questions the child or the parents asked afterwards. Keep an answered question separate from a point that still needs to be checked. Hysio does not independently provide the missing explanation. Also check who picks up the follow-up question, so that the report contains no commitment or reassurance that was not expressed. [5]
Describe the care, equipment and medication facts provided, with the values, times and completion status mentioned. Alongside that, set out which instructions were given to the family and what still needs to be explained or checked. Hysio does not create its own home care plan. Check in particular that a planned delivery or instruction is not presented as already arranged, received or understood. [6]
The data collection has separate domains for illness, safety, environment and development. Only the relevant information that was provided goes into them, with the source preserved. Hysio does not calculate a developmental score and does not establish safety on the basis of missing reports. Check that statements about functioning and coping capacity stay with the correct person and period. [3]
No. Hysio does not add missing measurement data, scores or its own clinical explanation. The care encounter report keeps what was actually reported, measured and professionally assessed, with time and source. A single value does not prove improvement. Check the coherence with the care moment concerned and keep a parent's impression separate from a nursing measurement or conclusion. [4]
Go back to the original paediatric care information and determine which passage deviates. A wrongly attributed statement, for example, requires correcting the speaker, without inventing content. Then also check the summary and agreements. For a record note that has already been saved, follow your organisation's correction procedure, so that it stays clear which information was changed and on what basis. [10]
Try to trace a poorly audible passage or check who said what. Do not turn an answer that presumably came from a parent into a statement by the child and do not fill in a missing dose yourself. When AI handles the admin, the uncertainty stays visible until the source has been clarified. Then read the adjusted text again in the context of the care moment. [10]
No. Paediatric care is usually funded under the Zorgverzekeringswet (Health Insurance Act), but depending on the care need the Jeugdwet (Youth Act) or Wlz (Long-term Care Act) may also be relevant. Zorginstituut Nederland describes the boundaries and the conditions. A report or job title does not determine the funding. Check the applicable needs assessment and arrangements with the responsible party; Hysio does not choose a funding route or billing code for the child. [15]
The Handreiking Indicatieproces Kindzorg supports paediatric nurses in assessing and organising the care a child needs. V&VN describes the document as a professional standard and aid. Use the guidance for the needs assessment process and its substantiation. A representation of the conversation is not automatically a needs assessment decision and does not in itself prove entitlement to funding. [23]
For care that falls under the Wkkgz, the report must be made within three working days of it being established that it is a calamity. If there is doubt about the classification, there are six weeks for investigation from when the event is identified. As soon as a calamity has been established, the reporting deadline applies. The care provider follows the IGJ route for this; a note in the child's record does not replace that report. [18]
A parent conversation describes what the child and parents ask, which explanation was given and what was agreed. A discharge handover passes current care and open points on to follow-up care. A conversation about leaving does not mean that discharge has already been decided or that the handover has been received. The contact purpose determines the appropriate report structure; check the status of every agreement. [5] [6]
No. Preparing, offering, sending, receiving and accepting are different steps. The documentation format records only which status was actually mentioned. Hysio must not assume that follow-up care has received the handover or taken responsibility. When using the text, check which party has confirmed something and which feedback is still needed; the document does not carry out the handover itself. [6]
The intake analysis connects the information provided and nursing findings. Any additional consideration sits in a separate section and is not an established fact or agreed care plan. It requires assessment by the paediatric nurse. So check not only the wording but also the origin and status before you use a passage or discuss a next step. [3]
Their reports stay side by side, with a recognisable source attribution. Hysio does not choose which account is correct and does not turn the parent's report into your observation. In addition, record what you saw or checked yourself. Check the report for unintentionally merged voices, so that the uncertainty or the difference does not disappear from the text. [4]
No. Presence does not prove parental authority or consent to a specific instance of data sharing. The documentation format keeps only what was actually discussed about this and by whom. Hysio does not determine who holds parental authority and does not carry out a consent check. Check the applicable situation separately and prevent something a parent reports from appearing in the report as an authority you have established. [5]
Compare similar paediatric care encounters, with roughly the same amount of parental input and nursing information. Measure both the recording and the checking and transferring of the text. Pay attention to errors in speaker, care action and follow-up agreement. That way you assess the usefulness in your work, without deriving a fixed saving for every contact from a short practice case or a general sector figure. [10]
First use a made-up paediatric care conversation with clearly different contributions from the child and parents. Discuss who checks the text and which information may be processed according to your organisation. Review together whether the output keeps care, explanation and consent properly apart. Only expand AI reporting once that way of working runs well in the team. [10] [9]
Hysio creates the report. You remain responsible for what you save in the record.
