Quick answer
Four documentation formats for neurology and stroke care, with body side, time of measurement and source always clear.
An overview of the reports Hysio writes on the neurology ward. You read every report back before it goes into the record.
The profile covers a Neurology Nursing Intake, Neurology & Stroke Observation Report, Neuro Ward Round & Family Meeting and Neurology Nursing Handover. They describe different moments: an initial assessment, a single observation, a discussion and a handover. Choose based on that purpose. The source of a diagnosis, measurement or agreement stays recognisable; Hysio does not turn your information into a new treatment choice or clinimetric score. With voice-driven reporting, you dictate your findings after an observation or neuro ward round; Hysio organises them and you check the side of the body, time of measurement and source. [3] [4] [5] [6]
The Neurology Nursing Intake organises the first contact on the stroke unit: the reason for contact and the admission, neurological observations, speech, swallowing and functioning, and the agreements you make with the client and relatives. The format keeps the source of every observation recognisable and leaves missing information open. The nurse decides which findings and agreements are documented in the record. [3]
This documentation format organises a single time-stamped observation moment during a shift. It preserves what was observed, measured, carried out and agreed. Earlier observations stay with their own source and time. Hysio does not calculate a score and does not make an escalation decision. Before use, check whether a reported change really belongs to this moment and has not been carried over from older record context. [4]
The report describes who was present, which information was discussed and which questions, concerns and agreements were expressed. A medical judgement stays with the medical source; the input of relatives stays recognisable alongside it. Hysio does not make its own prognosis. Check that an open question does not appear as answered and that a discussion is not presented as a joint decision. [5]
The handover brings together the purpose of the document, the recipient, current condition, scores mentioned, medication facts and open actions. It describes one transition between shifts, wards, institutions or partners in the care chain. State the source and reference time of the information. The result is a text for professional review, not proof of sending, receipt or technical connection to an electronic exchange format. [6]
The intake fits the first nursing data collection after admission or referral. An observation report, by contrast, describes one later or separate observation moment. The presence of neurological observations alone does not determine the choice. Look at the purpose of the contact and prevent a short moment during a shift from being expanded into an intake with medical history or care agreements that were not discussed. [3]
Describe the observation as stated and record who made it. Hysio does not add terms such as aphasia or neglect when the source does not use them, and does not infer clearance to swallow. Check the wording against your original observation. Keep a reported difficulty, a test that was actually carried out and a professional conclusion recognisable as different kinds of information. [4]
No. The format can describe one neuro ward round, one family meeting or, explicitly, both. The input determines what actually took place. Hysio does not add a family meeting to a ward round without relatives and does not turn a family meeting into a medical consultation. State the participants and the reason, and check that only the topics and agreements actually discussed come back in the report. [5]
For neurology and stroke nursing, Starter costs €20 per month, Pro €40 and Unlimited €80, excl. VAT. With an annual plan you pay for ten months instead of twelve. The price does not differ per intake, observation, neuro ward round or handover. The neurology nurse assesses the source, observation and follow-up before reviewed text goes into the record according to local agreements. [7]
Use only information that is necessary for the purpose of care and follow the privacy agreements of the care organisation. In a neurological intake or family meeting, it must be clear who provided information and who may receive the report. Access and data sharing require an appropriate legal basis; being present at the conversation is not sufficient for that. Hysio's privacy policy describes the processing roles and agreements. [9]
No. The Verpleegkundig Specialisten Register (register of nurse practitioners) has a separate legal basis in article 14 of the Wet BIG. Working in neurology does not automatically make someone a nurse practitioner. In a report, use the actual role and the source of a decision. A profile name or chosen documentation format gives no additional authority and does not replace checking registration and expertise. [17]
The Kwaliteitsregister V&V (quality register for nurses and carers) makes continuing professional development visible in a personal portfolio. That is a different purpose from the statutory BIG registration. For nurses, the basic standard is 184 points in five years, including at least 80 points of accredited training. Check any additional or temporary conditions with V&VN. This registration information belongs to your professional development and is not an evidence field in every client report. [16]
For re-registration as a nurse, a period of five years applies. The work experience requirement is at least 2,080 hours of relevant work; not every work activity counts automatically. In its assessment framework, the BIG register explains which hours and activities qualify. This concerns your own professional registration and work experience. This information does not belong in a client report as proof of registration. [25]
Certain reserved procedures can be carried out by a nurse on instruction. Functional independence means that for those designated procedures no supervision or intervention by the person giving the instruction is needed, provided you are competent and work within your area of expertise. It remains performance on instruction. In the report, state the procedure actually carried out and the relevant instruction; a report does not itself grant any authority. [24]
The CVA working group within Neuro & Revalidatie connects nurses and carers who work with people after a stroke and their relatives. The fields of work range from acute care to rehabilitation and the chronic phase. The working group contributes, among other things, to professional exchange and symposia. That is professional development, not automatic approval of an individual report or platform. [15]
No. The intake starts at admission or first contact after an authorised person involved in the care has decided that the client will be assessed on the stroke unit, the neurology ward or in the rehabilitation phase. Hysio organises the nursing information. It does not determine the type of stroke, the indication for admission or urgency. Check that the reason for contact and a medical decision are recognisably attributed to their actual source. [3]
A stroke service connects the parties that provide care in the acute, rehabilitation and chronic phase after a stroke. The emphasis and the disciplines involved can differ per phase. For continuity, it needs to be recognisable which information and agreements are passed on at the transition. A handover report supports that collaboration, but does not itself arrange the availability or acceptance of follow-up care. [15]
No. A family meeting records what was discussed with those involved. The handover is aimed at the party that continues the care and contains current information and open actions relevant to that. Only include an agreement with the family when it matters for the transition. Keep the speaker recognisable and do not equate a possibility that was discussed with follow-up care that has been arranged. [5] [6]
Typing yourself and dictating each require checking the same neurological facts. When dictating, you also assess how Hysio has organised the information. Pay attention to the source, side of the body, time of measurement and the difference between observation and conclusion. Compare the corrections needed and the readability for the next shift, so that you assess the whole way of working and not only the initial writing. [10]
The trial lasts two weeks. With a made-up handover, you can see whether observations, changes in functioning and agreements come back clearly. Also try an observation of speech and swallowing and a short family meeting. You can use all four documentation formats for neurology and stroke nursing during the trial. [8]
The Richtlijn Verpleegkundige en Verzorgende Verslaglegging (V&VN guideline on nursing and care documentation) provides a profession-wide basis for the record and handover. It is not an exclusive guideline for stroke care. Use it alongside the clinical guidelines and working agreements of your ward. The record must show the coherence between data collection, care plan, delivery and evaluation; a chosen text structure does not prove that coherence by itself. [26]
The multidisciplinary Richtlijn Herseninfarct en Hersenbloeding (guideline on ischaemic stroke and intracerebral haemorrhage) covers the clinical content of stroke care. The profession-wide V&VN guideline concerns nursing documentation and handover. They therefore have different purposes. Use the relevant current module and your organisation's agreements. A clinical recommendation gives no permission to write a measurement that was not carried out or a treatment that was not discussed into the record as a fact. [18] [26]
The record connects the nursing history and nursing problem description with the care plan, progress, evaluations and handover. This includes the relevant medical information, care delivered, jointly agreed goals and observations. Carry over a medical diagnosis from the medical source so that its origin stays recognisable; do not present it as a new nursing judgement. That way, a next colleague can trace how the care has developed. [12] [13] [14]
The guideline lists the neurological diagnosis, deficits and current functional status, psychosocial information, neuropsychological status and limitations in communication. The advice for medical, nursing and allied health treatment must also be available at discharge. Use these components to check whether the handover supports the receiving care. Carry over only established information and make missing data recognisable for follow-up. [18]
State every time with the event and source it relates to. The moment someone noticed symptoms is not automatically the onset of those symptoms, and the intake has its own time. Hysio may not calculate missing times. Above all, check that a report from a relative and a professional finding are not merged into one definite timeline. [3]
Link every observation to its source and time of measurement. Use an earlier note only if it was actually provided or cited, and keep its date visible. Hysio may not update an old finding because it still sounds plausible. Compare the report with what was observed during the current moment and leave unclear differences open for verification. [4]
Record the question and the answer actually given by the healthcare professional concerned. If the neurologist will discuss something later, that remains an open follow-up and not a prediction. Hysio does not fill in a recovery period or prognosis. Check that concerns of relatives do not appear as a medical conclusion and that it stays clear who picks up a follow-up question. [5]
Choose the Neurology Nursing Handover when another care team continues the care and needs to be able to find the current care, open actions and follow-up agreements. You include information from a neuro ward round or family meeting only when it is relevant to this handover. For the report of that ward round or conversation itself, there is Hysio Neuro Ward Round & Family Meeting. You assess the purpose, recipient and the right documentation format. [6]
Check whether observations, scores mentioned and follow-up agreements belong to the right neurological contact. Then transfer the reviewed text according to the route your organisation uses. A write-up in Hysio does not prove that the record note has been saved. Verify the current integration options and check at the destination that the source attribution and reference time stay clear after transfer too. [11] [10]
Check the report before you transfer it. Pay attention to who said something, the time of an observation and the difference between a reported change and your own finding. Admin with AI writes the text, but in a neuro ward round or family meeting you check whether each perspective stays recognisable. Correct what is wrong and check which next steps were actually agreed. [10]
No. Described symptoms remain observations; Hysio does not derive an independent diagnosis or a calculated score from them. Only a score that was actually taken and mentioned may be carried over, with the instrument, source and time of measurement. Check that a description of speech or strength is not quietly converted into a subscore, total score or medical interpretation that nobody has stated. [3] [4]
No. This format describes a nursing handover. A medical diagnosis or treatment advice can appear in it when it has been explicitly provided from a medical source and is relevant to the recipient. Hysio does not thereby write an independent medical discharge policy. Check the function of the document and make clear which agreements are nursing agreements and which were determined by another discipline. [6]
A neurology nurse can describe how a mobilisation or transfer went during care. That observation is not a physiotherapy assessment or treatment plan. Therefore keep it recognisable who made a finding and who a treatment recommendation comes from. Do not adopt physiotherapy methods or conclusions as your own nursing intervention when the source does not support that. That way, everyone's contribution to stroke care stays clear. [22]
The stroke unit guideline recommends determining serum glucose on admission and measuring body temperature at least daily. That is not a general instruction to measure every parameter mentioned daily. Record the value actually measured, the unit and the time. Keep an earlier measurement separate from a current finding and follow the applicable care policy for further checks. [18]
It keeps the stated side of the body and body part with the observation concerned. Hysio may not change a side based on what seems anatomically likely. An explicit correction by the speaker applies only to the corrected item. Check these details carefully and leave a contradiction visible until the right source has clarified it. [3]
No. Hysio does not calculate or estimate such scores from described symptoms and does not fill in a missing subscore. Carry over only a result that was actually mentioned, with the instrument, time and source. An impression of improvement is not a new score. Therefore also check that an old result is still recognisable as old and does not appear as a current measurement. [4]
State who says, assesses or agrees something. A relative's impression remains a report; a medical explanation stays with the medical source. Hysio does not turn several voices into joint consent. Review the report for this distinction, especially where there is uncertainty about recovery, preferences discussed and decisions that still require consultation or feedback. [5]
The Neurology Nursing Handover brings together the current care, open actions and the follow-up for the receiving care team. State an action with the agreed responsible person and timeframe when these have been discussed. A conversation about the follow-up does not yet prove that an action has been carried out or taken over. Information from a family meeting can be relevant, but does not turn the handover into the report of that conversation. [6]
Check the deviating sentence against the original observation or statement. With neurological information, side of the body, score, time and source deserve extra attention. Correct the specific item and check whether the same error recurs elsewhere in the text. Do not change a professional conclusion based on your own assumption, and follow the local correction agreements if the note has already been included in the record. [10]
Do not guess a side of the body, score or time when a dictation is unclear. Consult the original information or the person who made the observation. Keep the passage open as long as it is not verifiable. After clarification, check that the text describes the right time of measurement and that your own observation is not mixed with information from a relative after all. [10]
Any supplementary consideration is kept separate from the factual intake and the care plan discussed. It is intended for professional assessment and may not provide its own diagnosis, clinimetric score or clearance to swallow. Check what comes from the source and what is a point for further verification. Do not carry over a supplementary question as if it already describes an established finding. [3]
No. The observation report may not infer clearance to swallow or dietary advice from a single observation. It preserves what was seen, which test was carried out and which assessment was actually stated. A missing result stays unknown. Check the recorded source and the applicable decision separately; a sentence in the report that sounds logical is not a new care instruction. [4]
No. The documentation format does not assume consent or representation based on presence or family relationship. It records only what was explicitly said about this, with the speaker concerned. A statement by a partner is not automatically consent that you have established. Check the applicable situation outside the text and keep consent that was not discussed recognisable as an open point. [5]
No. Drafting, offering, sending, receiving and accepting each have their own meaning. Hysio states only the confirmed status and does not invent a recipient or acceptance. Check which action actually took place and which response is still missing. The text can support the transition, but does not carry out a handover and does not assume responsibility on behalf of another party. [6]
With a medical diagnosis that has been provided, write who it comes from and which document or contact it is based on. Hysio does not determine the type of stroke and does not interpret imaging. A nursing observation stays recognisable alongside it. Check that the summary keeps this distinction, so that a medical statement does not appear as an independent judgement by the nurse or by Hysio. [3]
The report describes which action actually followed, for example an attempt at contact or a consultation that took place, and which agreement was stated. Hysio does not choose an escalation decision or monitoring frequency. Keep attempted contact and consultation that was reached separate. Check that the report records only the action mentioned and does not already present a follow-up that is still open as carried out or accepted. [4]
Use the status that was actually stated during the discussion. A possibility of rehabilitation, a request for consultation or a question still to be answered is not a final agreement. Hysio may not write that distinction away to make the report read more smoothly. Check who picks up an action and whether a timeframe, confirmation or decision is still open. [5]
Include information from a family meeting when it is relevant to continuity of care, for example a question or follow-up agreement that was discussed. Keep it visible who provided the information. For recording the family meeting itself, the format to use is Hysio Neuro Ward Round & Family Meeting. The Neurology Nursing Handover is aimed at the receiving care team; the purpose of the document determines which information is relevant to this handover. [6]
Compare similar neurological contact notes across the whole process, from recording to checking and use in the record. Also record rework on source attribution, scores or times. A short writing time has little value when the next shift receives unclear information. Use this measurement of your own to assess the way of working, and do not expect a fixed percentage of time saved based on general figures about healthcare. [10]
Practise first with a made-up neurological observation and a family meeting in which several sources appear. Assess together whether speakers, timeline and follow-up agreements are right. Establish who checks, which data may be processed and how the text goes to the record. Only expand AI reporting once those agreements are settled in the team. [10] [9]
Hysio creates the report. You remain responsible for what you save in the record.
