Documentation for respiratory medicine

For breathing, lung function and sleep-related complaints, with a dedicated place for measured values, smoking status and inhaled medication.

Read how Hysio works in a respiratory consultation, an extended intake or a short follow-up contact. There is no nationally prescribed structure for your own record, but there is one for the letter to the GP.

What is voice-driven reporting for respiratory physicians?
Voice-driven reporting, also known as AI documentation, means that you say lung function and saturation out loud and that they appear in the report as you said them. For respiratory medicine there are three documentation formats: Respiratory Medicine Consultation, Respiratory Medicine Intake and Respiratory Medicine SOAP Note. Hysio records lung function values as you state them. [2] [3] [4]
What stays tied to the source?
History and dyspnoea stay separate from supplied results and from what the client says about symptoms in daily life. Your assessment and the agreements are included as you state them. [1] [10]
Who determines the content?
You, as the respiratory physician. You check lung function, saturation and management plan before you save the report. [1] [8]
How does the report fit the respiratory clinic?
The structure follows respiratory symptoms, chest examination and the course. Measured values come back as you state them, you check them, and the medical judgement stays with you. [1] [10] [9]
What does Hysio not do?
In the respiratory clinic, Hysio works as an AI scribe: a saturation you do not mention is not included. It does not interpret lung function or a CT scan, does not make a diagnosis and does not choose inhaled medication. [8] [9]

Voice-driven reporting in respiratory medicine

Which documentation format do you start with when breathlessness is the reason for the visit? #

Dyspnoea is not a documentation format of its own but a starting point in the source: the symptom lands under the reason for visit and the detail belongs in the specific history. The format follows the type of contact, not the symptom. A first assessment fits the respiratory medicine intake, an outpatient contact the respiratory medicine consultation and a short follow-up contact the progress note. Describe the breathlessness in the patient's own words and leave inhaled medication as it was mentioned. [2] [3] [4] [1] [10]

How do lung function and saturation each get their own place? #

Lung function and oxygen saturation each get their own place under a fixed heading, so that measured value and assessment do not run together. Both belong under additional investigations, with method, pre- or post-bronchodilator status, date, measurement conditions and unit as the source gives them. Hysio does not convert anything and does not derive a percentage or classification. A qualification stated by the lung function laboratory stays with the report it comes from. [2] [1] [9]

Where does smoking status go in the record? #

Smoking status belongs in the specific history or in the medical history, depending on how the respiratory physician states it. Record what was literally said: smokes, stopped since, or never smoked. You only include a number of pack years when the source itself states that figure; Hysio does not calculate anything. Who gave the information, the patient or a relative, stays visible. That way the record pathway remains traceable months later. [9] [1] [2]

How do you handle sensitive information about saturation with care? #

Only include the saturation values needed for this contact, with the measurement conditions, and leave the rest of the measurement protocol out of the report. Name, date of birth, patient number and citizen service number (BSN) do not belong in the text Hysio produces. Sections the hospital manages separately, such as allergies, adverse reactions and the active medication list, also stay out of the report. You decide yourself what you share with the GP when you send it. [7] [10] [1]

Do you pay for voice-driven reporting per COPD or asthma check-up? #

The price does not depend on the subject of your report: documenting inhaled medication counts the same as documenting a lung function result. Hysio works with a subscription, with a monthly and a yearly option, where paying yearly gives you two months free. The current amounts and the number of sessions per plan are on the pricing page. You will also find the free trial conditions there. [5] [6]

Would you rather use your own template for breathlessness? #

Sometimes your own template for dyspnoea fits better than the Hysio format, for example when your department uses a fixed heading layout for the lung function report. There is no nationally prescribed heading structure for your own record, so that freedom exists. The letter to the GP is different: there the national guideline for information exchange between GP and specialist applies. Compare both routes on what the recipient needs. [1] [9] [10]

How do you know whether the lung function in the report matches the measurement? #

Always compare the report and the spoken dictation with the value actually measured in the lung function report. Pay attention to the unit and to the pre- or post-bronchodilator status: a measurement after bronchodilation is something other than a baseline measurement. Also check laterality and the date. If the text contains a value that does not appear in the source, it does not belong there. If a unit is missing, that remains as an uncertainty. [1] [10] [8]

How do you stay with the patient when you ask about smoking? #

When it comes to smoking status, focus first on the conversation with the patient, not on the form. Stopping smoking is a sensitive subject; let the question land before you record anything. Then dictate what was said, in the patient's own words, without adding a judgement. Hysio organises those sentences under the right heading and does not add any advice or next step that you did not state. [1] [2] [9]

What does Hysio organise for saturation, and what do you assess? #

Hysio organises oxygen saturation and visibly links it to the source it comes from, instead of turning it into a separate input field. A form asks for a number; Hysio carries over the value with measurement conditions and unit and leaves the interpretation to you. A conclusion from a supplied investigation report also keeps its own label and never slides into your own conclusion. That distinction is the real difference. [1] [2] [9]

As a respiratory physician, how do you get started with voice-driven reporting for a conversation about inhaled medication? #

At the input stage, start with a familiar contact and dictate the inhaled medication as the patient names it, with the use they report themselves. Hysio does not turn that into a medication list and does not suggest a dose or setting. Agree who reviews the report before it goes into the record. Only expand to another contact type once that agreement holds up in practice. [6] [7] [1]

Documentation formats for respiratory physicians

When do you use the consultation report in the respiratory clinic? #

Hysio Respiratory Medicine Consultation is the format for the full outpatient contact. The structure follows the reason for visit or referral, the medical history, the specific history, the physical examination, the additional investigations, the conclusion and the management plan. An ongoing episode fits in conditionally, so that changes in symptoms and exacerbations land in the history and series of measurements stay under additional investigations. For a short interim note, the progress note is more practical. [2] [1] [10]

How do you record the first conversation about breathlessness? #

Hysio Respiratory Medicine Intake is the format for the first comprehensive assessment. Alongside the sections of the consultation, it has a social history and a section on expectations, so that exposure, work, home situation and the patient's question stay separate. What the referrer reported and what the patient says themselves remain separate source roles. Content that falls outside the intake is only flagged at the end and not carried over. [3] [1] [10]

When does a SOAP note fit a short respiratory check-up? #

The SOAP note is intended as a third format for the respiratory physician. It organises a short, planned or telephone follow-up contact within an ongoing episode as a progress note: subjective, objective, assessment and plan, preceded by a short summary. That is a different destination in the record than the letter to the referrer. This structure is common in the hospital record, but it is not a guideline requirement for this specialty; Hysio therefore does not present it as a standard. [4] [1] [10]

Which format fits smoking status: consultation, intake or follow-up contact? #

Which of the three formats fits best depends on the contact, not on the subject. Smoking status appears in all three: in the intake as part of the comprehensive assessment, in the consultation in the specific history or the medical history, and in the progress note only when the patient says something about it during that short contact. So choose by contact type and let the subject find its own place. [1] [2] [10]

When do you choose an intake because saturation is falling? #

Choose an intake when oxygen saturation is being recorded for the first time and there is no earlier measurement in the record yet. The intake covers more ground and has room for social history and expectations. If it concerns a further measurement within the same episode, the consultation or the progress note fits better. A comparison between two measurements only goes into the text when you state that comparison yourself. [1] [2] [10]

Which details belong with inhaled medication when sources are mixed? #

With a mixed source, for example a dictation supplemented by a written handover, it stays visible per sentence who supplied what. For inhaled medication, that means: the medicine and its use as the patient reports it, and separately what the referrer wrote. Hysio does not add any dose, strength or change and does not create a medication overview. Only a medication decision that you state goes under the management plan. [1] [2] [10]

What do you put about breathlessness in a handover? #

In a handover, only include what the recipient needs: the reason, what was said about the breathlessness, your conclusion and the agreed management plan. The national guideline for information exchange between GP and specialist asks for the reason for the message, the conclusion, the plan and a recommendation for follow-up. A full lung function report does not need to be included; refer to it and keep the letter readable. [1] [2] [10]

How do you keep lung function values apart from what the client says? #

Always keep the patient's account, your own observation and the assessment of lung function separate, even when they come up in the same conversation. The measured values belong under additional investigations, together with the source. A conclusion from the lung function laboratory or the radiologist stays there with its own label. Your conclusion stands apart; if you do not state it, the field stays empty instead of being filled with someone else's judgement. [1] [2] [10]

How do you compare smoking status between two contacts? #

Compare smoking status between contacts on the same fixed points, and only name a change when the source supports it too. So write down what was said in this contact, with the date, and leave the previous report as it was. Hysio does not line up a trend or a number of years and does not draw a conclusion from two separate notes. A course over time only emerges when you state it. [1] [2] [10]

How do you record a saturation result? #

Record an oxygen saturation result with date and source, and keep the measurement conditions with it: at rest, after exertion or with oxygen. What you explained to the patient and what you agreed belong with the conclusion and the management plan, not with the measurement itself. If the investigation was done but the result was not stated, that fact remains instead of a value being filled in. [1] [2] [10]

Lung function, saturation and smoking

Does saturation belong in the history or in the examination? #

Only describe oxygen saturation in the history once the reason for the visit and the course are in the patient's own words. Strictly speaking, the value itself does not belong in the history but under the examination; the history is at most the place for what the patient says about it, for example a home measurement. Then record it as the patient's report, with the time of measurement, and not as an established finding. [1] [10]

How do you note who listed the inhaled medication? #

Always record exactly who supplied information about inhaled medication: the patient themselves, a partner or relative, the referrer or a nurse. Those source roles stay apart in the report and are never merged into one sentence. A job title in the source does not, incidentally, prove any authority, authorisation or decision. What the lung function professional or the radiologist wrote stays linked to that person. [1] [10]

How do you record breathlessness with a scale and time? #

For each measurement of dyspnoea, record the instrument, the unit and the time of measurement. If you use a breathlessness scale, write down the score as it was taken; Hysio does not calculate, scale or translate a score and does not assign a class itself. What that number means for this patient is for you to decide. Also state whether the measurement took place at rest or after exertion, because without that condition the number says little. [1] [10]

How do you link lung function to date and result? #

Always link lung function to the investigation itself, the date it took place and the report that carries the result. Copy the values verbatim from the source with method and unit, and keep the measurements before and after bronchodilation apart. A qualification such as an obstructive pattern comes from the cited report and stays attributed to it. Your own conclusion about what the result means stands separately. [1] [10]

Does Hysio assess a chest X-ray or CT scan of the lungs? #

Hysio carries over a supplied imaging report for smoking status, but does not assess any images itself. Smoking status rarely appears in such a report anyway; what does appear is the radiologist's finding. That stays as a quoted conclusion under additional investigations, with the laterality and the lung lobe exactly as they were named. The translation into a respiratory medicine conclusion is yours to make. [9] [8] [10]

Which terms do you agree on for saturation and oxygen? #

Use the professional term for oxygen saturation that is common within your team, and spell out the abbreviation where confusion could arise. Stay within the language of respiratory medicine: breathlessness and oxygen saturation belong here, terms for cardiac pump function do not, even though both specialties share the symptom. Hysio copies your term literally and does not harmonise anything, so a spelling you choose carries through the whole report. [1] [10]

How do you note what a relative says about inhaler use? #

Always state explicitly who information about inhaled medication comes from: the patient, a partner or relative, or the referrer. Information about the patient and about another person is never mixed, not even when the relative says something about themselves in the same sentence. If you want to share that information with the GP, first check the purpose and the consent. What the relative said stays attributed to the relative. [1] [10]

What should a colleague be able to read back about the breathlessness? #

Make it visible to the team which discipline supplied the information about dyspnoea and who picks up the follow-up. In a thoracic oncology meeting or in a sleep centre, the radiologist, pathologist, nurse and sometimes a colleague from another specialty sit at the table; their contributions stay recognisably separate. Avoid primary care integrated-care language in a secondary care report, because the lead practitioner and the funding differ there. [1] [2] [10]

What should an example case with lung function values show? #

An example case shows the real course of a contact: the lung function values, the conversation and the follow-up discussed, without a diagnosis being invented. A useful case also shows what happens when the source is incomplete, for example when the measurement was done but the result was not stated. That gap is then recorded as such. That way admin with AI shows exactly where your own assessment is needed. [1] [10] [2]

What do you put about smoking in a handover? #

Limit a handover about smoking status to source and purpose, and let the recipient find the full account in the record instead of repeating everything. For the GP, this is usually enough: what was discussed about smoking, which agreement followed from it and who picks it up. Smoking cessation support often runs in primary care, so state clearly who does what. Origin and date stay with it. [9] [7] [10]

Sources, privacy and boundaries in the respiratory clinic

Does Hysio make a diagnosis based on smoking status? #

Hysio organises smoking status clearly in the record, but does not make a diagnosis and does not determine an indication. No number of pack years is calculated, no classification is assigned and no link is made between smoking and an abnormality found. Active diagnoses only appear in the report when you state them. If you do not state a conclusion, that field stays empty with a fixed sentence saying so, instead of a filled-in assumption. [8] [1] [10]

Who reviews the saturation values before saving? #

You ultimately choose which content goes into the record and assess oxygen saturation yourself; Hysio does not take over that decision, not even with an almost finished text. In practice, that means placing value, unit, measurement conditions and date next to the source before you save. If you work in a team, agree who reviews and who authorises. Those two roles are not automatically the same person. [1] [8] [10]

What do you do if information about inhaled medication is missing? #

Explicitly mark that inhaled medication is missing or was not discussed, instead of silently passing over it. Hysio does not add any medicine, dose or date that the source does not mention and does not infer anything from an earlier letter. With an unclear or missing unit, the uncertainty remains. An empty field is useful information for the next reader; a filled-in field without a source is not. [1] [8] [10]

Is Hysio meant for use in acute breathlessness? #

Do not use Hysio for triage in acute dyspnoea: act medically first according to the applicable protocol. Hysio does not determine urgency, does not assign a severity class and does not calculate any blood gas or saturation value. Only after the acute assessment do you record what happened and what you decided. The same applies to a statutory notification: whether the criteria are met is for you to establish, not the text. [8] [7] [10]

When do you share lung function with someone else? #

For every handover of lung function, check the purpose, the consent given and the recipient. The referrer receives what is needed for further care; another practitioner or a relative only when that has been agreed and recorded. Record what was actually agreed, not what is customary. Hysio does not send anything itself and does not choose a recipient; sending and that choice remain your action. [7] [10] [1]

Does Hysio make a smoking cessation plan? #

Hysio puts smoking status clearly under a heading, but does not choose a treatment itself and does not suggest smoking cessation support, medication or a follow-up appointment. The management plan only contains what you stated, in the words you used. A suggestion from a supplied report remains a quoted suggestion from that source and does not become a line in the plan. So the judgement stays visibly yours. [8] [9] [10]

What do you write down when in doubt about a saturation measurement? #

Simply let doubt about oxygen saturation stay visible in the text, and do not make the sentence more definite than the source allows. If it is unclear whether the measurement took place at rest or with oxygen, that question remains. If the unit is missing, no unit is assumed. An open point is more honest than a smooth phrasing, and the next reader sees straight away where a question remains. [1] [10] [8]

What do you note when the pharmacy and the client name different inhalers? #

Keep both readings separate when sources on inhaled medication differ, and write down who said what. The referral letter sometimes names a different medicine than the patient reports; that difference is information and not an error you need to smooth over. Hysio does not choose between two sources and does not merge them. Which reading is correct is for you to establish, and that judgement goes with your conclusion. [1] [10]

How do you note medication alongside the breathlessness? #

Copy what the patient says about medication and breathlessness literally, with source and time of measurement. What the patient reports about their use during the contact may appear, tied to the source, in the appropriate section. Hysio does not turn that into a medication list, does not suggest a dose or oxygen setting and does not link a medicine to a symptom. The assessment of whether medication and symptom are related stays with you. [1] [10] [8]

How do you review lung function before you save? #

For lung function, always read the text back next to the source and the date, and correct discrepancies before you save. Check four things: value and unit, the measurement before or after bronchodilation, laterality, and whether the conclusion is really yours and not copied from a cited investigation report. If that is right, the text can go into the record. If you are in doubt, leave the sentence open. [1] [7] [8] [10]

Introduction and daily use

How do you prepare a handover of lung function? #

Agree in advance within the team who reviews a handover about lung function, and decide which values the recipient really needs. For the GP, the conclusion with a reference to the full report is usually enough; a colleague within the hospital more often wants to see the series of measurements itself. Keep the agreed follow-up recognisable as a separate part. Add the date of the measurement, otherwise you will not be able to place the value later. [9] [7] [10] [2]

Which team agreement do you make about recording smoking status? #

Agree fixed headings for smoking status within the team and shared terms for use, so that everyone reads the same text the same way. Record whether smoking status belongs in the medical history or in the specific history; both are defensible, but using them interchangeably makes searching difficult. Also agree who signs off before a report is final. Without that agreement, the record differs per practitioner. [1] [2] [9]

How do you start with a small trial around saturation? #

Start with a small trial: test in one clearly defined contact, and collect the corrections you make yourself. Choose a contact type that occurs often, for example the outpatient follow-up contact, so that you have enough examples within a week. Look especially at the measured values: are unit, measurement conditions and date correctly included? If something does not work, adjust one thing and test again instead of changing everything at once. [6] [7] [1]

How do you align the report on inhaled medication with your respiratory clinic? #

Check inhaled medication against the local headings of your practice, and lung function against the authorisations that apply there. There is no nationally prescribed layout for your own record, so the hospital template leads and not the other way round. For the letter to the GP, a national guideline does apply; do not depart from it lightly. Let the designated colleague review the result before it becomes the standard. [9] [1] [10]

How does voice-driven reporting help in a COPD check-up? #

At a check-up you mention symptoms, saturation, lung function and the inhaled medication. You say it as you discuss it with the client, and Hysio puts it in the report: that is voice-driven reporting. Measured values stay as you state them, with their unit. You check the management plan before it goes into the record. [1] [10] [2]

How do you measure how much time the report of a lung function conversation takes? #

Measure, check and collect corrections around lung function together with your team, instead of estimating a saving. Time a series of comparable contacts with and without Hysio, and add the review time; otherwise you are only measuring half. Also count the corrections you have to make to measured values. An honest picture from your own contacts is more useful than a percentage from a brochure. [1] [9] [6]

Which conditions apply during the trial period for a respiratory clinic? #

For smoking status, read Hysio's central trial conditions before you start, because they apply to the whole platform and not per subject. The free trial is meant for assessing in your own contacts whether the organisation of the report is right, so use real working methods and not invented examples. Agree in advance who reviews alongside you and where you record the findings. The conditions and the duration are on the pricing page. [6] [5] [7]

Where are the pricing and terms, alongside your respiratory centre's agreements? #

Hysio publishes its subscription rates centrally on the pricing page. The legal pages cover the processing of data and the division of responsibilities. Oxygen saturation, spirometry and other respiratory findings are not separate priced products. When choosing the platform, you look at the terms of use; when recording a result, at the source and your professional assessment. [5] [7] [8]

How do you record your own terms for inhaled medication? #

Agree within your team how you write inhaled medication, oxygen saturation and common abbreviations, and record that in a place where new colleagues will find it. Choose one spelling per term and stick to it; Hysio copies your words literally and does not turn them into a standard term of its own. Also agree which abbreviations you do or do not use in a letter to the GP, because a reader from another discipline sees it. [1] [9] [10]

How do you keep track of a case with fluctuating breathlessness? #

Split a complex case involving dyspnoea into the contact and the underlying sources, and handle each report separately before you put them side by side. Lung function, imaging, sleep study and laboratory each keep their own conclusion with source attribution; they are not merged into one statement. Your conclusion sits above them and is the only one that weighs the whole. Whatever is still open, you name explicitly in the management plan. [1] [10] [2] [8]
