Quick answer
Hysio organises medical advice and expert assessment into assignment, statements, sources, own findings, functional translation, conclusion, advice and reservations.
Advice consultation, expert interview, report dictation and outcome with handover each have their own format. Hysio does not establish a medical diagnosis, causality, prognosis, FML, CBBS profile, AMA assessment or legal conclusion by itself, and does not send a report.
This physician assesses health and functioning within an explicit advice or expert assessment assignment. The physician distinguishes between source information, own findings, medical interpretation, functional translation, conclusion and advice. Hysio can organise those parts, but does not take over any medical assessment. The physician remains responsible for independence, substantiation and reporting. With voice-driven reporting, you dictate your findings after the consultation or interview, and Hysio keeps statements, sources and your own judgement separate. [1] [6] [5]
Medical advice answers a concrete question about health or functioning within an agreed context. An expert assessment is usually an independent examination with a formal assignment, sources and sub-questions. The precise meaning depends on the assignment and recipient. Hysio can distinguish both structures, but the physician determines the method, interpretation and conclusion. [1] [6] [4]
The person concerned is the person the assignment and the examination are about. Depending on the context, that person may be called, for example, insured person, employee, examinee or person under assessment. Hysio can record the chosen context without mixing up labels. The physician checks identity, assignment, consent, recipient and data category. [1] [6] [3]
The profile medical advice, policy, indication and examination has been recognised as of 1 July 2026 within the specialty Maatschappij en Gezondheid (society and health); the associated title is arts medisch advies (medical advice physician). The title is not legally protected under the Wet BIG (Dutch Individual Healthcare Professions Act), but it is a recognised profile register within that specialty. Hysio can mention this context, with its source, when it is relevant to the assignment. The physician remains responsible for registration, competence, assignment and substantive assessment. [1] [6] [2]
In this context the physician works on advice or expert assessment and not automatically within a treatment relationship. The purpose, the assignment and the recipient determine which information is collected and shared. Hysio can show this distinction in the contact framework. The physician decides whether a medical question outside the assignment calls for referral or consultation. [1] [6] [4]
Hysio can place the assignment, those present, statement, medical sources, own findings, functional translation, conclusion, advice and uncertainties in recognisable sections. That makes the reasoning verifiable without generating a judgement. The physician reads and corrects the report, determines the data category and authorises what is ultimately recorded as advice or report. [7] [5] [1]
No. Causality and prognosis require medical and contextual assessment of the individual case. Hysio can include a consideration stated by the physician in the report in a recognisable way, but does not itself make a connection between timeline, complaint and source report. The physician names the substantiation and uncertainty and decides whether the conclusion sufficiently answers the assignment. [4] [6] [5] [1]
Record the question posed, context, commissioning party, recipient, authorisation, examination method and data category before findings are described. That way it stays clear for which purpose information was collected and which sub-questions need to be answered. Hysio can structure this information. The physician checks that the assignment does not change unnoticed during reporting. [2] [6] [3]
Sources can consist of the statement of the person concerned, source reports, correspondence, own examination and other authorised documents. Note the author, date, content and limitations for each source. Hysio can organise the source inventory. The physician assesses reliability, currency, relevance and the significance for the medical interpretation. So name the source, date and context as soon as they affect the meaning of the information. [2] [6]
The physician remains responsible for assignment, examination, weighing of sources, medical interpretation, functional translation, conclusion, advice and authorisation. Hysio can structure a consultation or dictation clearly, but does not make a diagnosis or legal judgement. Before saving, check the recipient, consent, data category, uncertainties and version. A draft does not automatically become a final report. [5] [1] [3] [6] [4]
This format suits an advice consultation in which the assignment, those present, statement, medical sources, own findings, functional translation, conclusion and advice are discussed. The physician can name uncertainties and follow-up questions separately. Hysio organises the content, but does not write any medical meaning of its own. The physician checks the text and determines the report status. [7] [5] [2] [6]
The expert assessment interview organises assignment, authorisation, setting, method, statement of the person concerned, other statements, own interview findings, open verification points, professional interpretation and follow-up. This keeps the interview source distinct from a medical judgement. Hysio does not add any examination or conclusion. The physician checks the method, limitations and status. [8] [5] [2] [6]
Use report dictation when the physician dictates a formal report with assignment, method, facts, own findings, medical interpretation, functional translation, conclusion, advice and boundaries. The format also keeps track of source references, recipient and report status. Hysio works out the dictation, but does not add any reasoning. The physician authorises the report version. [9] [5] [1]
This format organises the established outcome, explanation, uncertainties, authorised functional information, necessary handover, follow-up agreements and document status. This keeps it clear what the physician has established and what may be shared with a recipient. Hysio does not send an outcome independently. The physician checks authorisation, recipient and content. Before saving, mark which passage still needs checking by the responsible healthcare professional. [10] [5] [3] [6]
Choose Medical Advice Consultation for an advice contact in which the question and medical interpretation are central. Choose Expert Assessment Interview when an authorised expert assessment assignment, interview method, statements and verification points are leading. The assignment framework is decisive, not only the type of conversation. Hysio helps you structure; the physician determines method and status. [7] [5] [8] [1] [6]
Start with the commissioning party, question posed, authorisation, setting and method. Then describe the statement of the person concerned, other sources, own interview findings and open verification points. Only after that, place the professional interpretation, recognisable as the physician's judgement. Hysio maintains this order. The physician determines which limitations and follow-up questions belong in the expert assessment report. [8] [5] [2] [6] [1]
Keep the commission, source references, facts, your own findings, medical interpretation, functional translation, conclusion, advice and uncertainties apart. Hysio can place the dictated text in that structure. The platform does not fill in missing reasoning or a missing source. The physician compares the report with the dictation and authorises the report status. That way admin with AI helps with the structure, while the content stays yours. [9] [5] [2] [6] [1]
First select the authorised information that is necessary for the recipient and keep the full medical assessment in the correct report framework. State what has been established, which uncertainties remain and which follow-up action has been agreed. Hysio can organise those parts, but does not share anything automatically. The physician checks data category and recipient. [10] [5] [3] [6] [4]
Yes, a suitable documentation format can organise a spoken or subsequently dictated advice or expert assessment note according to the assignment framework. Hysio makes source, own finding and the physician's judgement recognisable, but does not add any medical conclusion. The physician checks the output, corrects nuances and decides whether the text counts as a draft or an authorised report. [7] [5] [9] [1]
Check identity, assignment, authorisation, method, sources, own findings, medical interpretation, functional translation, sub-questions, conclusion, advice, uncertainties, data category and recipient. Make sure a source report does not read as your own examination. Hysio makes the sections visible. The physician determines the final status and any handover. A report always requires your own professional reading before it is saved. [5] [1] [3] [6] [2]
Use medical data only for the explicit purpose of the assignment and within the permitted data category. Note the source, necessity, recipient and any authorisation. Hysio can make those context fields visible, but does not decide independently about sharing. The physician makes the final selection and safeguards confidentiality and professional independence. [3] [6] [4]
A source report contains information from another physician, healthcare professional or organisation. Own findings come from the examination or interview of the physician carrying out the advice or expert assessment. Hysio keeps both separate. The physician assesses the source and only then formulates their own interpretation; a supplied report is not tacitly adopted as their own observation. [2] [6]
State in the document who the commissioning party is, who the authorised recipient is and for which purpose the information is used. Also note whether a limited functional handover has been agreed. Hysio can structure that information, but does not send a report. The physician checks authorisation, data category and the limit on reuse. [3] [6] [5] [1]
Functional translation describes what the medical assessment means for functioning within the question posed, without losing the source or the professional uncertainty. It is not an automatic FML, CBBS or AMA judgement. Hysio can organise a translation stated by the physician. The physician determines content, context and reservations. [2] [4] [6]
Name which source is missing, which finding could not be verified and what consequence that has for answering the question. Make clear whether a partial conclusion is provisional or whether additional examination is still needed. Hysio can show uncertainties separately. The physician determines the reservation and the final report status. [2] [6] [5] [1]
Limit the report to the information that is necessary and permitted for the assignment and tailor it to the recipient. Medical details do not automatically become part of an employer or insurance report. Hysio can record data category and handover conditions. The physician checks the selection and authorises the document. [3] [6] [4]
No. Hysio does not complete an FML, CBBS component or AMA assessment and does not calculate a percentage or outcome. It can organise the functional translation that the physician has explicitly stated. The physician determines the appropriate assessment framework, the substantiation and the conclusion. [4] [6] [2]
Note for each report the author, date, question and the concrete difference. Keep your own findings separate and describe which verification is still needed. Hysio can structure the comparison, but does not automatically choose a source. The physician weighs the documents, records uncertainty and determines the professional conclusion. [2] [6]
Independence requires a clear assignment, transparent weighing of sources and a conclusion that is not adjusted to the desired outcome of the commissioning party or another party. Hysio can make the assignment and sources traceable, but does not safeguard independence on its own. The physician determines the method, names interests and authorises a substantiated report. [1] [6] [5]
The Wet op de medische keuringen (Dutch Medical Examinations Act) forms a specific legal framework for certain medical examinations and must not be tacitly confused with every advice or expert assessment assignment. Hysio can show the chosen framework as context, with its source. The physician determines whether the framework applies and does not give a general legal conclusion based on the report structure. [4] [6] [2]
A fact comes from a source or statement and is recognisable as such. An own finding follows from the physician's examination or interview. The interpretation connects that information to the medical question posed. Hysio can show these layers separately. The physician determines which reasoning and conclusion are professionally justified. [2] [4] [6]
Answer the question with the available sources and own findings and name the limits of the assessment. Use a reservation when information is missing or has not been verified. Hysio can organise the stated text clearly, but does not make it more definitive or more cautious on its own initiative. The physician checks the wording. [5] [1] [4] [6]
Organise the date of first notification, relevant events, sources received, contact moments, examination and current status. Make clear when a source was received after an important reference date. Hysio can structure the timeline without assuming causality. The physician determines which moments are significant for the assessment. Keep actions not carried out and changed actions separate from what has actually been completed. [2] [6]
Mark the sub-question literally, name which information is missing and explain whether additional examination or a reservation is needed. Do not write an answer that merely sounds plausible. Hysio can keep the open question visible. The physician decides whether the report can be completed or first needs to be supplemented. [2] [6] [5] [1]
Note what was examined, when, with which method, and which findings the physician made personally. Keep the description apart from the medical interpretation and conclusion. Hysio can organise these parts according to the dictation. The physician checks completeness, limitations and the relation to the commission. With voice-driven admin, it helps to name the method before the finding. [9] [5] [2] [6]
First describe the medical finding and then its functional meaning within the concrete question, with context and reservation. Do not add a general statement about work when the physician has not assessed it. Hysio can keep the two layers separate. The physician determines the translation and its limits. [2] [4] [6]
Record both statements and the concrete difference with date and source. Note which parts could be verified during the interview or examination and which remain uncertain. Hysio can make the difference visible. The physician weighs the sources and describes the effect on the conclusion. [2] [6]
Make clear for each recipient which information is necessary and authorised for the purpose. Keep the full medical report separate from a limited functional or process-related handover. Hysio can show versions and data category. The physician determines content, recipient and authorisation before anything is shared. Then check whether consent, authorisation and document status still fit the recipient. [3] [6] [5] [1]
A reservation makes visible which limitation in source, examination, method or assignment affects the conclusion. As a result, uncertainty is not hidden in a general sentence. Hysio can organise reservations separately. The physician determines which limitation is relevant and whether the question can still be answered sufficiently. [2] [6]
Advice gives the physician's professional interpretation and recommendation within the assignment; a formal decision may lie with another competent party. Hysio can show advice and recipient separately, but does not make a decision. The physician states their own role clearly and prevents the report text from suggesting more authority. [1] [6] [4]
Organise the assignment, timeline, statement of the person concerned, supplied medical sources, own examination, medical interpretation, functional translation and uncertainties. Only describe a connection when the physician has assessed it professionally. Hysio does not draw a conclusion on causality. The physician checks sub-questions, source quality, reservation and authorised recipient. Compare the chosen structure with the purpose of the contact before you finalise the report. [8] [5] [2] [6] [4]
Record the assignment, recipient, source framework, relevant statement, own findings, functional translation, conclusion and advice according to the agreed data category. Do not share more medical information than necessary. Hysio can support the report structure, but does not determine a claim or percentage. The physician authorises the text and the handover. [7] [5] [3] [6] [4]
Check the assignment, authorisation, sub-questions, identity, setting, method and available sources in advance. Make clear which statement of the person concerned is requested and which verification points remain open. Hysio can organise these. The physician determines the examination method, consent and professional boundaries. Do not record a missing assessment as a conclusion, and first schedule the appropriate professional check. [8] [5] [2] [6] [1]
Keep source, own finding, interpretation, conclusion and advice in separate parts so that a change does not quietly alter several layers. Note the version and open points. Hysio can present a report clearly. The physician checks the correction against the dictation and authorises the new version. After AI reporting you can then quickly find the passage you want to change. [9] [5] [1] [2] [6]
Record the assignment and the request as context, but let the medical assessment follow from the examination and relevant sources. Do not adopt a conclusion because it is desired. Hysio can make source and assignment visible, but does not safeguard independence on its own. The physician formulates their own substantiated conclusion or states why the question is not answerable. [1] [6] [4]
Describe what the physician has established, what explanation was given, which uncertainties exist and which follow-up appointment was made. Keep necessary information for another recipient separate from the full medical document. Hysio organises the outcome, but does not share anything automatically. The physician checks authorisation, recipient and status. [10] [5] [3] [6] [2]
No. Hysio can structure a dictation or consultation and leave open points visible, but does not complete the expert assessment substantively: the platform does not determine a diagnosis, causality, prognosis or conclusion. The physician checks method, sources, reasoning and advice, marks reservations and status, and decides personally when the report is ready. [4] [6] [5] [1]
Record which passage does not match the source, check the original document date and only adjust the supported text. Then reassess the interpretation, conclusion, recipient and status. Hysio supports version control, but does not determine medical meaning. The physician authorises the correction and records the reservation where needed. [2] [6] [5] [1]
Agree which assignment fields, source roles, data categories and recipients you use and who checks the reports. Test with an advice consultation and an expert assessment interview and check whether fact, finding, interpretation, functional translation and conclusion stay separate. Hysio provides the structure. The physician determines the method, local way of working and authorisation. Link every agreement to the purpose of the contact and check the meaning before saving. [2] [6] [5] [1]
The physician remains responsible for the assignment, the examination, the weighing of sources, the medical interpretation, the functional translation, conclusion, advice and disclosure. Hysio makes a report traceable, but does not take over any medical or legal decision. Before saving, check the data category, recipient, uncertainties and version. A draft is not automatically authorised. [5] [1] [3] [6] [4]
Hysio creates the report. You remain responsible for what you save in the record.
