Acute right knee trauma after a non-contact twisting mechanism. Pop sensation, acute effusion and a feeling of instability: high suspicion of an anterior cruciate ligament rupture, possibly combined with a meniscal or MCL component. Urgent orthopaedic consultation plus MRI indicated.
Red flags, ligamentous injury
A pop sensation, acute swelling and instability: the classic triad of an ACL rupture. Acute haemarthrosis within 2 hours points to intra-articular bleeding. MRI is clearly indicated.
Further examination needed
Lachman test, anterior drawer, pivot shift; meniscal provocation (McMurray). Assess the medial collateral ligament and the posterolateral corner in a rotational injury.
Yellow flags, psychosocial
High return-to-sport pressure, placement context, young-adult age: risk of loading too early. Discuss a realistic timeline (6-9 months) and the possible surgical route.
Acute phase: PEACE protocol (Protect, Elevate, Avoid anti-inflammatories, Compression, Education). Lachman test plus pivot shift at intake. Referral to an orthopaedic surgeon within 1 week. Discuss conservative versus surgical pathways depending on activity level.
