Referrer

Dr S. van Daalen, GP

Patient

Mr M. de Zwarte, b. 02-02-2005

Complaint

21-year-old patient, football injury last Saturday during a match. Heard a clear pop/crack in the right knee during a twisting move with the standing leg planted. Acute swelling within 30 minutes. Unable to keep playing, a feeling of instability (the knee gives way). Full weight-bearing barely possible. Anti-inflammatories give limited relief. No prior knee complaints. Due to play in a promotion match this Saturday, high pressure to return quickly. Studying Sport and Exercise at university, placement at risk.

Question

Assessment and treatment, possible ligamentous injury to the right knee.

Text, PDF or DOCX. Every format is read in.
Core 01

Acute right knee trauma after a non-contact twisting mechanism. Pop sensation, acute effusion and a feeling of instability: high suspicion of an anterior cruciate ligament rupture, possibly combined with a meniscal or MCL component. Urgent orthopaedic consultation plus MRI indicated.

Points to watch 02

Red flags, ligamentous injury

A pop sensation, acute swelling and instability: the classic triad of an ACL rupture. Acute haemarthrosis within 2 hours points to intra-articular bleeding. MRI is clearly indicated.

Further examination needed

Lachman test, anterior drawer, pivot shift; meniscal provocation (McMurray). Assess the medial collateral ligament and the posterolateral corner in a rotational injury.

Yellow flags, psychosocial

High return-to-sport pressure, placement context, young-adult age: risk of loading too early. Discuss a realistic timeline (6-9 months) and the possible surgical route.

Next step 03

Acute phase: PEACE protocol (Protect, Elevate, Avoid anti-inflammatories, Compression, Education). Lachman test plus pivot shift at intake. Referral to an orthopaedic surgeon within 1 week. Discuss conservative versus surgical pathways depending on activity level.
