Follow-up consultation

Exercise Therapy Consult

Turns one spoken follow-up session into an exercise-therapy SOAP note, in line with the VvOCM Documentation Guideline 2022. For regular follow-up sessions, exercise sessions, interim evaluations and closing consultations.

  • Subjective: adherence to the home exercise programme and the ADL impact on work, household, sport and social roles.
  • Objective: posture and movement observation, static and dynamic, plus an update of your clinimetrics such as the PSK.
  • Evaluation: the progress in movement pattern, goal attainment and body awareness that you record.
  • Plan: home exercise programme, education theme and self-management agreements as you discussed them with your patient.
  • Extra: a separate consultation summary and a short EHR-ready SOAP summary.
