Quick answer
A request for help often touches housing, money, work and network at the same time. For each contact, Hysio sets out who said what, what you saw and what was agreed.
A contact report in social work follows the purpose of the contact and the frameworks of your profession, and the choices stay with you as a social worker.
Hysio Social Work Intake fits an initial request for help. This documentation format organises the contact into reason, contact and request for help, living situation and daily functioning, network and existing support, followed by professional synthesis and agreements. Clarifying the request for help thus gets its own place, separate from what the network brings in. As a social worker, you decide which request for help you note and which wording goes into the record. [1] [2] [3] [7] [8]
In Hysio Social Work Contact Report, the living context sits in the sections reason and contact, conversation and perspectives, and observations and professional interpretation. What the client says about housing, money, work and health thus stays separate from what someone close to them brings in and from what you have seen yourself. Describe the living context concretely enough that a colleague can follow the contact later without going back to the recording. [1] [2] [4] [8] [7]
You record a network meeting as a contact report, unless the meeting reviews the agreed goals. In that case Hysio Social Work Goal & Plan Evaluation fits, with the evaluation framework and plan source, progress per goal and the perspectives of client, network and professional side by side. Note who was present, who spoke on whose behalf and which agreement follows from the meeting. Without a recognisable earlier version of the plan, Hysio does not compare goals. [1] [2] [5] [7] [8]
A home visit is a setting, not a documentation format of its own. It usually remains a contact report. Hysio Social Work MDT Meeting & Handover only comes into play when the visit serves to hand over the case or to discuss it with several professionals involved; then the purpose, format and people involved, the perspectives brought in and the decisions made are what count. The purpose of the contact therefore determines the appropriate report structure, not the place where the conversation took place. [1] [2] [6] [8] [7]
In a follow-up conversation, the purpose of the contact determines the documentation format. If it concerns a new or substantially changed request for help, Hysio Social Work Intake fits, with its broad enquiry into living situation, network and existing support. If the conversation stays within the current plan, a contact report is sufficient. In both cases, state at the top who you spoke with, about what and why, so that a reader sees the link with the previous contact. [1] [2] [3] [7] [8]
In a handover, include only what the recipient needs: the purpose of the handover, the people involved, the status of the agreed goals and the open actions with owner and deadline. Keep separate what the client said themselves, what the network reported and what you established as a professional. Also record on what basis you share and with whom. Anything that does not emerge from the discussion is not passed on. [1] [2] [6] [8] [7]
Yes, provided the source remains the reference point. Hysio Social Work Contact Report organises a dictation into reason and contact, conversation and perspectives, observations and professional interpretation, and agreements and follow-up. Names of agencies, first names of family members and isolated numbers are error-prone in speech, so mark what you did not hear clearly and check it. What stays unclear belongs in the record as an open point and not as an established fact. This way, voice-driven reporting in social work remains an aid for organising the text, while you safeguard the content of the contact report. [1] [2] [4] [7] [8]
Someone close to the client gets their own source role alongside the client and the professionals involved. In Hysio Social Work Intake, what the network brings in sits in the section network and existing support, stating who said it and when. Write the statement as something reported, not as an established fact, even when it sounds plausible. If the picture of the person close to the client differs from the client's, let both stand side by side. [1] [2] [3] [8] [7]
You formulate the interpretation yourself, as a social worker, and you keep it apart from what others have told you. In Hysio Social Work MDT Meeting & Handover, that interpretation sits in the section information discussed and professional interpretation, so that a reader sees which conclusion comes from whom. If another discipline contributes a judgement, note it with the name of that professional and the date. Hysio does not write a conclusion of its own. [1] [2] [6] [7] [8]
Read the report alongside your source. Check whether the desired change is stated as the client put it in their own words, whether goals, owners and deadlines are correct, and whether missing information has visibly been left empty. Then check whether interpretation and observation can still be told apart and whether each conclusion has a recognisable author. Only when that is right do you save; that final check remains human work. [1] [2] [5] [8] [7]
Put a literal quote in quotation marks and note who said it, at what moment and in which contact. That way the social work record keeps showing that these are the client's own words and not your summary. Also state whether someone spoke on the client's behalf, for example a representative or an interpreter. Without that origin, a quote later becomes an anonymous fact. When you do your admin with AI, that origin is also the first thing you check. [1] [2] [3] [7] [8]
Use two different verbs. What someone tells you is reported; what you saw or heard is observed. In Hysio Social Work Contact Report, those two sit in separate sections: conversation and perspectives versus observations and professional interpretation. Describe an observation factually, without immediately attaching an explanation to it. That way a colleague can still see later what your picture was based on and where someone else's picture came from. [1] [2] [4] [8] [7]
Daily functioning says little without context. Note which life domain it concerns, since when the situation has been like this, what changed and what support is already in place from the network or from a service. Also state where the information comes from: the client, a housemate or a professional involved. Only with that background does an evaluation show whether a goal is getting closer or moving further away. [1] [2] [5] [7] [8]
Leave the gap in place and turn it into a point to follow up. Note what you do know, who the question lies with and when you expect the answer. That matters even more in a handover, because a recipient reads an empty field differently from an explicit open question. Never fill in a suspicion about existing help as fact; Hysio does not fill in missing source information and does not choose a plausible substitute either. [1] [2] [6] [8] [7]
Put the perspectives one below the other and let them differ. The client may understand an agreement differently from a family member or from you; that difference is information in itself and not an error to explain away. State for each statement who spoke and when, and close with what was actually agreed, with owner and deadline. Do not merge them into a joint wording that nobody said in exactly that way. [1] [2] [3] [4] [7] [8]
An agreement is verifiable when four things are settled: what happens, who does it, when it is done and how you can tell it has succeeded. In Hysio Social Work Goal & Plan Evaluation, each goal also gets a status relative to the earlier version of the plan. If one of the four is missing, note it as an open point instead of filling it in with an assumption. [1] [2] [5] [8] [7]
Name the difference itself, not just the outcome. Write who said what, at what moment, and why the accounts differ, as far as you know. Then give your own judgement as professional interpretation, recognisably separate from the sources. Do not silently choose the source that suits best, because the recipient can then no longer see that there were two accounts. Share only what the recipient needs for their task. [1] [2] [6] [7] [8]
Choose your verb deliberately. Write reported, observed or assessed instead of a bare statement, and leave the degree of certainty in the sentence. The professional code requires that an interpretation is recognisable as interpretation and not as established fact. Avoid diagnostic language about the client; a clinical characterisation belongs to the professional who made it, with name and date. What you do not yet know remains a question. [1] [2] [5] [8] [7]
Correct visibly, not silently. Leave the original passage in place, add the correction with date, reason and the source that supports the change, and state who made the correction. That way the timeline of the record stays correct and a reader can see what was known at which moment. A correction that only overwrites the final text no longer makes clear later what an earlier agreement was based on. [1] [2] [4] [7] [8]
The format follows the purpose of the document and what the source really supports. A broad introductory conversation calls for the intake structure, a single contact for the contact report, a meeting with several professionals for the MDT meeting and handover structure. If your source does not support the chosen structure, choose the lighter format instead of filling headings that nobody discussed. Empty headings suggest information that is not there. Voice-driven reporting works the same way: Hysio does not fill a heading you did not dictate. [1] [2] [6] [3] [8] [7]
The BPSW professional code provides a professional framework for social work. That code calls for record keeping that is relevant, necessary, careful and factual, with interpretations that stay recognisable as interpretations. The Wmo 2015 and the Meldcode (the Dutch reporting code for domestic violence and child abuse) provide additional context, but do not automatically determine which conclusion belongs in an individual record. The code does not prescribe a fixed heading structure; how you write your report therefore remains a professional choice that you account for. [1] [2] [3] [7] [9]
For documentation in social work there is no national guideline that prescribes one fixed layout. What does exist is the BPSW professional code plus the requirements that follow from the legal framework the work falls under, such as the Wmo 2015 for municipal support. Methodical working therefore translates into your own line: request for help, goals, progress and evaluation in the same order, so that a record stays readable over time. [1] [2] [4] [9] [10]
When a need for support is reported, the municipality carries out an investigation and records the outcome in writing; the Wmo 2015 attaches a period of six weeks to this. The formal decision on a tailored provision (maatwerkvoorziening) is a municipal power, not that of the social worker and certainly not that of Hysio. An intake or contact report from social work is therefore not automatically the same document as the statutory investigation report. [1] [2] [5] [10] [7]
You describe a complaint or incident factually, with source, date and follow-up. Note what happened, who reported it and what was done afterwards, without answering a question of blame in the same sentence. Keep the process note separate from the fact in the record: your organisation's complaints or incident procedure follows its own route, and a documentation format does not take over that route. You note anything still ongoing as open. [1] [2] [6] [7] [9]
The retention period follows the applicable legal regime and the record holder. Social work has no single uniform records law: which period applies depends on the framework the specific work falls under, and youth care has a different period from general support. So record for each file for what purpose and under which administrator it was created, and check the local agreement before you note a destruction date. Do not confuse the date of the event with that date. [1] [2] [3] [9] [10]
A quality register can describe requirements for professional development. Registerplein is a voluntary register for social workers with its own re-registration cycle; in youth care, SKJ registration is anchored in law, with the standard for responsible task allocation (norm verantwoorde werktoedeling) determining per task whether a registered professional is needed. Social work falls outside the Wet BIG (the Dutch Individual Healthcare Professions Act), so a registration says something about maintaining professional competence and nothing about whether Hysio has made an assessment for you. [1] [2] [4] [10] [7]
Record who gave consent, for what exactly, at what moment and for which period or recipient. Also note whether someone signed on the client's behalf and whether parental authority over a minor plays a role. Consent for one conversation is not consent for every later sharing. If someone withdraws consent, record the date and the consequence. Hysio does not infer consent from silence in the source. [1] [2] [5] [7] [9]
Go through four points: is sharing necessary for the recipient's task, is there a legal basis or consent, is the recipient correct, and are you not sharing more than needed. Social work records often contain special categories of personal data, so data minimisation is not a formality here. Professional confidentiality has exceptions, for example the right to report to the advice and reporting centre for domestic violence, but you make that judgement, not the platform. [1] [2] [6] [9] [10]
The synthesis remains yours. You may summarise information from others, but you write the judgement that follows from it under your own name and keep it separate from the sources underneath. If a judgement comes from another discipline, for example a practitioner treating the client, you note it as that person's statement with name and date. In an evaluation, your assessment stands alongside the perspectives of client and network, not woven into them. [1] [2] [5] [10] [7]
For each source, look at owner, version and date, and at whether it applies to your setting. The BPSW code, Beroepscode voor professionals in sociaal werk (professional code for social work professionals), has applied since 1 January 2022; legal articles and municipal agreements may have changed since then. So never take over a standard from an older summary without opening the source itself. Whatever you cite in the record, you name with the name and date of the version. [1] [2] [6] [7] [9]
Participation calls for concrete data: which activities someone does, to what extent, since when, and what helps or hinders. If your organisation scores with the Zelfredzaamheid-Matrix (Self-Sufficiency Matrix), that score only belongs in the record when a professional trained for it has determined it, with date and domain. Hysio does not calculate a score and does not infer a level from a conversation. If supporting evidence is missing, the field stays empty. [1] [2] [3] [7] [8]
Name the life domain by the name your organisation uses and add in plain language what is going on. Terms such as self-reliance, own strength (eigen kracht), customary help (gebruikelijke hulp) and tailored provision (maatwerkvoorziening) have a fixed meaning; do not use them as mood words. Do not adopt a clinical characterisation for a life domain, because that belongs to another discipline. Write so that the client recognises their own situation when they request the record. [1] [2] [4] [8] [7]
Leave the field empty and add what you do not yet know. Write, for example, that the burden on the informal carer has not been asked about, instead of noting that the network can cope. An empty field marked as a gap leads to a targeted question at the next evaluation; a filled-in assumption, by contrast, disappears from view there. Hysio does not fill in such a missing item. [1] [2] [5] [7] [8]
Put the accounts side by side, each with the person reporting, the moment and the literal gist. Do not draw a middle way from them and do not leave out a version because it seems less likely. You note a signal factually, with source and status; the assessment using the step-by-step plan of the Meldcode and the decision to report remain with you and your organisation. Hysio does not weigh severity and draws no safety conclusion from conflicting information. [1] [2] [6] [8] [7]
You may record a suspicion, provided it stays recognisable as a suspicion. Describe what you saw or heard, whom it came from and why it struck you, and keep that apart from the question of what it means. The boundary lies at the conclusion: an assessment of severity, a report or a decision on support belongs with the competent professional or agency. Moreover, silence in the conversation does not mean that nothing is going on. [1] [2] [3] [4] [7] [8]
Note the signal separately and factually: what you observed, at what moment, who was present and what was agreed. Keep a statement from a neighbour or family member separate from your own observation. If there are minors in the household, that fact also belongs factually in the report. Do not write a conclusion on severity or a decision to report in the same breath; that step follows your own route. [1] [2] [5] [10] [7]
Give the client's perspective its own place and leave it there as it was meant, even when you see it differently. You put your assessment in a following section, recognisable as your judgement. In a handover, the recipient can then see the difference between what the client wants and what you advise professionally. If you weave the two together, the client's perspective disappears behind your wording. [1] [2] [6] [7] [9]
No. A decision on support that has not yet been made stays missing from the record. Note who makes the decision, which step is under way and by when an outcome is expected, and leave owner and status open as long as they do not exist. For a formal provision, the decision moreover lies with the municipality, not with the social worker. A plausible-sounding substitute makes an open question invisible. [1] [2] [5] [9] [10]
Describe what changed and leave the cause open unless the source names it. So write that the client has reported fewer payment arrears since the contact, not that the support has resolved the arrears. Put expectation, observation and assessment in separate sentences, so that a reader sees which of them is a claim. For a statement about effect, you need a measurement or an explicit statement from the person concerned. [1] [2] [4] [10] [7]
In a goal evaluation, check three things. Is the comparison with the earlier plan version correct, does each goal state who thinks what, and is uncertainty named where it exists? Then check whether the follow-up agreements have an owner and a deadline, and whether an open point stays visibly open. If the earlier plan version is missing, you do not set a goal status; without a source to compare against, every statement about progress is an assumption. If you made the evaluation with voice-driven reporting, put the report next to the previous plan. [1] [2] [6] [3] [7] [9]
Hysio has three plans: Starter for €20 per month, Pro for €40 and Unlimited for €80, all excl. VAT. If you choose a year, you pay for ten months and get two more on top. Which plan fits depends on the number of contacts you record per month. Weigh up not only the price, but also how much editing you still do afterwards. [1] [2] [3] [11]
Start with purpose and legal basis: what you record for and what you base it on. Social work records often contain special categories of personal data, so data minimisation and the retention period deserve an explicit agreement. Also arrange who may read along, who approves and how you inform a client. Hysio processes data in line with the GDPR and hosts data in the European Union; the data processing agreement with your organisation and your own choices within it remain your responsibility. [1] [2] [4] [12]
Writing by hand gives the most control and the least speed. Dictating produces a literal text that you then have to organise into a report yourself. Hysio organises the spoken source straight away into fixed sections for social work and keeps source roles apart. In all three cases you read the result through and approve it; the gain lies in the organisation, not in skipping your own assessment. That third way of working is also called AI reporting. [1] [2] [5] [11] [12]
Hysio offers a free trial of fourteen days. Do not use it to try everything at once, but to look closely at one type of contact: is the organisation right, do source roles stay separate, and how much do you still change before you would save? Test with material that resembles your normal work, including a conversation with several voices. Note what you change; those notes say more than a general impression. [1] [2] [6] [11]
No. No automatic connection with every record system has been promised for social work. Moreover, the profession does not work with a medical record but with a client record or case management system in the social domain, and those systems differ greatly from one organisation to another. So expect to transfer text, and check in advance which fields your organisation makes mandatory. Also check who may save and approve in that system. [1] [2] [3] [12]
Agree who reads, when, and for what. A workable agreement is: the social worker who conducted the contact reads it through within the same working day, with consistent attention to source roles, professional language, open points and the follow-up agreements. Record who approves when someone is absent. For sensitive topics, a second reader can help. The text only becomes part of the record once a person has approved it, not when it has been generated. [1] [2] [4] [13]
Correct from the source, not from memory. Find the passage in the contact again, only then adjust the sentence, and keep the change limited to what is demonstrably supported. If you delete something because it was not in the conversation, briefly note why. If you sharpen the wording of an interpretation, leave the source line underneath unchanged. That way it stays visible that you adjusted the text and not the source. [1] [2] [5] [12]
Leave open what you did not catch and mark it as a point to follow up. Names of agencies, first names of family members and isolated numbers are the most error-prone in speech, so check those separately. Choose a lighter documentation format rather than filling headings the dictation does not support. What turns out to be correct after checking, you add with date and source; what stays unclear remains an open point in the record. [1] [2] [6] [12]
Measure two things at once: how many minutes you spend on a report and how much you still change in it afterwards. Compare similar contacts for this, for example ten intakes with ten intakes, and not a short note with an extensive report. Also look at record quality: are source roles still separate, are open points named, is the professional language right? Gains you pay back later in corrections are not gains. [1] [2] [5] [11]
A controlled introduction starts small. Choose one purpose of contact, for example the regular contact report, and agree a fixed final check for it. Record who reads, what goes into the record and how an error is reported back. After a few weeks, evaluate with the people doing the work: what saved work, where did it go wrong, which professional language was not right? Only expand when that round delivers something you can show. [1] [2] [6] [11] [13]
Hysio creates the report. You remain responsible for what you save in the record.
