Quick answer
Six formats for treatment, examination, evaluation and psychosomatic support.
Six documentation formats cover the treatment contact, the intake, the posture and movement examination, the evaluation and the two psychosomatic contacts. The layout shows how the report structure can match what you discuss, observe or evaluate.
Exercise therapy has six formats: consultation, intake, posture and movement examination, evaluation, psychosomatic consultation and psychosomatic intake. Which format fits follows from the contact you actually carried out. That way a treatment session stays a daily record, an examination contact an examination report and an evaluation conversation an evaluation. You assess posture, movement, professional interpretation and the final entry in the record. With voice-driven reporting, each exercise therapy contact therefore gets its own documentation format. [2] [3] [4] [5] [6] [7] [9] [10]
This format turns a treatment contact into an exercise therapy daily record with a compact summary above it. The client's experience, the procedures carried out, the evaluation by client and exercise therapist and the agreements until the next contact stay clearly separate from one another. The SOAP structure is a practical aid for the daily record here, not a requirement of the VvOCM guideline. [2] [10] [8]
This format organises a first conversation along the ODIM line: from registration and history via activity examination and specific examination to the exercise therapy analysis and the agreed treatment plan. Goals, phasing and evaluation stay exactly as concrete as the exercise therapist stated them. The plan section keeps the main goal, subgoals, strategy, evaluation points, frequency and the client's discussed consent together. [3] [8] [9]
Hysio Exercise Therapy Posture and Movement Examination records a separate examination contact with activity examination, specific examination, posture and movement observations and an exercise therapy analysis. Clinimetrics gets its own place as soon as you have named the instrument, moment and outcome. There is deliberately no treatment plan: this is an examination report, not a full intake. History or policy that is nevertheless in the source stays outside the examination sections. [4] [9] [10]
This format turns an evaluation conversation during or at the end of a course of treatment into a report of goals, product result, course of the process and follow-up. The product evaluation covers goal attainment and the judgement of client and exercise therapist; the process evaluation covers content, organisation and the way the client was treated. The reason for closure, aftercare and relapse policy are only included when you have discussed them. [5] [10] [8]
This format turns a psychosomatic treatment contact into a daily record in SOAP structure, with a compact summary above it. It sits alongside the regular consultation and is intended for psychosomatic practice. What the client reports, what the exercise therapist observes and carries out, the evaluation by both and the agreements until the next contact stay separate; a track or dimension that was not addressed stays out of the report. [6] [8] [9]
The intake layout organises a first psychosomatic conversation according to SSCEGS: soma, stressors, cognitions, emotions, behaviour and social consequences. This is followed by recorded findings on tension, breathing and body awareness, an explicitly stated working diagnosis and the treatment plan on two tracks. Dimensions or tracks that were not addressed stay outside the report. Hysio does not derive an explanation or treatment choice from isolated words. [7] [9] [10]
The consultation report is the format for an ongoing treatment session. Note the request for help, what you observed, which exercise therapy intervention was carried out and which response or agreement followed from it. The exercise therapist reviews the text and decides what is recorded according to the local record-keeping agreements. [2] [10] [8]
For a treatment session the consultation fits, for a first conversation the intake, for a separate examination contact the posture and movement examination and for a discussion of goals the evaluation. For each contact, record the request for help, observation, chosen format and agreement. The exercise therapist checks the source and decides what is included in the record. [2] [3] [4] [5] [6] [7] [10] [8]
Yes. The psychosomatic intake is a separate documentation format within the exercise therapy profile and stands apart from the regular intake. It organises soma, stressors, cognitions, emotions, behaviour and social consequences, whereas the regular intake follows the ODIM line. The examination focuses on tension, breathing and body awareness. Hysio does not turn this into its own psychosomatic interpretation and does not independently fill in parts that were not discussed. [7] [8] [9]
The specialist register distinguishes four recognised directions: psychosomatic exercise therapy, paediatric exercise therapy, pelvic exercise therapy and geriatric exercise therapy. Each direction has its own post-bachelor (post-hbo) or master's programme and its own register annotation. Since at least 2006, the profession has been directly accessible, meaning without a referral. Assess the exact registration requirements and competences against the current professional source before you use them in practice information or a record. [1] [8] [9]
Within the methodical approach, an evaluation point is part of step 7, with closure as step 8. Record the date of the final evaluation, the reason for ending care and the final report that was sent. Treatment result, objective measurement result, process, relapse policy and aftercare are part of this when you have discussed them. Only use measurement instruments that were actually administered and recorded. [1] [9] [10]
For long-term treatment, the guideline recommends an annual progress report to the general practitioner (GP). Describe the current course, the goals assessed, relevant outcomes, your professional conclusion and the agreed follow-up. For this, the VvOCM guideline refers to HASP-paramedicus; that set of agreements is being revised, so check the current version. Keep the report concise and only record information that has actually been assessed. [1] [10] [8]
When a treatment episode closes, the final report describes the result achieved, the follow-up, any aftercare and the policy discussed for when complaints return. The guideline makes that report mandatory at the end of the course of treatment and keeps it concise. After a referral it goes to the referrer; with direct access, to the GP. Only record what was assessed at closure and discussed with the client. [1] [8] [9]
The VvOCM guideline from 2022 states that uniform record data can also form a basis for national care registries. The guideline mentions the Landelijke Database Oefentherapie (national exercise therapy database); that submission now runs under the name Nivel Zorgregistraties Eerste Lijn. Unambiguous definitions and traceable source information are therefore important. A registration need may never add a care fact that was not discussed to the record. [1] [9] [10]
The VvOCM guideline names research into administrative burden as the reason for the 2022 update. Exercise therapists indicated a need for a lighter burden, with a view to billable time and job satisfaction. That reason does not change the substantive standard: the record must reflect the care process in a traceable way and the healthcare professional remains responsible for completeness, accuracy and the chosen wording. [1] [10] [8]
Exercise therapy and physiotherapy are both directly accessible and treat complaints of the musculoskeletal system, which is why clients and referrers sometimes confuse them. Within its own professional profile, exercise therapy places emphasis on posture, movement, behaviour and application in everyday activities. So describe concretely which contact, examination or intervention was carried out and do not use terminology from another profession when the source does not support it. [1] [8] [9]
The documentation formats safeguard the active, behaviour-oriented character of exercise therapy. They should not add any passive intervention, physiotherapy jargon, examination that was not carried out or undiscussed treatment proposal. Describe posture, movement, everyday activities and self-management only from the recorded source. That way the distinction from another allied health profession stays visible without filling in the professional assessment for the exercise therapist. [1] [9] [10]
A separate documentation format has been designed for the posture and movement examination within exercise therapy. It focuses on the examination context, observations, tests carried out, any clinimetrics and a separate analysis, without a treatment plan. A consultation, by contrast, records the treatment contact, the evaluation and the follow-up. Hysio does not choose an examination situation itself from isolated words. [4] [9] [10]
The evaluation brings together goals, product result, course of the process, exercise therapy assessment and follow-up agreements. The evaluation does not replace the daily record and does not create a new measurement outcome when none was mentioned. What the client says about change in complaints stays separate from your judgement of change in behaviour. A reason for closure or an aftercare agreement is only included when you have discussed it. [5] [10] [8]
The VvOCM guideline places exercise therapy record keeping within several legal and professional frameworks. These include the treatment relationship, the duty to keep records, privacy, information exchange and the responsibility for careful methodical practice. Use the guideline to organise relevant data, but assess current legislation and local agreements separately. You remain responsible for the accuracy, the necessity and the final place of information in the record. [1] [10] [8]
The underlying guideline is the Richtlijn Verslaglegging (documentation guideline), published by VvOCM (Vereniging van Oefentherapeuten Cesar en Mensendieck), updated version spring 2022, copyright 2022. The guideline was presented to the VvOCM board for approval on 11 March 2022 and adopted by the members at the General Meeting (AV) on 15 June 2022. [1] [8] [9]
The guideline is not a binding rule but a professional standard within the meaning of the WGBO (the Dutch Medical Treatment Contracts Act). An exercise therapist may deviate with justification, for example with multimorbidity or when parts of the guideline no longer match current scientific insights; a deviation from a guideline is, however, then noted in the record. [1] [9] [10]
The additional history deepens the health problem, the complaints and the request for help. Organise this information according to the ICF domains and link it to the steps of the exercise therapy methodical approach. Keep what the client reports separate from your observations and analysis. Personal or environmental factors that were not discussed are not added to make the scheme look more complete. [1] [10] [8]
The activity examination describes task, environment and movement behaviour; the specific examination records targeted observations, physical measurements and tests of the movement system that were carried out. This can include strategy, load and load capacity, coordination, strength, flexibility, muscle tone, relaxation and breathing, among other things. Only note parts that were actually examined and link each outcome to its source and moment of measurement. [1] [8] [9]
The exercise therapy diagnosis is the profession-specific judgement based on your clinical reasoning process. The mandatory elements are the client's details, such as sex, age, referral diagnosis or complaint and request for help, plus the findings from history and examination together with your analysis and conclusion. Factors that hinder and promote recovery and a prognosis, ranging from full recovery to not determinable, belong to it when you have established them. [1] [9] [10]
A treatment plan contains a concrete main goal, suitable subgoals, the chosen treatment strategy and agreements about evaluation points and criteria. Also record relevant aids, guidelines used, justified deviations, prognosis, treatment duration and frequency when these have been discussed. Make agreements with the client and other disciplines traceable and note the client's consent according to the applicable record-keeping procedure. [1] [10] [8]
The evaluation consists of a product evaluation (the extent to which goals have been achieved, the client's judgement of change in complaints, the exercise therapist's judgement of change in behaviour, with measurement instruments) and a process evaluation (how the methodical approach went in terms of content, organisation and the way the client was treated). For the product evaluation, note which instrument you used and at what moment you measured. A goal you have not evaluated stays visibly open. [1] [8] [9]
The exercise therapy reporting line connects to the contact and follows the ODIM steps: registration, history, examination, exercise therapy diagnosis, treatment plan, treatment, evaluation and closure. Each documentation format takes one part of that. The guideline helps determine which data are relevant, while you remain responsible for the choice, the completeness and the place of the entry in the record. [2] [3] [5] [8] [9]
First describe what you observed, heard or measured and link it to the source of the information. Then put a professional interpretation in the analysis or evaluation section intended for it. Hysio does not silently slip a suggestion in between observations. That way, with voice-driven reporting too, an explanation that was not stated stays out of the report. [4] [5] [9] [10]
In practice, exercise therapists use many abbreviations that the guideline itself also uses as a fixed list of abbreviations: ODIM, DTO, AO, SO, OTD (exercise therapy diagnosis), ICF, DCSPH, RPS, KP, PPN, EBP, NIVEL. If you say such an abbreviation aloud, check that it appears in the text as a fixed term and not as separate letters. An abbreviation you did not mention is never added afterwards. With voice-driven admin, it pays to say an abbreviation calmly and in full. [1] [9] [10]
The guideline contains one concrete instruction about what an exercise therapist explicitly records in the history, and therefore also says aloud: the type and amount of medicines that can influence the recovery process or the choice of interventions. This falls under personal factors within the ICF structure. When reading back, check the name of the medicine, the unit and the frequency; misheard doses are a real risk. [1] [10] [8]
Within exercise therapy documentation, the ICF serves as a textual framework for functioning, activities, participation and influencing factors. The guideline does not ask for formal alphanumeric ICF codes here. Only use the domains when they help describe the contact and keep the client's report, measurement data and professional interpretation separate from one another. A factor that was not discussed stays outside the report. [1] [8] [9]
An ICD or DSM classification is only mentioned when it is available as an existing external source and relevant to the contact. The exercise therapist does not make an ICD or DSM diagnosis. State who established the classification and prevent it from being presented as your own professional conclusion. A missing classification is not derived from complaints or observations. [1] [9] [10]
The DCSPH code consists of four digits: positions I and II give the body location, positions III and IV the pathology code; these two combinations together form the diagnosis code that is billed to the health insurer. The code is an administrative field: you or the practice administration determine it. Hysio does not choose, set or fill in a billing code itself and does not derive a condition from it either. [1] [10] [8]
Hysio Exercise Therapy Intake organises a first conversation along the ODIM line: from registration and history to examination, analysis and treatment plan. What the client tells you stays separate from what you examined, from your analysis and from the agreed plan. The activity examination and the specific examination remain two recognisable parts. A suggestion from Hysio itself sits in a separate advice section and never among the findings. [3] [8] [9]
This format is intended for a separate examination contact: an additional or repeated examination without treatment and without a new treatment plan. If the examination takes place together with a first broad inventory, the intake fits better. If it concerns goal attainment at the end of a course of treatment, the evaluation fits. Always describe the examination context yourself; Hysio does not derive it from isolated words. [4] [9] [10]
For an interim evaluation you record the evaluation framework, the goals, how product and process have progressed and the follow-up agreement. For a final evaluation, the reason for closure, the aftercare and the policy discussed for when complaints return are added. Both cases use the same documentation format. Hysio does not close an episode itself, does not calculate any change and does not add a policy of its own. [5] [10] [8]
Use concrete descriptions of posture, movement, task performance and behaviour that were stated or observed during the contact. Name who says or observes something when that is relevant. The guideline sees this qualitative observation as a complement to measuring and not as a replacement for it. A general label without task, environment or moment stays hard to reread. Hysio organises the source, but you decide whether the description is professionally correct. [2] [4] [6] [10] [8]
Clinimetrics fits in the report when the instrument, the moment of measurement and the outcome were actually recorded during the contact. The guideline has no fixed measurement frequency, but advises measuring at least in the first diagnostic phase and again in the final phase. Do not note a score because an instrument might fit the request for help. In the posture and movement examination, clinimetrics gets its own section. Hysio does not invent a missing value, unit or interpretation. [4] [8] [9]
Hysio's general individual Starter plan starts at €20 per month, excl. VAT. Pro costs €40 and Unlimited €80 per month, excl. VAT; with yearly billing you pay for ten months and get twelve. There is no separate rate for exercise therapy. See the pricing page for the current plan conditions. [1] [11]
Hysio's general privacy frameworks also apply when you record exercise therapy contacts. Work from the purpose of care and include only necessary client information. Agree who may start a recording, who has access, how the source is reviewed and which text goes into the record. In this relationship Hysio is the processor and the practice remains the controller. Consult the current privacy policy and legal information for processing, hosting and terms. [1] [13]
There is no automatic transfer. Hysio does not write the report text directly to an electronic medical record system, because there is no direct integration. You review the text and transfer suitable parts yourself according to your practice's way of working. For a consultation, that can be the client's perspective, procedures, evaluation and agreements. Ask your record system supplier which transfer route is possible; this page does not promise any specific integration. [1] [15] [14]
Reliability starts with a clear source. Compare posture, movement observations, measurement values, negations and agreements with what was actually said or examined. Hysio's output can contain mishearing or wording errors, especially with abbreviations, medicine names and units of measurement. The platform does not fill in a missing measurement or working hypothesis. In testimonials on hysio.nl, exercise therapists say their record keeping became easier and more professional. You correct the text and only approve a suitable version. [1] [14]
You can write by hand, use a fixed template of your own, choose a general dictation function or use a specialised platform. Working by hand can be appropriate for a short or unusual contact. Choose a way of working with separate space for observation, assessment and follow-up. Compare alternatives on professional terminology, source roles, correction time, privacy and transfer to your own record. [1] [14]
Hysio has a trial period of fourteen days for which you do not need a credit card. Which documentation formats you open during it depends on what is in your account; this page does not promise any specific format. Compare a consultation, a posture and movement examination and a psychosomatic intake on source roles, measurement data, professional terminology, corrections and usefulness for your own record keeping. Also note which data are missing and which follow-up agreements you record yourself. [6] [12] [11]
There is no measured profession-specific time saving. Recording or dictating, generating, reviewing the content, correcting and transferring to the record together form the chain you measure. Compare consultation, examination and psychosomatic contacts separately, because the correction effort can differ. Besides minutes, also look at completeness and corrective actions. That way you see what admin with AI brings your own practice, without adopting a general savings claim. [7] [14]
Decide in advance how you want to organise movement, measurements and agreements in the record. Start with one type of contact, for example the regular treatment contact, and keep track of how much you need to correct. Arrange privacy, corrections and transfer to the record before you expand to intake, examination, evaluation or psychosomatic contacts. After each contact, check whether the source and the follow-up agreement have been recorded correctly. [2] [13] [14]
No. A diagnosis or working hypothesis does not arise independently in Hysio. The platform only organises it when you record it as source information within the chosen documentation format. In an examination report, an explicitly stated analysis can be included recognisably. A pattern that could only follow from isolated data is never presented as your conclusion. The psychosomatic format also works with SSCEGS and does not produce a psychiatric classification. [4] [7] [9] [10]
Use the organised text as a basis for your own record-keeping process. Assess whether client statements, observations, measurement values, timeline, analysis and agreements are correct and adjust the wording where necessary. For each documentation format, only transfer the parts that your record actually has. Hysio does not independently determine which place in the record, classification or final wording your practice uses. [2] [3] [4] [5] [10] [8]
Hysio creates the report. You remain responsible for what you save in the record.
