Three documentation formats for the arts therapist: intake, session dictation and evaluation with MDT meeting. What the client did, what you saw and your interpretation stay separate.
For arts and psychomotor therapies, Hysio offers fixed report structures around three parts: discipline, medium and activity format; the observable course of the session and what the client expresses; and the therapist's interpretation and evaluation.
The profile contains Hysio Arts and Psychomotor Therapy Intake, Hysio Arts and Psychomotor Therapy Session Dictation and Hysio Arts and Psychomotor Therapy Evaluation & MDT. The intake focuses on the first contact, the session dictation on a single session and the evaluation format on an evaluation or meeting. In all three, the medium, the activity format, concrete observations, client expressions and your arts and psychomotor therapy interpretation stay separately recognisable. [2] [3] [4]
The intake organises the contact context, request for help, expectations and history. The explicitly mentioned discipline, the medium, the activity format and the observed course also get their own place. Then come your stated interpretation, goals and agreements. Hysio does not fill in any meaning of material or behaviour. Information from the client, a family member or a referrer stays with the relevant source. [2]
Session dictation suits recording a single current arts and psychomotor therapy session. You describe the material offered, the activity format, what was actually carried out and how the client responded. Your interpretation and the follow-up stay separate. The documentation format does not turn a single session into a full intake or evaluation of the treatment process and does not infer a treatment effect from an isolated observation. [3]
The evaluation format organises the goals discussed, the evaluation period, reported change and your arts and psychomotor therapy contribution. In a meeting, other contributions and open differences also stay visible. Decisions stay separate from actions, responsible persons and deadlines. Hysio does not formulate its own goal assessment. A change only appears in the report as the client or another source actually stated it. [4]
An intake records the request for help, background and the first contact. An evaluation looks at a named period and the goals that are actually discussed. The difference therefore lies in the purpose and source of the contact. Hysio should not reconstruct a course or achieved goals from an intake when you have not provided a current evaluation. [2] [4]
State the discipline explicitly, then the medium or material used and the concrete activity format. Also state whether an activity was offered, chosen, carried out or only planned. Hysio may not infer the discipline from a musical instrument, drawing or movement. By providing this information separately, it stays clear what actually happened during the contact. [2] [3]
Yes, when that information has been provided explicitly. State the goal, the period, any measurement data and who described the change. Hysio does not assume improvement on the basis of two numbers or one observation. A change experienced by the client stays distinct from your goal assessment. Check that both keep their original meaning in the text. [3] [4]
The arts and psychomotor therapist gives the professional interpretation. Hysio can include it when you state it explicitly, keeping any uncertainty or limitations. The platform itself does not assign psychological meaning to colour, material, movement, sound or silence. What the client says about a piece of work also stays a client expression and does not automatically become your interpretation. [2] [3]
Make clear who says something and whom the statement concerns. Hysio keeps the client's perspective, a contribution from a family member and your own observation apart. If someone present says something about someone absent, that indirect origin also stays visible. An unclear speaker is not guessed on the basis of the content. [2] [4]
No. The name covers two possible contact types. An evaluation stays an evaluation and an MDT meeting stays an MDT meeting; only an explicitly combined contact is described as both. Hysio may not add participants, meeting contributions or a joint decision because the catalogue name contains MDT. So state in your input which contact you actually had. [4]
Record what the client wants to change and which expectations are expressed during the intake. Keep the request of a referrer or family member distinct from that. Hysio can organise this information, but does not turn an expectation into an agreed treatment goal. State separately which goals you actually discussed together and which questions are still open. [2]
Only if the source also mentions that it was carried out. An offer, a choice, a start, an interruption and an agreement for later are different events. State that status clearly. Hysio should not present a planned activity as a completed intervention. When reviewing, check that the report correctly reflects the actual order and the client's participation. [2] [3]
No. The session report describes the current contact: the focus, the activity format used, the course, client expressions and agreements. Earlier context stays dated and is only included when you provide it. Even with voice-driven admin, Hysio may not turn that context into missing current findings. This keeps it visible what happened during this session and what comes from the history. [2] [3]
The evaluation format is intended for contacts that centre on discussing goals and a named period, possibly with other people involved. Session dictation is intended for the course of a single session. An arts and psychomotor therapy interpretation can appear in both and is not in itself a reason to choose an MDT meeting. The purpose of the contact determines which report structure fits. [3] [4]
Describe concrete actions, order, pauses and resumptions insofar as you have mentioned them. Keep the client's own words and your interpretation apart. Hysio should not, for example, explain an interruption as resistance or uncertainty. Only include such an interpretation if you have stated it explicitly, with the provisional status and context that go with it. [3]
The client's experience, information from a family member and your professional assessment stay separately visible. They do not have to coincide. Hysio does not turn them into a consensus and does not choose which source is right. If the difference leads to an agreement, you record it separately; without such an agreement it stays an open difference. [4]
First state your arts and psychomotor therapy interpretation and then describe what was actually agreed. A possible explanation does not automatically lead to a new activity format, frequency or goal. Hysio does not add that next step itself. In the report, it must stay recognisable what your interpretation is, what is only being considered and what has become a concrete agreement. [3] [4]
A client expression describes what the client says; an observation describes what you observed. Indicate in your input which source it is. Hysio may not write down a reported experience as your finding. Nor should it infer a feeling from behaviour that the client or arts and psychomotor therapist has not named. [3]
Then that information stays unknown. A client who does not respond audibly is not automatically described as satisfied, tense or in agreement. Hysio should not infer such responses from silence or from the chosen activity format. It is up to you to add what you actually observed or asked about, and to check that the report does not suggest a response that is missing. [3]
Describe which activity format was offered or carried out first, what changed after that and who indicated it. Keep a choice by the client separate from an intervention by the arts and psychomotor therapist. If you name a reason, it stays with the correct source. Hysio may not invent a goal or effect of the change when only the change itself has been recorded. [3]
State the contact, the discipline, those present and the focus of the session insofar as they are known. Keep earlier documents recognisable with their source and date. With voice-driven reporting, Hysio does not infer this information from material or surroundings. The context helps you understand later what an observation related to, without turning a single contact into a statement about the whole treatment process. [2] [3]
Write that meaning down recognisably as a statement or experience of the client. That is something different from your arts and psychomotor therapy interpretation of the work. Hysio may not turn one layer into the other. If you state your own interpretation, it must come back separately and with the limitations mentioned, without putting words in the client's mouth. [3]
Indicate who is giving the information and whom it concerns. A report from a partner about an absent family member stays indirect information, not a finding of the arts and psychomotor therapist. Only include what is needed for this client record. Hysio should not attribute consent, a feeling or a role in the contact to someone who is absent. [2] [4]
Record the purposes discussed separately: taking part in treatment, photographing or keeping work, and sharing data are different topics. Hysio does not infer consent for another purpose from participation. An unanswered question is not agreement either. Check for yourself which agreements apply before material or report information is kept or shared with a recipient. [3] [4]
Describe what stopped, when that happened and whether the activity was resumed later, insofar as this was mentioned. A pause does not prove resistance, overload or a decrease in motivation. Hysio does not add such an explanation. An explanation from the client and any interpretation of yours each get their own place alongside the concrete course. [3]
Bring the relevant goals, the course discussed and your arts and psychomotor therapy contribution, with the origin of the information. Keep other contributions and decisions separate. A client preference is not automatically a joint plan. State who carries out an action and when, if that has been agreed. You check which data are needed for the meeting and the recipient. [4]
A decision records what was agreed; an action carried out describes what was actually done. Hysio should not summarise a decision as its execution. Where needed, state an action owner, deadline and open follow-up. For example, do not write that information has been sent when it was only agreed to send it. Check this status before sharing the report. [4]
Personal experiences, emotions, client expressions and information about family members can contain sensitive data. The description of a piece of work can also reveal something personal. Include what is needed for the contact and keep observation and interpretation apart. Hysio does not assign its own meaning to the material. You decide which wording belongs in the record. [2] [3]
State the sender and the document date and make clear which information comes from the letter. Keep this separate from what the client says now and what you observe during the session. An older judgement does not automatically become a current finding. When sources differ, checking remains necessary; Hysio may not resolve that silently. [2] [3]
No. Hysio may not independently interpret colour, shape, choice of material, music, movement or play psychologically or psychiatrically. The report can, however, contain your explicitly stated, provisional arts and psychomotor therapy interpretation. The platform does not assess a piece of work as a psychodiagnostic test. So check that descriptions of the material have not quietly acquired a new meaning or conclusion. [2] [3]
Include the instrument name, version, administration date, provided result and source insofar as they have been mentioned. Keep your interpretation of it separate. Hysio does not calculate, standardise or interpret the score and does not fill in missing test items. If the information is incomplete, that stays visible instead of the report suggesting a fully completed assessment. [3] [4]
No. An action, choice of material or response in a single session does not on its own prove progress or a treatment effect. Hysio describes what was provided and includes an explicit goal assessment recognisably from its source. An improvement experienced by the client stays a client report. The arts and psychomotor therapist assesses what meaning the observation has within the treatment process. [3]
Explicitly provided safety information gets its own place alongside information on medication and substances. State the source and the moment the information relates to. Older information may not be presented as a current screening. Hysio does not give its own severity label, reassurance or risk judgement. Record a professional assessment and any follow-up action separately when they have actually been stated. [3] [4]
Note what the client experiences and when. A statement about relaxation or tension does not automatically become a judgement about the effectiveness of the treatment. Hysio should not make that connection itself. Your evaluation can be included alongside it when you state it, keeping the context and limitations you mention. [3]
If the measurement was carried out but no result was provided, the result stays unknown. The report instruction states that the result has not been documented verbally. That is not a normal or abnormal outcome. A planned measurement does not yet count as carried out. You check the source and only add a result when you can verify it. [2] [3] [4]
Keep each piece of data with its source and time if the difference has not been resolved. Hysio does not choose a most likely value, does not calculate an average and does not turn it into a trend. A clear correction during the same contact can be followed. Then check that only the corrected value is used and that other, still open differences stay visible. [3] [4]
Note which intervention was actually carried out and which response was then reported or observed. Keep that sequence distinct from your interpretation. Hysio may not assume that the intervention caused the response. An exercise agreed for later should also not appear as carried out in the course of the session. Check that offer, execution and agreement keep their own status. [3]
Include the uncertainty as you stated it. Words such as provisional or limited to this activity format may not disappear when summarising. Hysio does not turn a possible explanation into an established meaning, nor a preference into an agreed goal. Check that the evaluation stays as cautious as the assessment it is based on. [4]
Use the name that the arts and psychomotor therapist has explicitly mentioned. Hysio may not reconstruct a method from material, sound or movement. An unclear name stays a point to check. Also make visible what was actually done within that activity format, so that a well-known method name does not suggest that all associated steps or interventions were carried out during the session. [3]
Check the discipline, activity format, execution status, client expressions and your interpretation against the original information. Pay attention to source roles, uncertainty and the status of agreements. A summary may not suggest more effect or agreement than was discussed. Voice-driven reporting supports the write-up; you decide which text is correct and needed for the record or a separate handover. [2] [3] [4]
State that it is an intake and indicate the discipline, those present and the request for help. Keep current information separate from earlier documents. Only describe the material used and the activity format when they were actually discussed or carried out. State your interpretation and agreements separately, so that the report structure does not have to fill in missing meaning or follow-up. [2]
The fixed sections help you find the request for help, medium, course, the client's perspective and interpretation. They keep different kinds of information apart. An empty section does not mean that the topic in question was examined and found normal. So always check the content behind the headings. The structure supports reading, but does not prove the completeness of the contact. [2]
State your observation and interpretation recognisably and separately. Also mention any limitations of that interpretation. Hysio may not add psychological meaning, a choice of method or a treatment decision to what you have provided. When reviewing, you assess whether the wording is correct and has not become more definitive. This way you keep responsibility for the professional meaning of the report. [3]
Hysio Supplementary is a separate part of the intake for possible verification points, missing context and questions not asked. It is not a place for Hysio's own interpretation of material, a diagnosis or an alternative treatment plan. The session dictation and evaluation format do not have this section. A point of attention only becomes part of your practice when you assess and work it out yourself. [2] [3]
Arts and psychomotor therapies have profile code 080 in the Hysio catalogue. The three documentation formats are numbered from 080.01 to 080.03. These codes distinguish the intake, session dictation and evaluation format. They say nothing about the registration or authorisation of an individual arts and psychomotor therapist. Choose what fits the session based on the name and purpose of the documentation format. [2] [3] [4]
Hysio may not infer consent for another data destination from participation. Record what was explicitly discussed about the purpose, the recipient and which information it concerns. Consent for a limited action list does not automatically cover the whole report or photos of work. You assess the applicable agreements and check what may actually be shared. [3] [4]
Consider whether you want to record an intake, a single session, an evaluation or a meeting. The actual source determines the appropriate report structure; make the contact type explicit. Do not fill in missing sections with standard information. If the source contains too little content for a reliable report, you first need to clarify yourself what was discussed or carried out. [2] [3] [4]
Indicate which statement you are correcting and what the correct information is. If a speaker or term is not reliably identifiable, Hysio should not guess it. Check the passage yourself and keep unresolved differences visible. Make sure that a correction in, for example, a number or agreement also comes back consistently in the summary. [3]
The structure gives separate space to discipline, medium, activity format, observable course, client expressions and your arts and psychomotor therapy interpretation. This way, the report describes not only what was discussed, but also what was actually done within the activity format. Hysio does not assign its own symbolic or psychological meaning to that. You decide how the contact is professionally interpreted and recorded. [2] [3]
Source-bound means that every description goes back to explicitly provided information. Material, action, client expression and professional interpretation stay clearly separate from each other. Hysio does not add meaning, effect or a treatment decision on the basis of a piece of work or observation. Uncertainty and differences between sources also stay visible. You check that the text reflects the contact without adding to its content. [2] [3] [4]
Hysio creates the report. You remain responsible for what you save in the record.
