Quick answer Five documentation formats for your physiotherapy record 50 questions with cited sources

Quick answer

From treatment conversation and examination to your physiotherapy report

Choose Consultation, Intake, Anamnesis, Examination or Walk-in Clinic; each format has fixed sections. Hysio was conceived by a physiotherapist. In testimonials on hysio.nl, physiotherapists say their record keeping has become easier.

A follow-up treatment calls for a different report than a first intake or a separate examination. Choose the format that fits the contact and name out loud what is not audible. Hysio is built to be the most reliable choice for voice-driven reporting: the report follows your professional language and you are the last to check it.

What does Hysio make of your treatment contact?
An AI scribe for your treatment contact, from history-taking and examination to treatment plan: that is voice-driven reporting with Hysio. You speak and name your findings, and Hysio turns them into a report in SOAP or in the intake structure. You read it through before it goes into your record. This way of working is also called AI documentation. [4] [5] [6] [7] [8]
Does the consultation use SOAP?
Yes. The consultation consists of Summary, Subjective, Objective, Assessment and Plan. [4]
Does KNGF prescribe SOAP?
No. The guideline determines which data are relevant, but leaves the form and place of recording to the physiotherapist. [10] [11]
Does Hysio fill in missing measurements?
No. Values, results and conclusions that were not spoken are not added as fact. [4] [7]
Where does responsibility lie?
The physiotherapist remains responsible for the content and decides how and where the record is kept. [12]
Hysio Supplementary
Supplementary considerations, where available, are shown recognisably separate from the record sections. [5] [6] [7] [8]

Voice-driven reporting in physiotherapy

Which documentation formats does Hysio offer for AI reporting in physiotherapy? #

Hysio has five physiotherapy reports for different care encounters: Consultation, Intake, Anamnesis, Examination and Walk-in Clinic. The consultation organises a treatment contact in SOAP, while the other formats fit a full intake, a separate interview, a separate physical examination or a short advice contact. That way, with voice-driven reporting, you choose the documentation format based on what actually happens during the contact. [4] [5] [6] [7] [8]

Can Hysio listen in the background during a physiotherapy contact? #

Yes. Start Document within the Hysio session and let the recording run during the care encounter while you treat. You do not need to recount a treatment consultation in full afterwards. In a physical examination, findings that cannot be heard do still depend on what you name out loud. Hysio cannot infer a range of motion, test result or treatment response from an action that was only visible in the treatment room. [3] [1] [4] [7]

Do I have to dictate my whole physiotherapy report? #

No. During a conversation, the recording can keep running within the Hysio session while you focus on the patient. During an examination, briefly naming things out loud is valuable, because palpation, ranges of motion, resistance tests and treatment techniques do not become clear from the audio by themselves. You can therefore combine recording and examination dictation: treat where you can, and name things deliberately where a clinical finding would otherwise be lost. [3] [1] [4] [7]

Do I get a transcript or a structured physiotherapy report? #

You get a structured report, not a word-for-word dialogue as the end product. Each documentation format has fixed, profession-specific sections. The consultation uses Summary, Subjective, Objective, Assessment and Plan. The intake separates history-taking, examination, analysis and care plan. As a result, patient information sits in a different place from examination findings, and conclusions and agreements that were spoken stay recognisable in their own report section. [4] [5] [6] [7] [8]

Does Hysio fit different ways of working within physiotherapy? #

Yes, because you do not use the same template for every contact. A regular session fits Consultation, a first examination with history-taking and care plan fits Intake, a telephone interview fits Anamnesis and a separate physical examination fits Examination. For a short contact at the club, in the practice or by telephone, there is the Walk-in Clinic format. Within a documentation format, the content follows your own way of working, because only what you and the patient actually name is recorded. The content always stays limited to the chosen scope of the contact. [4] [5] [6] [7] [8]

Does the report go into my medical record system automatically? #

This page does not promise automatic saving or a direct integration with any specific medical record system. The report stays in your Hysio session until you transfer it. You read it through, correct what is not right and then place the parts you need in your own record. That way you decide the moment, the text and the fields that go into the record, not the platform. [4] [5] [6] [7] [8]

What should you look for when choosing AI for voice-driven reporting in physiotherapy? #

Look above all at how well contact and report match: does a treatment consultation really use SOAP? Can an intake separate history-taking and examination? And does the tool never fill in measured values, sides or test results without a source? Also check whether the platform makes clear what it does not do, how you use the result in your record and where current privacy and security information can be found. A long feature list matters less than reliable, profession-specific boundaries. That test applies to any AI tool, whether the supplier calls it a scribe, a dictation aid or an assistant. [4] [5] [7] [18]

Does Hysio independently establish a physiotherapy diagnosis or triage outcome? #

Not as an independent diagnosis or triage outcome. Intake and Examination may formulate a working hypothesis based on findings stated elsewhere in the report, with visible reasoning. That is source-bound synthesis, not a medical diagnosis. In the Walk-in Clinic format, the conclusion, the gut-feeling judgement (pluis or niet-pluis), urgency and route in the record sections stay limited to what the physiotherapist says. Any additional considerations appear separately under Hysio Supplementary and never as established fact. [5] [7] [8] [11]

What does voice-driven reporting cost per physiotherapist? #

The current individual plans start at €20 per month, excl. VAT. The pricing page states that all professions and all AI assistants are included in each of the three individual plans. The five physiotherapy reports therefore do not require a separate purchase. With yearly billing, you get two months free. Check the pricing page again when you choose, because prices, plan capacity and conditions for a practice or team may change. [9] [4] [5] [6] [7] [8]

Can I try Hysio with physiotherapy contacts first? #

Yes. The current pricing page offers a free trial of 14 days, and all five physiotherapy reports are available during that period. Test, for example, a treatment consultation, a full intake and a contact with concrete measurements. That way you can judge in different situations whether SOAP, the separation between patient statement and finding, the examination findings and the profession-specific terminology fit your daily documentation. [9] [4] [5] [6] [7] [8]

Five physiotherapy reports for five care encounters

What does a SOAP note from Hysio Physiotherapy Consultation look like? #

Hysio Physiotherapy Consultation turns a treatment consultation into a report with Summary, Subjective, Objective, Assessment and Plan. Subjective contains what the patient reports; Objective contains named observations, examination and treatment; Assessment contains the spoken conclusion or source-bound synthesis; Plan contains the next steps discussed. Values or clinical conclusions that were not mentioned are not added as fact. [4]

Which sections does an intake report from Hysio Physiotherapy Intake include? #

Hysio Physiotherapy Intake organises a full intake into Summary, History-taking, Examination, Analysis and Care plan. The history-taking contains the request for help, history, impairments and limitations; the examination contains named inspection, palpation, movement, tests and clinimetrics. The analysis connects findings supported by the source, and the care plan contains the goals, strategy, frequency and evaluation discussed. Any suggestions stay separate under Hysio Supplementary. [5]

What goes on the history-taking card from Hysio Physiotherapy Anamnesis? #

Hysio Physiotherapy Anamnesis turns a separate interview into a summary and a history-taking card with Request for help, History, Impairments and Limitations. This documentation format can also be used for a history-taking conversation by telephone. It stays within the subjective information of that contact and does not pretend that a physical examination took place. Possible follow-up questions can appear separately under Hysio Supplementary. [6]

What does Hysio Physiotherapy Examination make of the findings you name out loud? #

Hysio Physiotherapy Examination turns an examination that you talk through out loud into a summary, examination card and analysis. Only actions, findings and values that were actually named go into the factual sections. The Analysis may formulate a working hypothesis based on those findings and states that a full history is missing. Hysio Supplementary can show source-supported considerations separately, phrased as considerations. The documentation format contains no care plan and does not make a medical diagnosis. In the examination room, voice-driven reporting means: you name the test, side and outcome, and Hysio puts them on the examination card. [7]

Which report does Hysio Physiotherapy Walk-in Clinic make of a short contact? #

Hysio Physiotherapy Walk-in Clinic organises a short contact into Summary, Presenting complaint, Findings, Conclusion and advice, plus Follow-up and safety net. In these record sections, conclusion, advice and safety net stay limited to what the physiotherapist says or discusses. Hysio does not carry out triage. Where useful, Hysio Supplementary can separately show a source-supported consideration for further questioning, examination, safety net or follow-up. It does not replace a full intake. [8]

What is the difference between the physiotherapy intake and the separate anamnesis? #

The Intake covers the whole diagnostic contact: history-taking, examination, analysis and care plan. The separate Anamnesis covers only the interview and organises the request for help, history, impairments and limitations. Choose the separate format when the physical examination takes place later or in another contact, for example after a history-taking by telephone. That way there is no false suggestion that examination, analysis or a care plan has already been fully carried out. [5] [6]

When do you use the separate physiotherapy examination report? #

Use Examination when the physical examination is a contact of its own or a clear next step, for example after an earlier history-taking by telephone or for a targeted re-examination. This examination report focuses on the region examined, actions, findings, measurements and analysis. History-taking content and the policy discussed do not belong unnoticed in the examination card. That keeps it clear which information was gathered during which part of the diagnostic process. [7]

Is there a separate Hysio report for final evaluation or handover? #

No, this physiotherapy set currently includes five contact reports and no separate Hysio report for Final Evaluation and Handover. The professional guideline recognises a final evaluation as a record phase of its own, but that does not make it an available Hysio report. This page therefore names only Consultation, Intake, Anamnesis, Examination and Walk-in Clinic and makes no promise of availability for a sixth documentation format. [2] [10]

How do I have a physiotherapy intake written up automatically into an intake report? #

Start the Physiotherapy Intake documentation format and record the intake conversation within the Hysio session. During the physical examination, name out loud what you do and find, because those findings do not become clear from the conversation. Hysio then organises the source information into Summary, History-taking, Examination, Analysis and Care plan. Only data that were spoken or provided go into the report; missing measurements and conclusions are not added as fact. The sections can be copied separately into your own record process; Hysio itself saves nothing in your record system. [1] [3] [5]

Which Hysio documentation format do I choose for a follow-up consultation or re-examination? #

Choose Physiotherapy Consultation when treatment, course of the complaint, evaluation and follow-up are discussed together in a treatment contact. Choose Physiotherapy Examination when a targeted examination or re-examination is the central, stand-alone step. That second format focuses on named actions, findings, measurements and analysis and contains no care plan. That keeps it visible whether the source describes a treatment consultation or a separate examination. In AI reporting, that choice is your first step, because the documentation format determines the sections in which Hysio organises the report. [4] [7]

Recording during conversation and examination

Which examination findings should I name out loud? #

Name above all information that cannot be heard from the conversation: the side examined, active or passive movement, the exact measured value with its unit, a positive or negative test result, a palpation finding, the treatment carried out and the response. Hysio must not turn an action without a spoken result into a result. Naming things briefly and naturally is enough, for example the test name, side and outcome in one sentence. [4] [7] [18]

Can Hysio record a physiotherapy history-taking by telephone? #

Yes. The Anamnesis documentation format is also specifically intended for an interview by telephone. It organises what the patient tells you into a summary and the sections Request for help, History, Impairments and Limitations. Because no physical examination needs to take place in that contact, Hysio does not add any examination findings. A later examination can be recorded separately with the Examination documentation format. [6] [7]

How are the patient's statements and the physiotherapist's findings kept apart? #

Patient statements go under Subjective or History-taking; your observations, measurements and actions under Objective or Examination. Your spoken interpretation belongs under Assessment or Analysis. That way a patient statement does not silently become an objective finding. If it is unclear who said something, Hysio should preserve that uncertainty so that you can check the passage. [4] [5] [6] [7] [8]

What happens if information is missing during the care encounter? #

Missing information is not filled in as fact. The documentation format uses a fixed fallback, leaves out a non-mandatory section or states what was not documented verbally. Separately, Intake and Examination may, where there is enough coherence, formulate a working hypothesis based on recorded findings and show the reasoning. If the data are too thin, no reliable synthesis follows. Without a spoken conclusion, the Walk-in Clinic format does not draw a conclusion of its own in the record sections. [4] [5] [6] [7] [8]

Does Hysio fill in a measured value if I do not say it? #

No. ROM degrees, MRC scores, pain scores, test results and other values must be evident from the permitted source. If you do mention that an examination was carried out but give no result, the report may only record that the specific value was not documented verbally. Hysio does not calculate, round or extrapolate such a missing number, nor does it turn it into a comparison with an earlier measurement. [4] [5] [7] [18]

How do I use the Hysio report in my own record? #

Copy the sections that are relevant to your contact into the matching fields in your record. For a consultation these are, for example, Subjective, Objective and Plan; for an intake, History-taking, Examination and Care plan. Before saving, check the text for speaker, side, measured value, unit and conclusion. You choose which information belongs to this contact and where you keep it. [4] [5] [6] [7] [8] [11]

Who remains responsible for the physiotherapy record? #

The physiotherapist remains responsible for the content saved in the record. The KNGF guideline also leaves it to the physiotherapist to decide how and where the record is kept. [12]

Hysio organises the recorded source information into suitable report sections and does not add any finding, value or conclusion to it. You assess what you save yourself before it goes into the record; the professional judgement and the choices for the record do not shift to the platform. [4] [5] [6] [7] [8] [12]

Does Hysio automatically turn the patient's words into a physiotherapy finding? #

No, not without a source. The patient may say that something shoots, feels weak or does not work; such a statement stays subjective information until you state a physiotherapy finding or interpretation. Hysio can phrase language professionally and concisely, but must not independently change a lay description into a test result, diagnosis or objective limitation. That distinction protects the clinical meaning of the report. [4] [5] [6] [18]

Does Hysio work directly with my physiotherapy record? #

This page does not demonstrate a direct integration with a physiotherapy record. So there is no setting with which Hysio itself opens, fills in or updates your record system; you transfer the text yourself. If you want to know whether an integration exists for your practice, ask the supplier of your record system and check Hysio's current product documentation. In the end, the physiotherapist decides where and how record keeping takes place. [4] [5] [6] [7] [8] [11]

How much do I still need to correct a physiotherapy report from Hysio? #

There is no fixed correction percentage for every physiotherapy contact. Before saving to the record, check speaker, side, measured value with unit, test result, treatment, evaluation and plan against the conversation and the findings you named. Pay extra attention to actions that were only visible in the treatment room. Only transfer the text when it matches the contact. [4] [5] [6] [7] [8] [18] [12]

KNGF guideline and the physiotherapy record

How does Hysio align with the KNGF-richtlijn Fysiotherapeutische dossiervoering? #

Hysio uses profession-specific report sections that fit data from the physiotherapy process, such as the request for help, functioning problems, examination, plan, course of the complaint and evaluation. The Intake organises history-taking, examination, analysis and care plan; the Consultation organises course of the complaint, observations, evaluation and follow-up. That way the data the guideline lists per record phase sit in a recognisable place, without Hysio claiming that KNGF prescribes one fixed template. For voice-driven reporting, this means that what you name in a contact ends up in a recognisable report section. [10] [11] [4] [5]

What is the difference between Hysio's SOAP and S-O-P-A in the KNGF guideline? #

The Hysio consultation uses SOAP: Subjective, Objective, Assessment and Plan. For the treatment phase, the KNGF summary notes S, O, P and A, where P stands for the physiotherapy procedures carried out or a reference to the treatment plan, and A for analysis or evaluation. Note that difference: in the Hysio consultation the treatment carried out is under Objective, while Plan contains the follow-up discussed. That is allowed, because the guideline prescribes data but leaves the form, place and combination of recording to the physiotherapist. [4] [10] [11]

Which five phases does the KNGF guideline distinguish in the physiotherapy record? #

The guideline organises record keeping into five phases: registration and intake, physiotherapy examination and physiotherapy diagnosis, treatment plan, treatment, and final evaluation, which the guideline links to a contact at which the patient is present. The Hysio documentation formats are not a one-to-one copy of those phases, but cover different care encounters within that process. Intake, for example, combines history-taking, examination, analysis and care plan; Consultation focuses on a treatment contact. [10] [4] [5]

Can Hysio record a DTF screening outcome? #

Hysio can record a screening conclusion, advice, follow-up and safety net spoken by the physiotherapist within the appropriate contact report. The platform does not determine the DTF (direct access) outcome independently. The KNGF explanatory notes distinguish four follow-up profiles: a recognisable profile within one's own competences, a non-recognisable profile, a recognisable profile with possible serious pathology and a recognisable profile that is better suited to another care provider. Only the physiotherapist makes that clinical classification, and pluis or niet-pluis (the Dutch gut-feeling terms) only appear in the report when the physiotherapist uses those words themselves. [10] [11] [8]

Which data does the KNGF guideline list for a treatment contact? #

For the treatment phase, the KNGF summary lists Subjective course of the complaint and Plan, meaning the physiotherapy procedures carried out or a reference to the treatment plan, as data to be noted by definition. Objective observations and Analysis or evaluation are added when they are relevant. The Hysio consultation records those four types of information in SOAP order, insofar as the treatment contact supports them. What ultimately appears therefore depends on what was actually discussed, examined and carried out. [10] [4]

Which consent should be recorded in a physiotherapy record? #

The relevant consent depends on the contact. For DTF, the KNGF summary mentions consent for consultation with, or requesting data from, the doctor, and for examination or treatment plan, consent for a reserved or special procedure. External reporting also requires an appropriate basis of consent. Hysio may only note consent that is evident from the care encounter or another permitted source. [10] [11] [5] [7] [8]

Is a final evaluation part of physiotherapy record keeping? #

Yes, the KNGF guideline recognises final evaluation as the fifth record phase and mentions evaluation of the treatment result when the patient is present. Within the current Hysio physiotherapy set, however, there is no separate final evaluation report. This page keeps those two facts apart: the profession's own record includes that phase, but Hysio does not present it as though a separate available report already exists for it. [10] [2]

Is Hysio a report template prescribed or approved by KNGF? #

No, this page does not claim that. The KNGF guideline does not prescribe a mandatory SOAP or Hysio template and leaves the way, place and combination of recording to the physiotherapist. Hysio aligns in content with recognisable record data and physiotherapy care encounters, but does not say that the platform has been approved, assessed or prescribed by KNGF without a separate, explicit basis for that. [10] [11]

Which record data must a physiotherapist always note, and which only when relevant? #

For each record phase, the KNGF guideline distinguishes between data that are noted by definition and data that are only needed for a relevant finding or circumstance. At registration and intake, the request for help and the functioning problems are always included, while environmental and personal factors are only noted when relevant. With direct access, the screening conclusion, the indication for further examination and the associated consent are included by definition. The physiotherapist decides which content is ultimately relevant to the record. [10] [11] [12]

How does a Hysio intake relate to the KNGF record phases? #

The KNGF guideline distinguishes five record phases, while the Hysio intake is one contact report. That intake organises History-taking, Examination, Analysis and Care plan and so touches several steps of the physiotherapy process. That does not mean that one Hysio report replaces all five record phases. Treatment and final evaluation remain separate moments, and the current physiotherapy set contains no separate Hysio report for final evaluation or handover. [10] [5] [2]

ICF, measurements, terminology and clinical boundaries

How does Hysio use ICF in a physiotherapy report? #

ICF helps to organise functioning problems as impairments in body functions or structures, limitations in activities and restrictions in participation. The KNGF explanatory notes recommend this framework, but do not make it a mandatory template. In the Hysio intake and anamnesis, this structure mainly supports the distinction between impairments and limitations. Hysio does not add any ICF problem that does not follow from the source information. [11] [5] [6] [18]

Can Hysio include PSK or PSFS in the physiotherapy report? #

Yes, when the chosen activities and scores are spoken during the contact or are available as a permitted source. PSK or PSFS describes activity problems chosen by the patient and records a score per item. Hysio transfers those values to the appropriate section, but does not invent any activity, total score or clinical interpretation. If a score is missing, it also stays missing in the report. [16] [18] [4] [5] [7]

Can Hysio include a TSK score in the physiotherapy report? #

Yes, when the TSK outcome is present as source information. The Tampa Schaal voor Kinesiofobie (Tampa Scale for Kinesiophobia) is a patient questionnaire for pain-related fear of movement. Hysio can record the score mentioned, but does not add any cut-off value, norm, change or meaning that does not come from a validated source or from what the physiotherapist says. A score therefore does not automatically become a clinical conclusion. [17] [18] [5] [7]

How are ROM, MRC and NPRS noted in a Hysio report? #

Exactly as you name them, with side, value, scale or unit and the measurement context if you name it. ROM, for example, is recorded in degrees, MRC as a strength score and NPRS as a pain score. Hysio does not calculate a difference and does not create a trend when an earlier value is missing. Nor are cut-off values, MCID, MDC or other interpretation norms added from general model knowledge. [18] [4] [5] [7]

Does Hysio name red flags or a safety net by itself? #

Hysio does not classify a signal as a red flag by itself and does not determine a pluis or niet-pluis (gut-feeling) outcome. The record sections contain only what the physiotherapist says, advises or agrees about it. The Intake includes only explicitly mentioned flags as fact. The Walk-in Clinic can separately show a source-supported consideration for safety net or follow-up in Hysio Supplementary, phrased as a consideration. That is not patient advice or an urgent route, and it does not replace the professional judgement. [11] [5] [8]

When does a diagnosis or working hypothesis appear in the Hysio report? #

That depends on the documentation format. In Assessment, Consultation only connects source facts and includes a spoken conclusion, attributed to its speaker, without a working hypothesis of its own. Intake and Examination may formulate a working hypothesis based on recorded findings and show the reasoning. Intake connects history-taking and examination; Examination visibly states that a full history is missing. An alternative consideration only appears separately under Hysio Supplementary and never as a diagnosis or established fact. [4] [5] [7]

Where can I find reliable privacy information about Hysio for physiotherapy? #

The privacy policy describes how Hysio processes personal data, including purposes, retention periods, recipients and rights. The data processing agreement describes the arrangements between the practice and Hysio. The legal hub brings these and other current legal documents together in one place. [13] [14] [15]

Health information falls under the special regime for special categories of personal data. So read the current documents yourself for product details that may change; this professional page does not repeat any individual retention period, deletion promise or certification claim. [11]

How reliable is voice-driven reporting with physiotherapy terminology and abbreviations? #

There is no general accuracy percentage for every physiotherapy term, speaker or recording. Check above all left and right, ROM, MRC, NPRS, test names and units in their context. A small difference can change the meaning. Hysio must not independently turn patient language into an objective finding and must not make up missing values. So treat admin with AI as help with the writing; checking the terminology remains your job. [18] [4] [5] [6] [7] [8]

Can Hysio compare measurements from several moments in a physiotherapy report? #

That is only possible when the source captures the individual measurements clearly enough. For each measurement, name the side, value, unit and the moment or order. Hysio can then faithfully record what was measured and which comparison you state. The platform should not calculate, round or extrapolate a missing number, and without a source it draws no conclusion of its own about improvement or deterioration. [4] [5] [7] [18]
