Quick answer
Hysio organises insurance medicine assessments into assignment, sources, examination, functional interpretation, conclusion and caveats.
Consultation, intake, reassessment and report dictation each have their own format, with the sources you use for them. Hysio does not set an FML, CBBS profile, percentage of work incapacity, claim decision, diagnosis, prognosis or medical conclusion by itself.
The insurance physician examines the health and functioning of the person concerned in relation to the question at hand. The assessment is based on the conversation, examination and relevant sources. Hysio can organise that information into a traceable report, but does not take over any medical judgement. The insurance physician remains ultimately responsible for interpretation, conclusion and reporting. With voice-driven reporting, you dictate your findings after the consultation, so your socio-medical reasoning stays recognisable in the report. [1] [6] [5]
A report may be needed for an assessment, reassessment, expert assessment or internal reporting. What is relevant depends on the assignment, the chosen reference date, the data category and who may receive the report. Hysio makes that context explicit before facts and findings are brought together. The insurance physician checks whether the report fits the assignment and the record framework. [1] [6] [3]
Within the assessment assignment, the insurance physician carries out an examination and gives a socio-medical judgement; that is something other than a treatment relationship. A report should not confuse that role with treatment by a GP or specialist. Hysio can name the source and the purpose of the contact. The insurance physician decides which medical interpretation belongs in the report. [1] [6] [4]
The Functionele Mogelijkhedenlijst (functional capabilities list) describes functional capabilities in fixed sections when the insurance physician uses that method for the assignment. The list is a professional assessment, not a text that Hysio can complete independently. Hysio can organise spoken components or explanations. The insurance physician checks the content, substantiation and date of every assessment. [2] [6] [4]
The FML records functional capabilities; the CBBS is a separate system for jobs and job demands in certain assessments. They should not be treated as one automatic document. Hysio can include a source-bound reference to an FML or job analysis, but does not fill in any section, match, score or percentage. The authorised healthcare professional remains responsible. [2] [6] [4]
An insurance physician can take a wide range of sources into account, such as the person's own statement, examination results, medical correspondence, previous assessments and other relevant documents. Each source is given a recognisable origin and date. Hysio can organise the inventory and differences between sources, but does not determine which source is decisive. The insurance physician assesses reliability, relevance and meaning for the question at hand. [2] [6]
A reassessment compares a current situation with a previous assessment at set moments. Without a clear reference date, it is hard to see which change the report describes. Hysio can place the reason, previous assessment, new sources and comparison side by side. The insurance physician decides which date is medically and procedurally relevant to the conclusion. [2] [6]
The insurance physician draws the conclusion on the basis of their own examination, relevant sources and professional reasoning. Hysio can place the conclusion under a recognisable heading and answer sub-questions clearly. The platform does not generate a final conclusion from isolated complaints or measurements. The insurance physician checks wording, boundaries, uncertainties and report status. [5] [1] [4] [6]
State which information is missing, which sources contradict each other and what effect that has on the answer. Do not claim certainty that does not follow from the assessment. Hysio can show uncertainties and caveats separately. The insurance physician decides whether additional examination, a new source or an explicit caveat is needed. [2] [6] [5] [1]
An assessment for social security, private insurance, an employer or an expert assessment assignment, for example, can have a different purpose, framework and recipient. That context must not be mixed up unnoticed. Hysio asks for the assignment and addressee in the structure. The insurance physician checks whether the terms and conclusion used belong to exactly this context. [1] [6] [3] [4]
This format suits a clinic consultation in which the assignment, sources, reported information, findings, reasoning, conclusion and open points are recorded. The report shows which part of the text comes from the statement or the examination and which part is the professional assessment. The insurance physician checks the conclusion and the draft status before the document is used. [7] [5] [1]
Hysio Insurance Medicine Intake & Examination organises the assignment, history, socio-medical problem analysis, work and participation context, examination, analysis and open points. That suits an initial or in-depth examination. Hysio does not add to the medical history or functional assessment. The insurance physician decides which information is relevant and validates the professional interpretation. [8] [5] [2] [6]
The reassessment format puts the reason and reference dates, previous assessment, new sources, current findings and the comparison in one traceable order. Reasoning, conclusion and uncertainties follow. Hysio does not present a change as plausible when the sources do not show it. The insurance physician decides which differences are relevant to the current assessment. [9] [5] [2] [6]
Use report dictation when the insurance physician dictates a formal report with assignment, source inventory, examination, functional interpretation, partial conclusions and caveats. The format also keeps track of data category, permitted recipient and draft status. Hysio does not turn it into a report of its own. The insurance physician remains responsible for the dictated content and the final authorisation. [10] [5] [3] [6]
Choose a consultation report for the direct consultation and a dictated report for a fully worked-out report with assignment and sub-questions. For an intake or reassessment, the dedicated documentation format is recognisable by its timeline and source structure. Hysio helps with organising, not with the legal qualification of the assignment. Always check the addressee and draft status. [7] [5] [10] [1]
The intake sets out the main question, history, socio-medical problems, work and participation, examination and analysis separately. This allows the insurance physician to see which information was told by the person concerned and which findings come from their own examination. Hysio does not fill in missing complaints or capabilities. Open points remain available for follow-up examination. [8] [5] [2] [6]
Both are needed, but they have a different function. The previous assessment forms the reference, and new sources describe the current situation on the relevant reference date. Hysio can make the comparison visible without itself concluding an improvement or deterioration. The insurance physician weighs the differences and decides what consequences they have for the assignment. [9] [5] [2] [6] [4]
First record the commission, recipient, method and inventory of sources. Then organise facts, your own findings, functional interpretation, conclusion, advice and uncertainties in line with the report dictation. Hysio does not turn a stated conclusion into wording of its own with extra meaning. The insurance physician compares the write-up with the dictation and approves the report version. Voice-driven admin saves retyping, but the report version stays yours. [10] [5] [2] [6] [1]
For each source, Hysio places the origin, the date and the nature of the document in the source inventory and does not weigh which source counts more heavily. The insurance physician assesses reliability and meaning. A contradiction remains visible until the healthcare professional has discussed it. [2] [6]
Check the assignment, reference date, identity, sources, examination, functional interpretation, sub-questions, conclusion, caveats, data category and recipient. Make sure that no FML section, CBBS matching or percentage appears as established without a professional assessment. Hysio supports that check with recognisable sections. The insurance physician determines the status and only then makes the report final. [5] [1] [4] [6] [3]
The insurance physician assesses functional capabilities and, when the assessment framework requires it, completes the FML. Hysio can organise spoken explanations or source information around that assessment, but does not take over any section. An empty or unclear source is not filled in by the platform. The insurance physician checks every functional choice and the accompanying substantiation. [2] [6] [4] [5] [1]
The FML works with fixed sections covering, among other things, personal functioning, social functioning, adjustment to physical environmental demands, dynamic movements, static postures and working hours. Hysio can include a section named by the insurance physician in a recognisable way. It does not make any selection or score itself. The meaning of a choice remains part of the professional assessment. [2] [6] [4]
An FML choice is not a summary of isolated complaints, but a professional assessment of functioning at a particular moment and within a particular framework. Automatic completion can suggest a conclusion that was not stated. Hysio therefore limits itself to organising the text and sources provided. The insurance physician determines the sections, reasoning, date and status. [4] [6] [5] [1]
An assessment must align with the assignment, the sources used, the examination and the professional reasoning. Facts, findings and conclusion should follow on from each other traceably. Hysio can make that relationship visible and show uncertainties separately. The insurance physician remains responsible for the method and for whether the substantiation is sufficient. [2] [6]
Include only medical information that is necessary for the assignment and permitted for the chosen recipient. State the data category and purpose of the report before you share the text. Hysio can make those fields visible, but does not independently filter on legal necessity. The insurance physician decides what is included, withheld and disclosed. [3] [6] [4]
Record in the assignment who the commissioning party is, who receives the report and for what purpose it is used. An insured person, client, insurer or other party can have a different data boundary. Hysio organises that context alongside the report. The insurance physician checks authorisation and prevents a provisional report from going to the wrong recipient. [3] [6] [1]
Hysio places the contradictory sources side by side with date and author and does not choose between them itself. The insurance physician discusses which information is relevant to the assessment. Record that professional judgement as a separate source and leave unresolved uncertainty visible. [2] [6] [5] [1]
Hysio shows the description of the complaint and the functional finding as separate lines and does not draw any connection between them. The insurance physician assesses the meaning of both sources and records the professional conclusion themselves. Note the date, author and source status before the report is saved. [2] [6] [4]
Data category describes what kind of information the document contains and which recipient it is intended for. That helps to distinguish an internal medical document from a report or functional feedback. Hysio can record the classification as part of the dictation. The insurance physician checks whether the chosen category and recipient match the assignment. [3] [6] [2]
No. Hysio can organise facts, sources and a spoken conclusion, but does not make any decision on entitlement to a benefit, claim, percentage or legal status. Such decisions require an authorised professional and procedural assessment. The insurance physician determines the socio-medical conclusion; other authorised bodies make the formal decision within their own remit. [4] [6] [5] [1]
Hysio labels fact, finding and interpretation as separate parts and does not move anything from one to another. The insurance physician determines the professional meaning and the next steps. For each part, note the source, the date and the responsible author. [2] [6] [4]
Hysio includes only the capabilities that the insurance physician has stated and does not derive any functional capacity from them. Note the source, the moment and the wording verbatim. The insurance physician determines the weighting and the final conclusion. [2] [4] [6]
The work capacity specialist (arbeidsdeskundige) can use an FML when analysing jobs and job demands, after the insurance physician has assessed the functional capabilities. That is a different step from medically establishing capabilities. Hysio does not swap those roles and does not fill in an FML or job comparison. The authorised healthcare professionals remain responsible for their own part. [2] [6] [4]
Note who completed the questionnaire, when that happened, which answers are relevant and whether the insurance physician has discussed them. A questionnaire is source information and not automatically a medical judgement. Hysio can place answers and explanations side by side. The insurance physician determines the reliability, meaning and any need for additional examination. [2] [6]
Hysio places the assignment, the reference date and the data category above the content and leaves the judgement to the insurance physician. Note which source was used for the assessment and which information is still missing. The insurance physician determines the functional capacity and the next steps. [2] [4] [6]
Start with the question at hand and state only what the sources and the examination support. Distinguish between an established finding, a professional interpretation and a remaining caveat. Hysio can structure a spoken conclusion readably, but does not write a closing sentence of its own. The insurance physician checks that the text does not become more definite than the assessment allows. [5] [1] [2] [6]
Place uncertainties with the relevant finding or in a separate section for caveats and missing information. Explain whether they limit the answer and which follow-up step may be needed. Hysio can keep those points visible through to draft status. The insurance physician decides whether the information is sufficient for a conclusion or whether further examination is still needed. [2] [6]
Include the contact date, reference date, dates of sources, previous assessment and any change in the correct order. State when a source was received after the reference date. Hysio can organise the timeline without assuming a cause or development. The insurance physician decides which moments are relevant to the assessment. [2] [6]
Keep the assignment and the professional assessment separate from the wishes of a commissioning party, employer or other interested party. Record sources and differing perspectives transparently. Hysio can support the report structure and the origin of sources, but does not influence any conclusion. The insurance physician assesses independently, gives reasons for the judgement and keeps in mind the importance of a traceable report. [1] [6] [5]
An audit trail shows which assignment, sources, version, reference date and authorisation belong to a report. That helps with corrections, reassessments and questions about the origin of a passage. Hysio can organise that administrative data. The audit trail is not a substantive assessment and does not change anything about the insurance physician's responsibility. [2] [6] [5] [1]
Place the reason, reference date, previous assessment, new sources, current findings and the comparison in a fixed order, so that another insurance physician can follow the reasoning. Describe reasoning, conclusion and uncertainties without internal contradiction. Hysio does not fill in a percentage or FML and does not determine an outcome. The insurance physician checks whether every conclusion remains traceable to a source or their own finding. [2] [6] [9] [5] [4]
A first assessment starts with the assignment and the main question, followed by history, socio-medical context, work and participation, examination and relevant sources. Hysio can organise that information in the intake. The platform does not make a diagnosis or functional choice based on a single piece of reported information. The insurance physician completes the analysis and conclusion from their own expertise. [8] [5] [1] [6]
First identify which components the dictation already mentions and which are missing, such as a source, functional interpretation or a caveat. Do not fill in a missing component yourself; leave a visible gap until the insurance physician completes it. Hysio places the text that was actually dictated into the report structure of the report dictation. The insurance physician reads through the complete report and decides whether the text is complete and correct. [10] [5] [1]
A percentage does not follow from an isolated complaint or from the report structure. It requires a formal assessment under the applicable framework and authorised decision-making. Hysio therefore does not give a percentage and does not predict an outcome. The insurance physician can explain which question was examined and which conclusion has been recorded within their own professional role. [4] [6] [1]
The insurance physician responsible for the assessment checks the representation of sources, findings, reasoning, conclusion, caveats, data category and recipient. Hysio can present the report clearly, but does not replace a professional content review. Corrections remain part of professional record keeping. Only after their own assessment does the insurance physician decide whether the report becomes final. [5] [1] [3] [6]
The occupational physician works from occupational medicine expertise on health, work, prevention and return to work in the working environment. The insurance physician assesses health and functioning for a socio-medical or other explicit assignment. Hysio can support both professional contexts separately. It does not confuse work advice with an assessment for social security or an expert assessment. [1] [6] [4]
Describe the source, duration, reference date, concrete consequences for activities and the insurance physician's own findings. Avoid a fixed duration or prognosis if that has not been assessed. Hysio can place previous and current information side by side. The insurance physician gives reasons for the functional interpretation and states uncertainties or a necessary reassessment. [2] [6] [4]
Include the source, date, question and relevant findings, but keep the treatment perspective separate from your own assessment. A treating practitioner does not automatically answer the insurance medicine question. Hysio can organise the source inventory and differences. The insurance physician decides which information is taken into account and which conclusion is justified from their own assignment. [2] [6] [3]
Hysio keeps the reported information, examination, functional interpretation and conclusion in separate blocks and does not add any interpretation itself. The insurance physician determines the weighting and checks which source information supports the record. Missing data stays open instead of Hysio filling it in. [2] [6] [5] [1]
The insurance physician remains responsible for the assignment, examination, weighing of sources, functional interpretation, conclusion and authorisation. Hysio can structure the report from the information provided and leave uncertainties visible, but does not make any medical or legal decision. A provisional report is only usable once the insurance physician has personally checked the content and the disclosure of data. [5] [1] [4] [6] [3]
Hysio creates the report. You remain responsible for what you save in the record.
