Quick answer
Three profession-specific documentation formats for psychology, in which the client's perspective, observations and professional formulation stay clearly separate.
Hysio distinguishes between intake, treatment session and evaluation and keeps the client's perspective, observations and professional formulation apart. What was not said in the encounter is not added. You assess the content and decide what goes into the record.
For psychology, Hysio has 3 available documentation formats: Hysio Psychological Intake, Hysio Psychology Treatment Session and Hysio Psychology Evaluation & Closure. The intake organises a first conversation around registration and the request for help. The treatment session organises what was discussed and done in a therapy session. The evaluation sets goals, measurement outcomes, your professional evaluation and the decision side by side. You choose which format fits the encounter. [2] [3] [4] [6] [7]
Hysio Psychological Intake organises an intake conversation into a psychological intake report, with the request for help, the client's perspective, relevant history, observations, the psychologist's explicit formulation and the plan discussed. The report follows fixed sections: summary, registration and request for help, perspective, complaints and context, relevant history, observations and professional formulation, and goals, plan and agreements. A diagnosis, classification or treatment decision is only included when you have stated it yourself. [2] [7] [5]
Hysio Psychology Treatment Session turns a therapy session into a session report in which the client's perspective, content discussed, interventions carried out, observations, measurement information and follow-up agreements stay clearly apart. The fixed sections are summary, reason and session focus, the client's perspective and other sources, content discussed and interventions, observations and professional assessment, goal progress with measurement and risk information, and agreements, follow-up and open points. What was not said in the session is not added. [3] [5] [6]
Hysio Psychology Evaluation & Closure turns an evaluation conversation and linked care pathway sources into an internal evaluation and closure report with goals, measurement outcomes, the explicit professional evaluation, the decision and open review points. The sections run from reason, period and sources, via goals and evaluation criteria, the client's perspective, progress and measurement outcomes, to your professional evaluation, an explicit decision and the points that are still open. The decision remains yours. [4] [6] [7]
Within psychology, you use Hysio Psychological Intake for the first conversation after a registration or referral, when the request for help, complaints, context and relevant history are still being mapped out and you agree on goals and a plan together. Once the care pathway is under way and you are recording a regular session, the treatment session fits better. If the conversation is about goal progress and a decision on follow-up or closure, you choose the evaluation format. [2] [7] [5]
For psychology, Hysio Psychology Treatment Session organises this profession-specific information: the reason for and focus of the session, what the client brings in, information from other sources, the content you discussed, the interventions you carried out, your observations and professional assessment, goal progress with measurement and risk information, and the agreements, the follow-up and the points that remain open. Each section stays linked to the source that supports it. [3] [5] [6]
Hysio Psychology Evaluation & Closure is intended for the planned evaluation moment in an ongoing care pathway and for the conversation in which you close a care pathway. You then set goals and evaluation criteria alongside progress and measurement outcomes, formulate the professional evaluation yourself and note an explicit decision on follow-up or closure. Points that are not yet settled remain as open review points. [4] [6] [7]
After the relevant care encounter, Hysio Psychological Intake creates a psychological intake report for your own record. It contains the request for help, the client's perspective, the complaints and the context, relevant history, your observations and your professional formulation, and the goals, the plan and the agreements. The report is a proposal for your record: you read it through, adjust what is needed and decide on the final text. [2] [7] [5]
Hysio offers a psychological intake, a psychological treatment session and a psychological evaluation and closure. The formats distinguish between registration and the request for help, the content of a session and the assessment of goal progress and follow-up. Hysio organises what you record as the source and does not take on any psychological assessment or treatment decision of its own. [2] [3] [4] [7] [5]
The intake organises registration and the request for help, the client's perspective, complaints and context, relevant history, observations, professional formulation, goals, plan, agreements and consent. These sections stay linked to their source role. Hysio does not make its own classification and does not fill in risk information or history when it was not mentioned in the conversation. [2] [5] [6]
The title healthcare psychologist (gezondheidszorgpsycholoog) has been protected since 1 April 1998, when the Wet BIG provisions for this profession and the Besluit gezondheidszorgpsycholoog (the decree on healthcare psychologists) came into force. Only those listed in the BIG register may use that title; the general designation psychologist is not protected by law. For your documentation, this mainly means that your role and registration must be recognisable in what you record. Hysio does not assume that role and does not derive any registration or competence from a conversation. [1] [5] [6]
No. There are no reserved procedures for the healthcare psychologist within the meaning of Article 36 of the Wet BIG. Prescribing and medical reserved procedures are not part of this profession; title protection and disciplinary law do apply. Which procedures you may carry out follows from your own registration, role and setting. Hysio does not grant or assess any competence: the report records what you did and decided, and you decide what goes into the record. [1] [6] [7]
Recommended training intake: 1,885 places per year (2023 advice from the Capaciteitsorgaan) compared with 965 places granted by VWS from 2024. That difference affects training capacity and with it staffing and waiting times in practice, not the way you build a record. What goes into the record depends on the encounter itself and on what you as a psychologist establish and decide. [1] [7] [5]
Waiting time after registration and treatment waiting time. For both generalist basic mental health care and specialist mental health care (GGZ), the Treeknorm (the Dutch maximum acceptable waiting times) sets four weeks of waiting time after registration, ten weeks of treatment waiting time and fourteen weeks of total waiting time as the norm. In practice, the waiting time after registration often turns out to be longer than treatment waiting time, and the norm is consistently exceeded in parts of the sector. That affects the planning of a care pathway, not the content of what you record per encounter. [1] [5] [6]
For psychology, Hysio Psychology Treatment Session organises the recorded care encounter straight after the session: you record the conversation or dictate it briefly afterwards, and the result is divided over the fixed session sections. This is how voice-driven reporting works after a session: you do not have to come up with a structure yourself first. You then read the report through, correct where needed and put it in your record. The session itself remains undisturbed. [3] [6] [7]
The report sections of Hysio Psychology Evaluation & Closure are: summary; reason, period and sources; goals and evaluation criteria; the client's perspective and other sources; progress and measurement outcomes; professional evaluation; explicit decision and follow-up; open points and review. Each section stays empty when the conversation says nothing about it. The professional evaluation and the decision are never filled in automatically: you formulate those yourself. [4] [7] [5]
Hysio Psychological Intake fits the moment when you have just held an intake conversation. Instead of writing up your notes later, the intake report is already there in the structure you are used to, with the request for help, perspective, history, observations and the plan discussed. You read it through between two appointments and save what is correct. [2] [5] [6]
The specific purpose of Hysio Psychology Treatment Session is to record a single therapy session, without that session becoming an intake or an evaluation. The format captures what the client brought in, what you discussed and did, what you observed and which agreements followed. A judgement on the whole care pathway does not belong in it; that is what the evaluation format is for. [3] [6] [7]
A treatment session describes the focus of the conversation, the client's perspective, content discussed, interventions, observations, professional assessment and agreements. An evaluation also looks at goals, measurements, progress, an explicit decision and follow-up or closure. Hysio does not automatically turn a single session into an evaluation decision. The precise meaning remains with the psychologist who carried out the encounter. [3] [4] [6] [7]
Four source roles matter: what the client tells you, what comes from other sources, what you observe and what your professional formulation is. Agreements, consent and evaluation criteria also have their own place in the report. Hysio may organise those roles, but never presents a professional hypothesis as a statement by the client and does not invent an external source. [2] [3] [4] [7] [5]
Article 1 of the Wkkgz names as a care provider an institution or a solo healthcare provider (solistisch werkende zorgverlener): someone who provides care professionally without being employed by or working on behalf of an institution. If you work in independent practice, you are therefore a care provider yourself, and the duty of quality and the rules on complaints apply to you directly. Your record is the evidence of careful practice. Hysio organises that report; accountability remains with you. [1] [7] [5]
Retention period under the Beroepscode: a second rule, distinct from the WGBO. Article 34 of the Beroepscode voor psychologen 2024 states that the record is kept for one year after the professional relationship ends, or as much longer as its purpose requires. That rule expressly gives way to a statutory retention period. For care under the WGBO, the statutory period therefore takes precedence; the code of ethics only sets a minimum where no statutory period applies. [1] [5] [6]
The Beroepscode voor psychologen 2024 (the Dutch code of ethics for psychologists) supports careful and professional practice and sets a clear requirement for the record. Article 33 asks you to keep notes of your professional activities in such a way that you can account for your conduct. That is a principle-based requirement, not a prescribed list of fields. According to the code, personal working notes are expressly not part of the record. Hysio organises your report and leaves that consideration to you. [1] [6] [7]
For psychology, there is no single document that prescribes the sections of a report as fixed field names in a fixed order. The practice standard is a sum of the WGBO, the Beroepscode, the Landelijk Kwaliteitsstatuut GGZ and the care performance model (zorgprestatiemodel). What is fixed is that your practice must be traceable, not how you write it down. So for AI reporting in mental healthcare there is no prescribed template either. Hysio therefore offers a recognisable structure for each encounter type; which structure you use remains your choice. [1] [7] [5]
Kwaliteitsstandaard Diagnostiek (Akwa GGZ, formerly the generieke module Diagnostiek) describes what good diagnostics in mental health care involves: screening after registration, looking at complaints and capabilities from different perspectives, and continuously re-evaluating diagnosis and classification. That re-evaluation is your work. Hysio records the diagnostic step you took in the encounter and does not itself revise an earlier classification or working hypothesis. [1] [5] [6]
The relevant parts are: identifying and administrative details, the reason for registration and the request for help, diagnostic findings and classification, the care demand type, a treatment plan you draw up together with the client, progress notes per encounter, evaluation moments, correspondence with the referrer and the closure. Personal working notes are expressly not part of it. Hysio only organises what was mentioned in the encounter and does not fill in missing sections itself. [1] [6] [7]
Kwaliteitsregister GZ-psycholoog NIP. A new register, opening on 1 January 2026. Not required by law. Shows ongoing training, peer review and the like, in addition to the BIG registration, which only confirms initial competence. NIP membership was a condition for access until 2026; that lapses as of 1 January 2027. The register asks for points registration, not free text, and is therefore separate from your documentation per client. [1] [7] [5]
Landelijk Kwaliteitsstatuut GGZ (the national quality statute for mental health care). Registered as a quality standard with Zorginstituut Nederland under Article 66b of the Zorgverzekeringswet, which is relevant for substantive checks (materiële controle) by health insurers. Every provider of curative mental health care within the Zorgverzekeringswet publishes an approved statute stating how care is organised and which roles and responsibilities apply. That is an obligation at practice level, not a task that recurs with each client encounter. [1] [5] [6]
Observations should stay concrete and time-bound: what you perceive during the encounter, which reaction is visible and in what context it happens. A professional interpretation then follows, clearly recognisable, in the section intended for it. Hysio does not add a character trait, intention or severity on the grounds that it would seem plausible according to a general pattern. [2] [3] [5] [6]
No. A diagnosis, classification or professional formulation is only included when you record it as the psychological source and the chosen documentation format is intended for it. Hysio does not derive a classification from isolated complaints, words or observations. The system organises information, while the psychologist remains responsible for interpretation and decision-making. [2] [4] [6] [7]
With Hysio Psychology Evaluation & Closure, the spoken evaluation conversation is converted into the language of your record. What the client says stays in the client's words under the client's perspective. Professional terms such as a goal, a measurement outcome or a decision only appear in the report when you used them in the conversation. Hysio translates spoken language into readable report language and does not add a professional term that was not used. [4] [6] [7]
After using Hysio Psychological Intake, a report is ready for psychology in which each section is filled with what was actually discussed. Just as important is what is not in it: a section about which nothing was said stays empty. History, risk information or a classification are not filled in on the grounds that they would be usual in this kind of conversation. That way it remains clear where the conversation stopped. [2] [7] [5]
The report structure of Hysio Psychology Treatment Session is fixed for psychology: summary; reason and session focus; the client's perspective and other sources; content discussed and interventions; observations and professional assessment; goal progress, measurement and risk information; agreements, follow-up and open points. That order shows at a glance who contributed what. You can adjust or merge sections before you save the report. [3] [5] [6]
You choose Hysio Psychology Evaluation & Closure as soon as the conversation is no longer about one session but about the care pathway as a whole: goals against measurement outcomes, an explicit professional evaluation and a decision on follow-up or closure. If it concerns a regular therapy session within an ongoing care pathway, the treatment session is the appropriate format. The difference lies in the decision. [4] [6] [7]
Nothing appears in the factual sections of the intake report that was not mentioned in the conversation. The request for help, history, observations and agreements stay strictly linked to the source that supports them. Hysio's own input is limited and appears separately at the end, under its own heading, so that you can see at a glance what comes from the conversation and what does not. You decide whether that closing section goes into your record. [2] [7] [5]
Unlike the intake report and the evaluation report, the session report has no closing section with Hysio's own input. Everything in it comes from the session itself: what the client brought in, what you discussed and did, what you observed, which measurement information came up and which agreements followed. If something is missing, that section stays empty instead of being filled in. This format is therefore fully tied to the source. [3] [5] [6]
Only in a separate closing section at the end of the evaluation report. The report itself stays fully tied to the conversation and the care pathway sources you provided. The professional evaluation is expressly excluded from any input of its own: that formulation comes from you, not from Hysio. The closing section is intended for remarks and points of attention and is separate from the report you include in the record. [4] [6] [7]
No. Consent only appears in the report when it was stated in the encounter, stating what it was for and to whom it was given. If nothing was said about consent, it is not filled in and not assumed on the grounds that it would be usual at an intake. Nor does a letter to the referrer follow automatically from the end of a care pathway. You assess whether consent has been recorded fully enough. [2] [7] [5]
A hypothesis stays recognisable as a professional thought or formulation and is not presented as an established fact. If the hypothesis was not stated, Hysio does not create it itself. Record the source, uncertainty and any testing when these were discussed during the encounter. That way observation, hypothesis and decision stay distinct from each other. [2] [3] [4] [7] [5]
Name the questionnaire or measurement, the moment, the source and the outcome when these were discussed in the encounter. A score is not given a diagnostic meaning that you have not explained. Hysio can organise the information, but does not calculate a new score, does not fill in missing answers and does not draw its own treatment conclusion. [3] [4] [5] [6]
Which rate belongs to which time band depends on the practitioner's profession, the type of consultation and the setting in which the encounter took place. A consultation is not billed by the minute but in fixed time bands. For your documentation, this mainly means that consultation type, duration and setting must be traceable from your records. Hysio organises the report and does not determine a service, time band or claim. [1] [5] [6]
For the diagnostic and treatment consultations of healthcare psychologists, psychotherapists and clinical psychologists in independent practice, maximum rates apply that are set and indexed again every year. Which amount applies to your practice in 2026 can be found in the current rate regulation; it changes every year. For your documentation, that makes no difference. Hysio organises the report and does not calculate, choose or check a rate, nor a service code. [1] [6] [7]
Direct and indirect time. Under the care performance model (zorgprestatiemodel), indirect time, such as preparation, documentation and brief discussions around a client encounter, no longer has to be registered separately. The rate for a time band applies to the combined direct and indirect time around that encounter. This is mainly what people mean when they say admin with AI cuts write-up work. Finishing your report faster primarily frees up room in your schedule, as long as the encounter stays within the same time band. [1] [7] [5]
The statute requires the lead practitioner (regiebehandelaar) to draw up a treatment plan together with the client, with fixed elements named in the statute, including the goals for a period to be evaluated, the way in which you intend to achieve those goals and who is responsible for what. Evaluation moments should also be in that plan. Those elements must therefore be traceable in your record. Hysio organises them, but does not formulate the goals and the plan itself. [1] [5] [6]
For your documentation per client, this register has no significance. It is a voluntary, additional form of accountability on top of the mandatory BIG re-registration, in which you keep track of training and peer review. That is done with points, not free text, and it is separate from a client's record. What you write in an intake, session or evaluation report therefore does not change because of it. [1] [6] [7]
Hysio does neither. No risk level is formulated, nor is it written that there are no indications of risk when nothing was said about it. Only what you name yourself goes into the report. The applicable guideline also advises against using risk assessment instruments to predict future suicide risk or to determine severity. Record yourself which instrument or source you used and what uncertainty comes with it. [1] [7] [5]
There is no broadly supported consensus framework for the minimum conditions for AI in mental health care. Professional standards and field norms are still missing, and professional groups do not agree among themselves. Each practice or institution therefore assesses an application itself, based on purpose, risk, data source and the degree of human oversight in its own situation. Hysio does not take over that consideration and does not record it for you either. [1] [5] [6]
The risks cited for AI in mental health care are bias in the source data, an unclear boundary between what the system does and what the practitioner does, and too little transparency towards the client about the use of AI. For psychology, that means three things: record which source you used, keep visible where your own formulation begins, and discuss with the client that you use assistance with your documentation. [1] [6] [7]
Hysio can include goals, plan and agreements when these were discussed together with the client or within the professional encounter. With voice-driven reporting too, Hysio does not create its own treatment plan and does not choose an intervention. The psychologist remains responsible for the professional formulation, the decision, the recorded consent and the way the record is used. [2] [3] [6] [7]
Assess source roles, the client's perspective, observations, professional formulation, goals, measurements, agreements and follow-up. Make sure that a provisional thought is not presented as fact and that a decision is only included when it was discussed. Hysio delivers a source-bound result; you assess the content and decide what is saved in the record. [2] [4] [7] [5]
Hysio creates the report. You remain responsible for what you save in the record.
