Quick answer
Hysio organises the occupational health contact into employee information, work context, findings, professional assessment, agreements and a separate non-medical feedback section.
Each report type belongs to its own moment in sickness absence support, with its own questions and sources. Hysio does not determine work incapacity, diagnosis, prognosis, causal relationship or a return-to-work decision, and does not send medical information to the employer or any other recipient by itself.
Occupational medicine connects health, work and prevention based on the professional judgement of the occupational physician. The occupational physician assesses and advises and does not treat: the question is always how health and work relate to each other. A working day consists of consultations, sickness absence support and preventive examinations, with the employee's account, the work, findings and follow-up side by side. Hysio can organise those parts and does not take over the medical assessment. After the consultation, you use voice-driven reporting to describe what the employee reported and what you assessed; Hysio places this in the right sections. [1] [6] [5]
The internal report is intended first and foremost for the occupational physician and the occupational health record. The employee has the right to inspect it and to receive a free copy. An employer receives only the work-focused information that is permitted for work and follow-up. A problem analysis also has its own employer version without medical data. Hysio keeps those destinations apart; the occupational physician decides which version belongs with which purpose. [1] [6] [3]
During a consultation, Hysio can place the reason for contact, the employee's account, work context, findings, assessment and agreements in recognisable sections. The format follows Hysio Occupational Physician Consultation: seven sections stay within the protected record and only the eighth is non-medical and work-focused. There is no profession-specific prescribed heading structure for the consultation report, so this is an arrangement and not a standard. Hysio does not carry out its own examination and does not fill in a missing diagnosis, work capacity or prognosis. [7] [5] [1] [6]
The employee describes their own complaints, experience and abilities from a personal perspective. The employer can provide information about tasks, schedule, work organisation and factual progress. Those sources have a different role and must not be silently merged as medical facts. Hysio keeps the origin recognisable, so the occupational physician can weigh differences and ask follow-up questions where needed. [3] [6] [2]
Prevention looks at factors that can affect health and work before sickness absence arises or during a recovery process. This covers the open occupational health consultation, periodic occupational health examinations and, under strict conditions, a pre-employment medical examination. Work organisation, exposure, recovery options and timely identification also belong here. A report can name those topics separately when they were discussed in the contact. Hysio does not turn them into a risk assessment or prevention plan without an interpretation stated by the occupational physician. [2] [6] [4]
A professional assessment should be clearly distinguished from what the employee reports, what an employer provides and what was recorded earlier. Every judgement about work capacity, return to work or prognosis is therefore attributed verbatim to the occupational physician. Hysio can place such a judgement under its own section and leave the occupational physician's wording as it is: it is not softened, sharpened or turned into a more far-reaching conclusion. If no judgement has been stated, that place stays empty. [5] [1] [2] [6]
No. Hysio can organise a stated assessment of functioning or return to work, but does not determine whether someone is temporarily unfit for work. Nor does it independently derive a diagnosis, prognosis, cause or legal conclusion. The occupational physician advises on work capacity and return to work; an assessment of incapacity for work for a benefit lies with the insurance physician. Hysio records only what was discussed and stated in the contact. [4] [6] [5] [1]
An occupational health record keeps separate data sets: administrative data, medical data from the employee themselves, medical data from third parties and other documents such as the problem analysis. Within the contact, the reason for contact, sources, employee perspective, work context, findings, assessment and agreements together form a traceable record entry. Non-medical feedback gets its own place and recipient. Hysio leaves missing information open and does not turn every detail mentioned into a record fact. [2] [6] [3]
For an employee with neck complaints, the report can separately record the reported complaints, screen-based tasks, opportunities for breaks, observations and the next steps discussed. Any work-focused recommendation comes only from the occupational physician's stated assessment. Hysio does not add an ergonomic cause, medical diagnosis or fixed phased return-to-work schedule when these were not mentioned in the conversation. [2] [6] [4]
An internal medical record entry and feedback for the employee or employer have a different purpose and a different data boundary. The version that goes to the employer is derived from the full picture but itself contains no medical data. In Hysio Occupational Physician Problem Analysis & Feedback, medical content therefore appears only in the internal part; the feedback and the audit trail stay non-medical. The occupational physician authorises content and recipient. [3] [6] [10] [5]
Hysio Occupational Physician Consultation suits an occupational health consultation in which the reason for contact, the employee's account, work and functioning, findings, assessment and agreements are discussed. The format keeps the medical consultation report and the non-medical advice apart. In an open occupational health consultation, that non-medical part stays empty as long as an explicit destination and consent are missing. Before saving, check whether every professional judgement was actually stated and whether the recipient is correct. [7] [5] [1]
Hysio Occupational Physician Intake organises a first or in-depth conversation around the request for help, work and process context, health and functioning, sources, findings, analysis and open points. The SWOEP name describes the chosen documentation format and is not an independent medical standard. This format has one section in which Hysio thinks along, and that stays process-focused: it points to a missed process question, a gap in the sources or a missing evaluation moment, never to a judgement about work capacity or direction. The occupational physician completes the analysis themselves. [8] [5] [2] [6]
Hysio Occupational Physician Follow-up Consultation brings together earlier agreements, the current account, the course of work and reintegration, new findings, the assessment and the follow-up. This keeps visible what has changed since the previous contact. A comparison with that earlier contact is only included when both moments are in the source; Hysio does not derive a trend or prognosis itself. The occupational physician weighs which adjustment follows from the conversation. [9] [5] [2] [6]
This documentation format suits recording a problem analysis with an internal medical record entry, non-medical feedback and an audit trail. It is suitable when the request for help, work context and next steps must stay separate for different recipients. Personal contact and the occupational physician's own determination are firm conditions here. Hysio does not produce an action plan, a first-year evaluation or an insurance medicine judgement, and does not send anything independently. [10] [5] [3] [6]
Yes, the designated documentation format can organise an occupational physician note that was spoken or dictated afterwards according to the corresponding source structure. SOAP and SWOEP appear only in the name of the format; for this profession they are not an established standard. The output therefore does not use S, O, E or P headings, but the profession-specific sections of the occupational health contact. Hysio can organise the content readably without adding medical facts. The occupational physician reads, corrects and approves the result. [7] [5] [8] [1]
Choose based on the purpose of the contact: consultation, intake, follow-up consultation or problem analysis with feedback. Then look at the recipient and the distinction between the internal medical record and work-focused information. The name of the report format helps with the choice, but does not replace professional assessment. If in doubt, record the reason for contact and the desired outcome, and check the selection before saving. [7] [5] [8] [9] [10]
The consultation format shows the reason for contact and contact framework, the employee's account, work and functioning, professional findings, the occupational physician's assessment, agreements and non-medical feedback side by side. The first seven sections belong to the protected record; only the last is intended for another recipient. That structure supports checking source and recipient, but does not mean every section has to be filled. Missing information stays recognisably open. [7] [5] [2] [6]
First note who stated the agreement, what information it is based on and for which period it applies. Then describe the concrete action, owner and evaluation moment in the follow-up section. Return-to-work advice remains a professional judgement of the occupational physician. The action plan itself is work for the employer and employee together; Hysio does not write it and does not turn a passing remark into a binding reintegration decision. [9] [5] [2] [6] [4]
Use only the work-focused information that the occupational physician has stated and authorised for this recipient. For example, describe abilities and limitations in the work, agreed adjustments, contact moments and an evaluation date when these apply. Medical cause, diagnosis and treatment stay out, even when they explain the advice. Without an explicit destination and consent, the feedback stays empty; Hysio does not assume consent, signature or sending. [10] [5] [3] [6]
Check that the right person, date, documentation format and recipient are linked. Then review the source references, the employee perspective, the professional findings, the assessment and the agreements. Pay extra attention to medical details in feedback and to unanswered open points. Saving happens only after the occupational physician has personally reviewed the content and status. [5] [1] [3] [6] [2]
An employer can receive only work-focused information that is intended and authorised for that recipient within the applicable framework. The yardstick is necessity: what does this employer need to meet their obligations regarding continued payment of wages and reintegration? Think of work abilities, agreed adjustments and an evaluation moment, not diagnosis, cause or treatment data. Hysio does not independently select what may be shared; the occupational physician assesses the feedback and the recipient. [3] [6] [4]
That depends on the purpose, the type of data, the recipient and the professional basis. Requesting information from a treating professional and transferring the record to another occupational physician in principle require the employee's explicit consent. Hysio can record that consent was discussed, given or is missing, but never assumes agreement and does not give a legal judgement on the situation. [3] [6] [5] [1]
The medical record contains information the occupational physician needs for professional assessment and record keeping. Work advice translates an authorised assessment into what the employee and employer need to know about work and follow-up. The two purposes require a different selection of data. Hysio makes those parts recognisable, while the occupational physician remains responsible for content, access restriction and disclosure. [3] [6] [1]
Do not lump accounts, sources and professional assessment together. Keep the occupational physician's text recognisable as a judgement and mark open questions separately. If Hysio is unsure whether an item belongs in an employer-facing section, it leaves that item out there and marks it as a point to verify. This way the platform does not decide on incapacity for work, causality, prognosis or suitable work, and validation stays with the occupational physician. [4] [6] [5] [1] [2]
Note that information from the GP is an external source and keep the facts provided separate from the occupational physician's own findings. Medical data from third parties form their own data set in the record, separate from what the employee says themselves. Include only what is needed for this contact and this professional question. Hysio does not invent missing context and does not share such a source with the employer. [3] [6] [2]
With task delegation, it must be clear which task has been delegated, who the healthcare professional is, which instructions and supervision apply and where final responsibility lies. The medical judgement itself, such as the problem analysis, is personal and cannot be handed over unseen. Access to the record remains limited to those directly involved in the support. Hysio does not grant any authority and does not confirm a medical conclusion. [4] [6] [1]
Use the date of the contact and also make relevant reference dates or earlier agreements explicit when they are mentioned in the source. A document date is not automatically the date of the assessment. Hysio does not calculate a statutory week or deadline from a first day of sickness and names a point in the reintegration process only when the conversation or the record itself contains that point. If a date is missing, it stays open for the occupational physician. [2] [6]
Describe which perspective belongs to which source and state the difference concretely, for example about perceived abilities, tasks or progress. Do not add your own judgement to smooth over the difference. Then note which question is still open and who will pick it up. This keeps the distinction between account, observation and assessment visible in the record. [2] [6] [5] [1]
Leave a missing detail as an open point and state why it matters for the follow-up. Avoid filling it in based on assumptions, earlier patterns or a likely diagnosis. Hysio can organise a list of missing information with an action owner. The occupational physician decides whether further information is requested and how it is professionally weighed. That way admin with AI helps you organise and never becomes a source of new facts. [2] [6] [4]
No. Hysio does not send any report independently. It can make a desired recipient, document status and information boundary visible, but the occupational physician checks the content and authorises every disclosure. This keeps an internal medical record entry or draft feedback out of reach of an employer or other party until the occupational physician has assessed purpose, data category, recipient and final version. [3] [6] [5] [1] [4]
Use language that makes the source visible: the employee reports, an employer describes, an occupational physician observes or assesses. A professional finding can consist of an examination or observation during the contact, while an account reflects the personal perspective. Hysio places both in separate report sections. The occupational physician decides which finding is meaningful for the advice. [2] [6]
Functioning concerns what someone actually does, experiences or can discuss in a particular work and living situation. In this field it extends to work capacity and employability: what someone can manage in their own work, with which tasks, at what pace and with which adjustments. It is therefore broader than a diagnosis and it is not a judgement of incapacity for work. Hysio can organise that context. Only the occupational physician interprets what the described information means for work and follow-up. [2] [4] [1] [6]
No, Hysio does not fill in an FML or CBBS for occupational health documentation. These belong to the insurance medicine framework; work capacity information from the occupational physician has no fixed format and the profession's own instrument is called the employability profile. Hysio does not mark any item in it as limited or not limited and does not assign a CAS code either. A stated assessment of work and functioning can be recorded, without a score, percentage or legal conclusion. [4] [6] [2]
Describe an observation as concretely as possible and keep it separate from the interpretation. For example, note what was visible during the conversation, who observed it and in what context. Do not add a presumed cause. Hysio can place observation, account and assessment in separate sections. The occupational physician assesses whether the observation is relevant and reliable enough for the follow-up. [2] [6]
A measurement is only usable when it is clear who carried it out, with which method, at what moment and for what purpose. Occupational medicine also has no validated measurement instruments of its own: the employability profile, too, is qualitative and not a validated measurement. So do not detach a value from its context and do not draw a conclusion the healthcare professional did not state. Hysio can keep origin and outcome side by side; the occupational physician determines the meaning. [2] [6] [4]
Phrase only the advice the occupational physician actually stated and link it to work, contact or follow-up. A recommendation for breaks or task adjustment is not automatically a medical cause. Hysio can organise advice readably, but does not add a diagnosis, prognosis or causality. If the interpretation is missing, the point stays open. [4] [6] [5] [1]
Source status makes clear whether an item comes from an account, observation, measurement, earlier document or professional interpretation. This helps avoid mixing facts, interpretations and uncertainties. Hysio can make source roles and open points visible. The occupational physician checks whether the source is represented correctly and whether the information is relevant for this record purpose. [2] [6]
Use concrete work language about tasks, adjustments, contact and evaluation, without medical labels or unnecessary background. State who does what and when things will be reviewed again. Hysio can structure the authorised text clearly, but does not translate medical content on its own initiative. The occupational physician checks whether the wording is both understandable and within the information boundary. [3] [6] [2]
Place the employee, employer, treating professional and occupational physician side by side as separate sources. Describe similarities and differences factually, without silently siding with one source. Hysio supports a traceable sequence from perspective to finding and assessment. The occupational physician decides which additional question is needed and which information belongs in the final report. [2] [6]
An audit trail shows which source, date, version and authorisation underlie a piece of feedback. That is useful when several versions are circulating or when work advice has to be traced later. Hysio can organise that administrative origin. The audit trail changes nothing about the medical assessment and is not proof that a draft has already been sent. [2] [6] [3] [5] [1]
Start with what the employee reports about the neck complaint, the recovery and the current working day. Then note the occupational physician's relevant findings and only the advice actually stated about tasks, hours or adjustments. Put the evaluation date and action owner in the follow-up, and make visible who is monitoring the return to work. Hysio does not create its own work capacity profile, phased return-to-work schedule or recovery prognosis. [2] [6] [4]
Record who made contact, what the reason was, what information was discussed by telephone and which points could not yet be assessed. Use the follow-up or consultation format that fits the purpose of the contact. Hysio does not fill in observations that could not take place. The occupational physician decides whether a consultation, additional examination or just an agreement is needed. [9] [5] [2] [6]
A good follow-up agreement names the action, the person responsible, the timeframe and the moment the situation will be reviewed. Without an owner and timeframe it is not an agreement but an intention. Also keep visible whether the agreement comes from the employee's perspective, an employer's proposal or professional advice. At the next encounter Hysio can place the previous agreement and the current position side by side; the occupational physician confirms or adjusts. With AI reporting you dictate the agreement as you made it, with owner and timeframe included. [2] [6] [5] [1]
Record both accounts without presenting the employee's response as agreement. State which part is disputed, what explanation was given and which question is still open. An employee can request correction of factual inaccuracies, but not of the occupational physician's own observations, considerations and advice. Hysio organises the difference and the follow-up action; the occupational physician decides whether reassessment, additional contact or adjusted feedback is needed. [2] [6] [5] [1] [3]
Correct the source-bound text visibly and then check the date, source, recipient and professional assessment again. Do not let a correction quietly become a new fact or new conclusion. Hysio supports editing a text, but does not decide which version is legally or medically authoritative. The occupational physician approves the corrected record entry. [5] [1] [2] [6]
No. A prognosis requires a professional assessment of the individual situation and the available information. Hysio can recognisably include a prognosis stated by the occupational physician, with date and context, but does not invent a timeframe and does not extrapolate from earlier contacts. If a prognosis is missing, that remains an open point for the occupational physician. [4] [6] [5] [1]
First ask what purpose the recipient has, which data are necessary for it and whether the occupational physician has authorised this disclosure. Then check whether medical details stay out of the feedback and whether the employee has been informed in an understandable way. Hysio can show those checkpoints, but does not give consent and does not take over a privacy decision. [3] [6] [4]
The occupational physician assesses health, functioning and work in the context of the contact, drawing on occupational health expertise. The work capacity specialist mainly examines the relationship between person, work, workload and abilities within the framework of work capacity assessment. Hysio can describe both roles separately. It does not confuse a medical assessment with an analysis of tasks or jobs. [1] [6] [4]
Agree in advance which contact type you use, which source roles you apply, who checks drafts and which recipients may receive feedback. Start with one recognisable consultation situation and test above all the separation between the medical record and work advice. Hysio provides the structure; the occupational physician determines the local way of working, quality of content and saving. [2] [6] [5] [1]
Responsibility remains with the occupational physician who conducted the contact or who assesses the report within their own authority. That healthcare professional checks facts, interpretation, data boundary, recipient and status. Hysio organises the information provided and can make a proposed text readable. The platform does not make any medical decision and does not automatically turn a draft into a final record entry. [5] [1] [4] [6] [3]
Hysio creates the report. You remain responsible for what you save in the record.
