Quick answer

Documentation for cardiology

A profession-specific structure for cardiology, with room for the reason for visit, history, investigations with their source and date, your conclusion and the management plan discussed.

This is how Hysio works in an outpatient consultation, a results discussion or a short note after a phone call or ward round.

How does voice-driven reporting work for cardiologists?
In an outpatient consultation or results discussion, voice-driven reporting works like an AI scribe: you keep your attention on the conversation, Hysio writes the report. This is also called AI documentation. For cardiology there are four documentation formats: Cardiology Consultation, Cardiology Intake, Cardiology Results and Follow-up Consultation, and Cardiology SOAP Note. [2] [3] [4] [5]
What stays tied to the source?
Cardiac history, supplied results and the patient's own account each stay separate. Your assessment and the agreements are only included when you say them. [1] [11]
Who determines the content?
You, as the cardiologist. You review the ECG and echo findings and the management plan before the report goes into the record. [1] [9]
How does the report follow the work in the cardiology clinic?
The structure follows cardiac symptoms, investigations such as ECG and echo, and the follow-up discussed. ECG and echo findings appear as you state them, and the management plan stays yours. [1] [11] [10]
What does Hysio not do?
Hysio does not read an ECG or echo and does not make a diagnosis. It does not choose a treatment and does not take any decision in acute chest pain. [9] [10]

Voice-driven reporting in cardiology

Which documentation format fits a consultation in which you discuss an ECG result? #

If you are discussing a result that has already been assessed, the results and follow-up consultation fits; if it is a first extensive history-taking, the intake fits. In every format, the result itself goes under additional investigations, with the type of investigation, the date and the role of whoever assessed it. The ECG conclusion keeps its own label there. Only what you as the cardiologist state goes into the conclusion section. [2] [3] [4] [5] [1] [11]

How do you organise heart rhythm and blood pressure in the record? #

Measurements get a fixed place and keep their origin. What you establish during the physical examination, such as blood pressure and pulse, sits with the examination; a rhythm from an ECG or Holter stays with that investigation, with date and source role. Hysio reproduces number, unit, posture and time of measurement verbatim and does not convert anything. Your interpretation of those values stays separate from the value itself, so it remains clear later who established what. [2] [1] [10]

How do you document a referral to cardiac rehabilitation? #

Cardiac rehabilitation only appears in the report when the encounter was actually about it. The discussion with the patient sits with the history, the indication you state under the plan, along with who takes up the follow-up. An ordinary cardiology intake does not automatically become a cardiac rehabilitation intake: without source content, that topic does not appear. Hysio does not fill in a participation decision, programme or goal and does not take over any indication decision. [10] [1] [2]

Which information do you deliberately keep out of a cardiology report? #

The report carries no letterhead, addressing, salutation or signature and no identifying fields: no BSN, patient number, document number, BIG number or name of a practitioner. You limit the text to what this encounter needs. You note information about third parties, for example in the family history, with their role and no more extensively than the care requires. What you share with a relative or another healthcare professional, and on what legal basis, you record separately. [8] [11] [1]

What does voice-driven reporting cost if you hold many results discussions? #

The rate does not depend on your specialty. Hysio has three plans: Starter for €20, Pro for €40 and Unlimited for €80 per month, excl. VAT. With yearly billing, you get two months free. There is no separate rate per profession and no surcharge for cardiology documentation formats. The current amounts and the fourteen-day free trial are on the central pricing page. [6] [7]

When is your own template a better fit than a Hysio documentation format? #

Your own template fits when your hospital prescribes fixed headings, authorisation rules or structured fields that carry more weight; Hysio does not fill in coding and registration fields in any case. A Hysio documentation format fits when you want running text from a spoken source in which source roles, investigation dates and your conclusion stay recognisably apart. Compare the two on two points: whether your department reads the headings easily, and whether every line stays traceable to whoever supplied it. [1] [10] [11]

How do you check a cardiology report against the source? #

Read the report next to what you dictated and check four things. Are number, unit, date and side exactly right? Does the conclusion of an ECG, echo or Holter still sit with that investigation and not with your conclusion? Is an investigation that was only requested or announced not shown as a result? If a section is missing from the source, a fixed sentence should be there instead of an addition. A fixed format does not make a report correct on its own. [1] [11] [9]

How do you keep your attention on the patient during the conversation? #

You do not need to type along during the encounter: afterwards you summarise what was said and done. What the patient tells you about symptoms, exercise tolerance, concerns and expectations stays recognisable as their own report and is not merged with your finding. That way the record shows which part of the story comes from the patient. Before the text goes into the record, you read it back yourself. [1] [2] [10]

What sets a Hysio documentation format apart from an empty template? #

An empty template gives you headings; a documentation format organises the supplied source. Every line keeps its origin, an investigation conclusion stays separate from your conclusion, and a section the source says nothing about gets a fixed sentence instead of an assumption. That difference matters most in an encounter with several results and several people telling the story. Assessment, diagnosis and management plan stay with you, unchanged. [1] [2] [10]

As a cardiologist, how do you get started with voice-driven reporting in the outpatient clinic? #

Start small: take a contact type that comes up often, for example the regular outpatient consultation, and leave the other encounters as they are. Agree in advance who reads a report before it goes into the record and which information does not belong in it. Over a number of encounters, check whether the headings match what your department reads and what you had to correct. Only then do you add a second contact type. [7] [8] [1]

Documentation formats for cardiologists

For which contact in the cardiology clinic do you use the Cardiology Consultation? #

For the regular cardiology encounter in the outpatient clinic, by telephone or digitally. The format opens with a summary and then keeps three layers apart: the source information with reason for visit, referral, medical history and history of the presenting complaint; the findings with physical examination, additional investigations and your conclusion; and the follow-up with the management plan discussed. The part you copy into a letter runs from reason for visit to plan, without addressing or signature. [2] [1] [11]

What do you record in the Cardiology Intake? #

For the extensive first history-taking. The format records the referral question, the relevant medical history, the symptoms, the family and social context, the patient's expectations, the examination and the source-bound follow-up. Blocks on active diagnoses, social history and expectations disappear, heading and all, as soon as the source says nothing about them. Open questions and prompts from Hysio sit in a separate advisory section and therefore outside the part you copy into a letter. [3] [1] [11]

Which format fits a conversation about a new result? #

For the follow-up encounter in which you discuss a result that has already been assessed and adjust the management plan. The course since the previous encounter and the current symptoms go under the history of the presenting complaint; there is no separate heading for the course. The conclusion of an ECG, echo, Holter or exercise test stays under additional investigations with type of investigation, date and source role. An announced result, or one not yet authorised, does not appear as a result. [4] [1] [11]

When do you write a Cardiology SOAP Note? #

For your own progress note after a short follow-up contact, a telephone contact or a ward round during admission. The format places a summary above four parts: Subjective, Objective, Assessment and Plan. That layout is common for such a short note and is not a prescribed cardiology format. The letter to the referring clinician follows the other three formats. [5] [1] [11]

Which format fits which cardiology encounter? #

The choice follows the encounter, not the topic. A first extensive history-taking calls for the intake. A regular outpatient, telephone or digital contact calls for the consultation. A discussion of a result that has already been assessed calls for the results and follow-up consultation. A short note after a telephone contact or a ward round calls for the SOAP note. If you are unsure between two formats, choose the one whose headings carry this encounter; the text stays tied to the source in all four. [1] [2] [11]

When do you choose an intake and when a follow-up report? #

Choose the intake when the starting point still needs to be recorded: referral question, medical history, social context and expectations. Choose a follow-up report when that starting point is already in place and only the change matters: the course since the previous encounter, new results and the adjusted management plan. If you take over a patient from a colleague or it concerns a second opinion, you record the starting point again, even if a record already exists. [1] [2] [11]

How do you document a procedure such as a catheterisation? #

Hysio does not create a stand-alone procedure report. What was said in the encounter, the reason, what was done and the finding as you name it, goes under additional investigations with date and source role. The formal intervention report remains a separate document in your own system. Material, size, side or complication only appear when you name them; Hysio does not infer anything and does not fill in any registration field. [1] [2] [11]

Which information belongs in a message to the referring clinician? #

The core is short: the reason for the message, your conclusion, the agreed management plan and what you recommend to the referring clinician for follow-up. You add history, course, examination and medication to the extent the recipient needs them to continue. The letter section runs from reason for visit to plan and contains no letterhead, addressing or signature. Whether this message may be sent, and to whom, you judge yourself. [1] [2] [11]

How do you separate what the patient reports from what you establish? #

Three layers stay apart: what the patient or a relative reports, what a nurse or colleague observed, and what you established yourself during physical examination or additional investigations. For each section it stays visible who supplied it. As a result, a reported symptom does not become a finding and a finding does not become a conclusion. That separation keeps admin with AI verifiable, especially when several people spoke during the consultation. [1] [2] [11]

How do you describe change between two encounters? #

Only when source, method and time of measurement are comparable, or when you state the comparison. Hysio does not calculate a difference, percentage, class or trend; two values stay two values with their own date and origin. If you name an improvement or a deterioration, it appears in the report as your statement. That way it remains clear later whether the change was measured or assessed by you. [1] [2] [11]

Recording ECG, echo and heart rhythm

How do you describe the symptoms in the history of the presenting complaint? #

In the patient's words and as concretely as possible: the nature and location of the symptom, what triggers it, how long it lasts, which phenomena accompany it and what the patient says about their own exercise tolerance. If you state a functional class, it is recorded verbatim; Hysio never determines such a class itself and does not derive it from an account of symptoms. What was not discussed stays empty with a fixed sentence. [1] [11]

How do you record who supplied information? #

Every supplied line gets its origin: the patient, a relative, the referring clinician, a nurse or an earlier report. That way it stays visible that a family history comes from the patient and a night-time rhythm observation from the ward. A supplied investigation report is also a source and keeps its own date and assessor. What you observed yourself stays separate from that, so that no one later mistakes your observation for someone else's statement. [1] [11]

How do you note a measurement with its time of measurement? #

Value, unit, method and time belong together. A blood pressure without posture or time says little, and neither does an ejection fraction without an investigation date. Hysio reproduces number and unit verbatim, rounds nothing, converts nothing and does not fill in a missing unit; what the source does not support stays visibly open. What the value means for this patient is your judgement and sits in your conclusion. [1] [11]

How do you link an investigation result to the investigation? #

The result stays with its investigation: which type, which date, who assessed it and the conclusion as it was stated. That investigation conclusion keeps its own label and is not copied into your conclusion, not even when both say the same thing. Distinguish between requested, planned, performed, assessed and authorised investigations. Only an investigation with a result that was actually stated carries a result. [1] [11]

Does Hysio assess an ECG, echo or raw measurement data? #

No. Hysio reads no echo image, no ECG recording, no Holter and no scan, and does not interpret raw data. What goes into the report is the investigation report that has already been assessed or the conclusion that was stated, with source role and date. A symptom pattern or a single measurement does not in itself prove a cardiology conclusion. You state that conclusion, or it is not there. [10] [9] [11]

How does Hysio handle cardiology abbreviations? #

Abbreviations stay exactly as you say them, whether known or unknown. An abbreviation not being recognised is no reason to write it out or replace it. Language from another field does not belong: the fixed headings of general practice, an allied health scheme or a nursing classification framework do not go into a cardiology report. That way the text stays readable for everyone in your department who reads along. [1] [11]

How do you note information from a relative? #

With their role: partner, child or informal carer, and exactly what that person reported. For a referring clinician, you note the referral question as it was asked, not as you would summarise it. Information about a third party stays limited to what this care needs; a family history does not need to become a record of a family member. If you want to share something with a relative, that requires the patient's consent and you record it. [1] [11]

What does the heart team need from your report? #

The question at hand, the findings with their investigation date, your conclusion and what has already been discussed with the patient. Because it stays visible for each section who supplied it, the discussion does not have to rest on an assumption. An outcome of the discussion is only a decision once it has been stated and recorded; Hysio does not turn a consideration into a decision and does not fill in a missing participant or date. [1] [2] [11]

What does a report of a short follow-up contact look like? #

Take a check-up after an earlier admission. The patient tells you how things are going and what they notice on exertion; you note your own findings, mention the result that has already been assessed with its date, state your evaluation and record the management plan with who does what. With voice-driven admin, sections that were not discussed simply stay empty. No diagnosis appears that you did not state and no appointment that was not made. [1] [11] [2]

How do you keep a handover tied to the source? #

Write only what was said in the encounter or is in a supplied report, each time with the role. Keep the text to what the recipient needs to continue; a full copy of the record is not a handover. Keep your own conclusion recognisable as your conclusion, separate from the conclusion of an investigation. Name what is missing as missing, instead of filling it in based on what is likely. [10] [8] [11]

Sources, privacy and boundaries in cardiac care

Does Hysio make a cardiology diagnosis? #

No. Hysio organises what was said and records what you established and decided. A rhythm classification, a severity grade, an indication or a diagnosis only appears when you state it. If your conclusion is missing from the source, a fixed sentence stays there saying that no conclusion was stated, and the conclusion of an investigation is not put in its place. You determine the meaning of the findings. [9] [1] [11]

Who is responsible for the saved report? #

You. You read the report through, correct what deviates from the source and decide what goes into the record. The legal record-keeping obligation and the retention period lie with the practitioner and their institution, not with a tool. Hysio does not take over any professional decision, does not monitor any time limit and authorises nothing. What ultimately sits in the record is the text you approved. [1] [9] [11]

What happens if the source does not support a section? #

Then that section stays empty, with a fixed sentence saying it is not in the supplied source. No date, value or conclusion is made up and nothing is inferred from the context of the conversation. If the source as a whole is too thin for a report, a single refusal sentence follows instead of a half-completed document. You add information yourself, from what you actually saw, heard and established. [1] [9] [11]

Can you use Hysio in an acute situation? #

Not for the acute assessment itself. Hysio does not triage, does not determine urgency and does not flag any alarm symptom. With chest pain, an arrhythmia or an unstable patient, you act first according to your own assessment and the agreements of your ward. Only then do you record what happened, what you did and what you decided. Documentation comes after care, not before it. [9] [8] [11]

Who may receive the cardiology report? #

Within the treatment relationship, those involved in the care read along; outside it, you share only with the patient's consent and for a defined purpose. The referring clinician receives what is needed to continue the care, no more. Record what was agreed, with whom and for what, and note an objection as carefully as a consent. Hysio does not send anything itself and does not assess any legal basis for sharing. [8] [11] [1]

Does Hysio propose a treatment plan or medication plan? #

No. Hysio does not choose a treatment, procedure or medicine and does not propose a dose adjustment. A medication fact only appears as it was stated in the encounter, and a medication plan only as your plan. No medication list is built and no name, strength or dosage is smoothed over. Separate blocks for active medication, allergies or side effects do not belong in these documentation formats. [9] [10] [11]

How do you record doubt or an open question? #

As doubt. If you say something is probable, or still unclear, it stays that way in the text; a statement is not made firmer, nor more cautious, than the source supports. You note an open question, an investigation still to be assessed or an appointment still to follow as open, with who takes it up. That way a colleague later reads what had been established and what had not yet. [1] [11] [9]

What do you note when two sources contradict each other? #

Both readings stay, each with its origin and its moment. The patient may report something different from the referral letter, and an earlier result may differ from the current one. Hysio does not choose between two sources, does not average them and does not leave either out to make the text fit. You weigh the sources against each other and put that judgement in your own conclusion, so it stays visible what your decision rests on. [1] [11]

How do medication and measurements end up in the report? #

Literally. Name, strength, dosage, unit, number and time of measurement stay exactly as you say them, and the same applies to the side, left or right. Nothing is converted, rounded or pulled towards a common spelling. Coding, billing and registration fields stay completely outside the documentation format. What you did not state is not added; a missing dosage is not filled in out of habit. [1] [11] [9]

What do you check before you save the report? #

Go through the sections. Is the reason for visit there as discussed? Do numbers, units, dates and side match your dictation? Does the conclusion of an investigation still sit with that investigation? Is the management plan the plan you stated, with the right owner and time frame? Remove what does not belong in this record and add what the source missed. Only when that is correct do you transfer the text into your record system. [1] [8] [9] [11]

Implementation and day-to-day use

How do you prepare a handover for the on-call cardiologist? #

Put the question first, then the findings with their date and source, then what was agreed and what is still open. Name who takes up the follow-up and within what time frame, as far as that was stated. Agree within your department who reads the text before a colleague continues with it. Keep it to what the recipient needs; a shift handover is something other than a full record. [10] [8] [11] [2]

Which agreements on ECG and echo reports do you make within your department? #

Record four things: which documentation format belongs with which contact type, who reads a report before it goes into the record, how you write recurring terms and abbreviations, and which information stays out of the report. Also agree who authorises and what happens if a result has not yet been assessed. Without such agreements, the text varies from colleague to colleague and the record becomes harder to read. [1] [2] [10]

How do you start small, with one clinic session in cardiology? #

A defined assessment of the report structure covers one contact type and a defined period, with two colleagues checking the same structure against real encounters. Collect what you had to correct and why: was a heading missing, was a result in the wrong place, did a term differ from your writing conventions. Discuss that list with the department, adjust the agreements and only then add a second contact type. [7] [8] [1]

How do you align the report with your hospital's standard headings? #

Place the sections of the documentation format next to the headings your record system expects and agree where each section goes. If the order or naming differs, the local agreement applies: the report is a working text that you transfer yourself, not an input that controls the system. Authorisation, access rights and retention period follow your institution's policy. You fill in structured fields and codes in your own system. [10] [1] [11]

What does AI reporting give you after a busy clinic day? #

After clinic you no longer have to type up notes from memory. That is voice-driven reporting: after each consultation you say what you heard, saw and agreed, and Hysio puts it into the chosen documentation format. Echo result, ECG and a referral to cardiac rehabilitation each get their own place. How much time it saves is best measured within your own department. [1] [11] [2]

How do you measure whether cardiology clinic reports are finished sooner? #

Measure it in your own practice. Note how many minutes you now spend on an outpatient message, how many corrections a report needs and how many encounters you have per half-day, and compare the same contact types over a number of weeks. Include the review afterwards, because it is part of the work. A national percentage says nothing about your outpatient clinic; only your own measurement and the quality of the report give the answer. [1] [10] [7]

What do you assess during the trial period for results discussions and check-ups? #

Fourteen days are enough to see whether the sections fit your encounters, whether numbers, units and dates come back verbatim, and how much you have to correct before a text can go into the record. Assess that on real encounters that you read through yourself, not on an invented case. The terms of the free trial are set out centrally on the pricing page and are not different per specialty. [7] [6] [8]

Where are the pricing and terms, separate from the agreements in your heart centre? #

On the central pages: the plans on the pricing page, processing and retention in the privacy policy, the boundaries of the platform in the disclaimer and the contract terms in the legal information. Those pages apply to all professions and are not a cardiology standard. Nor do they replace your hospital's policy: the data processing agreements and the information security policy of your institution still take precedence. [6] [8] [9]

How do you record how cardiology abbreviations are written? #

Make a short list of the terms and abbreviations you use and the spelling that belongs in the record. Agree what you write out in full, what you abbreviate and how you label an investigation conclusion, so that it does not merge with your conclusion. Share that list with new colleagues and with junior doctors. Consistent language makes a record readable for everyone who has to continue with it later. [1] [10] [11]

How do you keep track of a case with several heart problems at once? #

Split the encounter into three questions: what was supplied, what did you establish yourself and what did you decide. With several conditions, you keep the finding and the management plan together per problem, so that the follow-up stays readable per problem. Results from different dates stay with their own investigation. Do not add an overarching conclusion that you did not state in the encounter. [1] [11] [2] [9]

Hysio creates the report. You remain responsible for what you save in the record.
