Quick answer Podiatric reports for each contact 50 questions with cited sources

Quick answer

From DTP screening to therapeutic insole

Hysio fits the podiatric documentation moments: registration, history, examination, conclusion, treatment plan, delivery and closure. Examination findings, the client's perspective, clinimetrics and your professional assessment stay apart. Only information that was actually spoken ends up in the source-bound report structure.

For each contact, you can read what Hysio records about screening, foot examination and insoles, and what you assess yourself as a podiatrist.

How does it work after a foot examination?
After a foot examination or treatment contact, you no longer have to type afterwards. With voice-driven reporting, also called AI documentation, Hysio writes the report from what you say. There are four formats: consultation, intake, biomechanical examination and evaluation. [7] [8]
What stays tied to the source?
The screening, your foot and gait examination and the agreements about insoles or treatment each sit separately in the report. [1] [7]
Who determines the content?
You do. As a podiatrist, you judge for yourself what the foot and gait findings mean. The report follows what you say, and you approve it. [1] [8]
How does the report follow podiatric record keeping?
The layout follows registration, DTP screening, foot and movement examination, clinimetrics, professional conclusion and evaluation. This keeps client information, measurement data and podiatric choices separately traceable. [2] [7]
What does Hysio not do?
Hysio does not make a podiatric diagnosis, risk classification or treatment choice and does not fill in missing source information. [2] [8]
  • Documentation formats 4 profession-specific documentation formats: Hysio Podiatry Consultation, Hysio Podiatric Intake, Hysio Biomechanical Examination, Hysio Podiatric Evaluation.
  • Origin Request for help, DTP screening, foot examination and clinimetrics each keep their own source.
  • Boundaries Hysio does not add a side, FPI-6 score or insole choice that was not mentioned.
  • Responsibility The podiatrist reads the report through and chooses what goes into the foot record.

Voice-driven reporting in podiatry

When do you use Hysio Podiatry Consultation for a check-up? #

For the foot care record, Hysio Podiatry Consultation is a format for an ongoing podiatry consultation with its goal, the treatment given, clinimetrics, agreements and plan. The report runs from the summary and main goal, through treatment details, agreements with the client, details of liaison with others and the plan of approach, to clinimetrics, notable points and closure. The request for help, foot finding, measurement and your conclusion each keep their own place. A follow-up contact with an adjustment of a therapeutic insole fits here; an initial assessment belongs in the intake format. [1] [3]

What does Hysio Podiatric Intake organise at a first foot contact? #

Hysio Podiatric Intake is intended for the initial podiatric assessment with the request for help, DTP screening, history and expectations. The report runs from the Hysio summary, through Registration and route, podiatric history and podiatric examination, to analysis and treatment plan. DTP stands for direct access to podiatry (directe toegankelijkheid podotherapie); that route requires the approved screening form and the GP details in the registration phase. A side, FPI-6 score or risk class that is not mentioned stays empty and is not derived from other foot examination. [1] [4]

Which foot and movement examination suits Hysio Biomechanical Examination? #

Hysio Biomechanical Examination is for foot and movement examination with source-bound findings, clinimetrics and a podiatric conclusion in the foot care record. The report starts with the examination question and the context, describes the parts actually carried out and the results, and keeps your analysis and conclusion separate from them. Every measurement carries its side, method, value and time of measurement. The format suits a targeted first or repeat biomechanical examination, not a full intake or an evaluation of the whole course of care. [1] [5]

When do you close a care pathway with Hysio Podiatric Evaluation? #

Hysio Podiatric Evaluation is intended for an interim or final evaluation with the complaint outcome, goal attainment, the management carried out and follow-up. The summary and evaluation framework stay separate from the complaint and goal outcome, clinimetrics, the management carried out and deviations from the expected course of treatment. The client's judgement and the podiatrist's judgement each get their own place, and only then do follow-up and closure follow. An outcome you have not established stays open. [1] [6]

How does Hysio organise what you dictate about foot, side and gait? #

Within the foot care record, Hysio Podiatry Consultation organises the information from the contact you dictated. What the client tells you, what you observe during the foot examination, which measurement you take and which conclusion you state end up in different parts of the report. The origin stays visible throughout. What was not said stays empty: no side, value or conclusion is added that is not in the recording or your note. For podiatrists, voice-driven reporting therefore stays traceable to what you actually said. [1] [2] [3]

Which foot findings does Hysio never fill in itself? #

For a foot examination or a treatment contact, the report layout for foot examination and evaluation remains an ordering of your own words. Hysio does not make a podiatric diagnosis, does not assign a risk classification and does not choose a therapeutic insole, orthosis or other management. No score is calculated and no cut-off value is added. If the treatment goal and the client's consent are missing, that section stays empty until you fill it in yourself. [1] [2] [3]

How do you keep the client's story separate from your foot observation? #

In the foot care record, the podiatrist separates the request for help, DTP screening, foot examination, clinimetrics, professional conclusion and treatment agreement based on where each actually came from. What the client says about pain, footwear or load remains client information. What you see or feel yourself in the foot posture, the gait pattern or the skin is recorded separately as observation. A monofilament or tuning fork result carries its side and time of measurement. Your interpretation sits alongside as your own assessment. [1] [2] [3]

What do you check before a foot report goes into the record? #

Before saving to the foot care record, the podiatrist compares foot, side, measuring instrument and time of measurement with the original note. Is the terminology correct, is every finding linked to the right source, and is no conclusion phrased more firmly than you stated it? Missing information stays open instead of being filled in. Only then do you save the version you have read through and approved yourself. [1] [2] [6]

Does a foot report go to your own record system by itself? #

A foot care record does not automatically get a technical link with every medical record system. You transfer the report text yourself into the system your practice uses, whether that is a broad allied health package or foot-care-specific software. When doing so, check which client, which foot and side, which device and which approved version you transfer, and who the recipient is. Access, retention and version control remain a matter for your own record system. [1] [2] [6]

How does the podiatrist choose between consultation, intake, biomechanical examination and evaluation? #

For the foot care record, the podiatrist chooses between consultation, intake, biomechanical examination and evaluation based on the actual purpose of contact. Ongoing care with treatment and agreements belongs to the consultation. The first request for help with DTP screening and history belongs to the intake. A targeted examination of the foot and kinetic chain belongs to the biomechanical examination. Goal attainment and results at the end of an episode belong to the evaluation. If you are in doubt, choose the format that best describes the contact. [1] [2] [6]

From registration and screening to reporting

How do you record the screening for direct access? #

The podiatrist documents the registration and the direct access screening with the details the guideline asks for in that phase: client number, personal details, GP details and a referral letter or a completed and approved DTP screening form. Hysio Podiatric Intake keeps that registration route separate from history and examination. Note when the contact took place and which reason the client gave. If such a detail is missing from the source, that field stays empty until you have checked it. [2] [4]

How do you distinguish the request for help from the expectations in the foot care record? #

The request for help and expectations stay recognisable as client information in the foot record. The history phase asks for the reason for contact, request for help, expectations, problems with functioning and relevant medical details; these sit side by side and are not mixed together. A DTP screening, foot observation, gait or monofilament finding gets its own source, foot, side and examination date. Your conclusion only follows after that. With voice-driven reporting too, an expectation never becomes a screening outcome or diagnosis by itself. [2] [4]

Where does the foot and movement examination go in the record? #

Within the seven phases of methodical practice, you document the foot and movement examination in the third phase, the podiatric examination. That phase follows registration and history and comes before the conclusion, the treatment plan, delivery with evaluation and closure. There, the guideline asks for the most relevant examination outcomes. Hysio Biomechanical Examination preserves that order and does not derive a cause or treatment choice itself; that step remains your own clinical reasoning. [2] [5]

How does a podiatrist carry over foot clinimetrics faithfully to the source? #

Link an FPI-6, FFI or monofilament finding to the foot, side, method and date from the same examination note. The same applies to a FAOS questionnaire, a tuning fork test or an ankle-brachial index. A value from another contact is not quietly carried over. Hysio does not calculate a total score, does not fill in a missing item and does not choose a risk classification for the diabetic foot; that figure comes from the source or is left out. [2] [5]

How do you describe foot risks in diabetes without too firm a conclusion? #

In the foot care record, the podiatrist gives diabetes-related foot risks the same certainty as the source. A suspicion stays a suspicion as long as you have not established it yourself. Record the finding, for example loss of protective sensation or an indication of vascular disease, separately from the inference. In addition, the risk classification for the diabetic foot is initially determined by the GP, internist or elderly care physician; from a certain classification onwards, you carry out the follow-up checks. [2] [5]

Which context is needed for the treatment goal and consent in a foot examination or a treatment contact? #

For the treatment goal and consent, choose only context that genuinely clarifies the purpose of that examination or contact: the objective, the therapy proposal, the expected recovery and the client's consent. Add foot, side or measured value only when the source mentions them. Share purpose-bound foot data only with an established recipient and leave out background that is not relevant. Hysio organises what you have provided; you decide which context is necessary and stays readable. [2] [3]

Which measurement or source data belong with the therapeutic insole or orthosis? #

In the foot care record, link the therapeutic insole or orthosis only to a measurement or observation that was actually carried out during that contact. Record method, location or side, outcome and date as far as the source shows them. Suitable examples are an FPI-6, an FFI or FAOS result, a monofilament or tuning fork test, an ankle-brachial index or a pressure measurement, provided you actually carried them out. What the insole means for the complaint and functioning is for you to assess. [2] [3]

How does a podiatrist link the evaluation of functioning to the follow-up appointment? #

In the foot care record, connect the evaluation of functioning to a goal you agreed earlier or established professionally. Keep the client's judgement and your own apart, then note the follow-up action, who carries it out and when you will evaluate again. Hysio Podiatric Evaluation does not make a plan from loose data and does not infer goal attainment from an improved measured value. You make that link between finding and management yourself. [2] [6]

Which documentation format fits the closure of care in the foot care record? #

For the closure of care, Hysio Podiatric Evaluation is the appropriate format, because it sets out the complaint and goal outcome, clinimetrics, the management carried out and follow-up side by side. In the record, the closure phase mainly asks for the date and the reason care ended. The same report structure is intended for an interim moment, without filling in the closure. The choice and the approval stay with the podiatrist. [2] [6]

How does a podiatrist document the result and agreements when reporting? #

When reporting to those involved, the foot care record documents the outcome of the request for help, the relevant foot findings, the clinimetrics actually taken and the chosen follow-up separately. The guideline has three variants for this: a DTP report to the GP, an initial report after referral and a report on the treatment results. Each variant has its own recipient and its own minimum content. Check foot, side, time of measurement and the purpose of the transfer, and share only the version you have approved. [2] [6]

NVvP guideline, law and the foot record

Which professional rules apply to the podiatric record? #

The NVvP professional profile and the practice guideline guide foot care record keeping; as an article 34 profession, the podiatrist does not fall under article 3 of the Wet BIG. The duty to keep records itself follows from the WGBO: you record details about the client's health, the procedures carried out and other necessary information. In the record, refer to the NVvP practice guideline on record keeping with publisher, date and type of source, and keep professional standards separate from legal obligations. [7] [8] [10]

How does the guideline for podiatric record keeping organise your report? #

The revised guideline on podiatric record keeping organises registration, DTP screening, examination, conclusion, treatment plan, delivery with evaluation and closure; within this, the history forms a phase of its own. It is the Praktijkrichtlijn Podotherapeutische dossiervorming (practice guideline on podiatric record keeping) of the NVvP, amended in November 2019, with a 2024 revision in the online version. It replaced the reporting guideline from 2014. Note which document version you follow; a local practice agreement stays recognisably local. [8]

Does a podiatrist have to be in the BIG register? #

The protected training title of podiatrist (podotherapeut) does not mean mandatory BIG registration and does not in itself make you subject to statutory disciplinary law. The title is protected through article 34 of the Wet BIG, set out in the decree covering, among others, the dietitian, occupational therapist, exercise therapist and podiatrist. That differs from the article 3 professions, which do have a statutory register and disciplinary law. If you also work in another care role, you check that status separately and state only a capacity you can demonstrate. [10]

In podiatry, what is the difference between the Wkkgz and an association procedure? #

For complaints in a podiatry practice, you keep Wkkgz obligations, an association procedure and any disciplinary law apart. The Wkkgz requires every healthcare provider to have a complaints procedure and to be affiliated with a disputes body. A procedure of the professional association exists alongside this and does not replace that obligation. Record a report in the record with facts, date and follow-up, and leave the legal judgement to the designated body. [12]

How long do you keep the foot record? #

Link the retention period of the foot care record to the specific treatment agreement, the applicable legal text of the WGBO and the scope of the NVvP guideline. Record which agreement and which guideline version your retention policy rests on, so the choice is traceable later. With a new episode of care or a change in the law, you reassess that choice. Do not derive a period from the name of the profession and do not adopt a figure you have not checked in the legal text. [8] [11]

What does an allied health quality registration show about podiatrists? #

Registration under allied health quality criteria shows professional competence, but does not change the podiatrist's legal position. Record the register holder, criteria period and date of checking, and keep these separate from the status as an article 34 profession. Hours of work experience count towards re-registration, with work that is not client-related, such as record keeping, making up at most a quarter of those hours. You never present an expired or unknown registration as valid. [9] [10]

How do you record consent for a foot examination? #

For a foot examination for which consent was requested, note which answer the client gave and when that conversation took place. Keep that question linked to the intake or consultation note. A report to the GP after a DTP registration requires the client's consent in advance; for a referred client, contact with the referrer is part of the treatment agreement. Never infer consent from the mere fact that the examination was carried out. [4] [8]

What do you check before transferring foot data and device information? #

Share a podiatric report only after the care goal, recipient, necessary foot data and a secure sending route have been established. Limit the content to what that recipient needs: personal details, reason and request for help, conclusion or findings and the management started belong with the document type you choose. Check whether consent is needed and whether the destination has been verified. Only send the version you have read through and approved yourself. [6] [8] [11]

Who decides on diagnosis, foot risk and device in the podiatry record? #

Diagnosis, risk classification and treatment choice remain decisions of the podiatrist, even when a report text looks complete. Hysio organises sources and does not formulate a judgement. The podiatric diagnosis comes from your own clinical reasoning; a risk classification for the diabetic foot also comes initially from the doctor who assesses the client for it. Voice-driven admin does not change that: you decide which version becomes final. [2] [3]

How do you check the version of the NVvP profile and the practice guideline? #

With a new NVvP version, you compare the date, changed sections and target group before reusing existing record claims. Put the professional profile next to the practice guideline: the profile describes the competence, the guideline describes what you must record. Note which record section actually changed and adopt only the reviewed version and target group. If a change remains unclear, you do not use it yet. [7] [8]

Clinimetrics, foot risk and professional boundaries

Which foot measurements belong in the podiatric report? #

FPI-6, FAOS, VAS/NRS and monofilament findings are only recorded if the test or scale in question was actually used. The same applies to an FFI questionnaire, a tuning fork test or an ankle-brachial index. Link each outcome to foot, side, method and time of measurement. The guideline asks for evaluation with clinimetrics, but does not prescribe an instrument itself. Hysio does not fill in a missing value and does not calculate a risk class; you assess the meaning for functioning, device or follow-up yourself. [2] [5] [8]

How do you keep foot care terms exactly as you said them? #

In a foot examination or a treatment contact, the podiatrist uses the terminology that was actually spoken. Hysio adopts those terms and does not translate them into another profession: it remains a podiatric diagnosis, and history-taking models or code lists from physiotherapy or general practice do not belong here unmarked. The term client remains the profession's form of address. An abbreviation keeps its side, value and time of measurement. Before saving, you assess necessity, precision and readability. [2] [3] [7] [8]

What happens in the foot care record if information about the foot and movement examination is missing? #

The podiatrist leaves the relevant part of the foot care record empty when the source says nothing about the foot and movement examination. Silence never automatically becomes a negative finding: a field entry for not applicable or for nothing of note belongs only with a question that was actually asked. A side, measured value or test result that is not mentioned stays unknown. Ask a targeted follow-up question, or note in the record that this information is missing. [2] [5]

How does a podiatrist document conflicting information about foot clinimetrics? #

When information about foot clinimetrics conflicts, the podiatrist keeps source, moment and wording separate. Leave both values in place with their origin instead of choosing one yourself, and note that verification is needed. Then compare the original history, test record or measurement note. If you correct a value, that correction follows the original record and it stays visible what was changed and who did it. [2] [5]

Which boundary does a podiatrist safeguard with diabetes-related foot risks? #

With diabetes-related foot risks, the report layout for foot examination and evaluation safeguards a clear boundary: no podiatric diagnosis, no risk classification and no treatment choice. The result of a monofilament or tuning fork test remains a test result and does not automatically become a risk profile or a check-up frequency. Fluently worded text does not replace a missing source. Only you connect a foot finding to a diagnosis, a device or the management plan. [2] [3]

How do you document a red flag concerning the treatment goal and consent in the foot care record? #

A worrying foot finding or outcome from the DTP screening stays linked to foot, side, source and date of the examination. The podiatrist distinguishes what the client reports from what was observed during the foot examination and notes the agreed follow-up action. The documentation format does not choose a risk class, urgency or referral. That judgement stays with the practitioner. [2] [3]

How do you keep gait observation, clinimetrics and insole choice apart? #

In podiatric evaluation, perceived functioning, gait observation, clinimetrics and the choice of a device are not on the same footing. What the client fills in themselves in an FFI or FAOS questionnaire remains client information. What you see or measure yourself, for example an FPI-6 or a monofilament finding, carries foot, side and time of measurement. Your conclusion follows as a separate assessment. This keeps it clear what was said, what was examined and what was decided. [5]

Does Hysio calculate an FFI or FPI-6 score? #

FFI, VAS/NRS and FPI-6 remain recognisable measurement forms that were actually used, with foot, side and time of measurement. You take a score literally from the examination note; it is not calculated from other foot data and not added up to a total. Hysio also does not derive a risk classification for the diabetic foot. The conclusion also stays in text: no code list from another field is added. You check the value and decide what it means. [2] [3] [8]

How do you report fewer foot complaints without an efficacy claim? #

Fewer foot complaints, better functioning and a changed measured value are reported separately. A change the client reports stands apart from what you measured and from your own assessment. Do not write down a causal relationship you have not established: a lower pain score after a therapeutic insole does not prove an effect of that insole. Hysio provides no evidence of efficacy and no causal conclusion; interpreting the course remains your professional judgement. [2] [6]

What do you compare side by side in the final check of the foot report? #

Before saving to the foot care record, compare foot, side, FPI-6 or other clinimetrics taken with the source those data come from. Check whether every finding is under the right heading, whether the terminology is correct and whether nothing is phrased more firmly than you stated it. If information about reporting to those involved is missing, that part stays open. Remove any independent diagnosis, risk classification or treatment choice from the text before you approve it. [2] [6]

Practical questions from podiatry practices

What does voice-driven reporting cost for a foot care practice? #

For the general platform, podiatrists can expect Starter from €20 per month, excl. VAT, with Pro at €40 and Unlimited at €80. If you choose a yearly plan, you pay for ten months and get twelve. The current subscription terms are on the pricing page. Weigh the costs against the number of contacts per month and the time checking takes. There is no separate rate for voice-driven reporting in podiatry. [13]

Which GDPR agreements do you make about foot data and photos? #

In a podiatry practice, foot data are limited to what is needed for examination, treatment, device or transfer. Record who in the practice has access, with which parties you conclude a data processing agreement and by which route you send data. Under the GDPR, foot, side, measurement outcome and any orthosis data stay linked to the right client and source. The podiatrist checks access and recipient before saving or sending; that judgement is not taken over. [14] [15]

Which alternatives to voice-driven reporting suit foot care? #

For the foot care record, the podiatrist compares writing by hand with dictation. Writing by hand takes time, but keeps the structure in your own hands. Dictation produces running text without fixed headings, so you still have to organise it yourself afterwards. A general chatbot does not know the phases of podiatric record keeping and can fill in a side, score or conclusion that nobody stated. A profession-specific documentation format does organise according to those phases. The practice decides for itself which way of working fits. [2]

How do you assess the 14-day trial with check-up consultations? #

For a podiatry practice, the general Hysio trial lasts 14 days. Start with one recognisable type of contact, for example a DTP registration or a follow-up consultation. Agree in advance who reviews the text and on what: are foot and side correct, is every measurement linked to the right source and how much correction is still needed? Then assess whether the terminology stays recognisable and whether the report can go into the record without additions. [13]

Do foot and orthosis data go to every record system automatically? #

No, an automatic transfer to every record system is not guaranteed. You transfer the approved report text yourself into your practice's package, and that remains human work. Record a diabetes-related foot detail as it was reported or examined, with foot, side, method and date, so that nothing is lost in that transfer. Then check client, foot, side and version, and agree within the practice who does that step. [2]

Who reads the foot report through in the practice? #

The podiatrist compares the report text with the DTP information, foot findings and clinimetrics actually used. Diagnosis, diabetic foot risk, the choice of an orthosis and referral are never determined by the documentation format. The practice records who saves the reviewed version. Only after professional assessment may foot, side, conclusion and agreed follow-up go together into the final foot care record. [2] [3]

How do you correct an error in the details of a therapeutic insole or orthosis? #

An error in foot, side, clinimetrics or orthosis data is corrected using the same examination or device source, not from memory. Keep the original record and check that diagnosis, risk class and follow-up do not change unintentionally because of the correction. Note who made the change, when and why. The foot care record then still shows which version counts as final. [2] [3]

What does a podiatrist do with incomplete source information about the evaluation of functioning? #

When information about functioning is missing at the evaluation, the podiatrist first looks at the client-reported source from the same time of measurement, for example a completed FFI or FAOS questionnaire or the evaluation note. A gait observation or a check of the insole does not replace the client's judgement. If that source does not exist, you ask a targeted follow-up question or the result stays open. Hysio does not invent an outcome. [2] [3]

How do you measure where the report time goes in a podiatry practice? #

The podiatrist measures dictation, checking of foot, side and clinimetrics, correction work and transfer to the record separately, because only then can you see where the time goes. Compare your own practice before and after. Start with the check-up consultation, which occurs most often, and with the intake, which is the longest. There is no national figure. So you only know what admin with AI saves you after your own measurement. [2]

Which podiatric contact do you start with when introducing it? #

For the introduction, first choose one recognisable podiatric contact, for example a DTP registration or an evaluation. Appoint a reviewer and describe how they check foot, side, clinimetrics, conclusion and saving. Record which terminology you follow and which fields may stay empty. Only after a successful source and record check do you add the next type of contact. [2]
