Quick answer
Hysio supports haptotherapy documentation with attention to the client's words, the observation during the encounter and the professional interpretation. No claim of national registration or promise of effect is made. Only the recorded session, agreements and evaluation are shown.
For each session, you can read what Hysio records of the conversation, the exercise and what the client experienced, and which interpretation stays with you as a haptotherapist.
For the session record, Hysio Haptotherapy Session Report remains a format built around a haptotherapy session involving the client's input, observation, what is offered, response and follow-up. The six report sections run from summary and purpose of contact, via the haptomimic observations, to what was offered with the response, the meaning discussed and the follow-up. What the client reports, what you observe and what you offered each keep their own origin. A meaning discussed is only included when it was actually expressed. [1] [3]
Hysio Haptotherapy Intake is intended for the haptotherapy intake with the request for help, life context, body experience and expectations. The seven report sections run from referral, problem and request for help, via health, development and background, to haptomimics and introductory sessions, analysis, care plan and a supplementary section. A haptotherapy conclusion you have stated stays clearly recognisable as your judgement. If that conclusion is not given, at most a modest working hypothesis is added, based on what the client reported and what you observed. [1] [4]
Hysio Haptotherapy Evaluation is for a haptotherapy evaluation of experience, process, observation and the agreed completion. The report starts from the previously recorded goal and its source, and places alongside it the client's current experience, the current haptomimic observation and any measurement value provided. A difference between two moments is not calculated. Continuation, adjustment, referral, aftercare or closure are only included when you have actually discussed them. [1] [5]
Hysio Haptotherapy Intake is for a first conversation about the request for help, life context, body experience and expectations. Introductory sessions also belong to it as one whole. The client's words stay separate from your haptomimic observation and from your own conclusion. Screening and care already involved are only included if they were discussed. Missing experiences are not filled in; you assess and approve what later goes into the session record. [1] [2] [4]
Within the session record, Hysio Haptotherapy Session Report organises the information you recorded yourself or dictated afterwards. What the client reported, what you observed haptomimically, which conversation, experiential exercise or touch you offered and which response followed appear in separate report sections. Unintelligible or missing passages are not reconstructed. Names of people, the practice and the location are replaced by a general description. You check whether the origin of each section is correct. In this way, voice-driven reporting leaves the haptotherapeutic interpretation with you. [1] [2] [3]
For a session on body experience and contact, the report layout for haptotherapy sessions stays with what you have said yourself. No medical, psychological, trauma, attachment or personality diagnosis is added, and no physiotherapy language such as range of motion, muscle strength or massage. A haptomimic observation does not become a psychological explanation. Consent, touch, indication, effect or referral that the source does not support stay out. A measurement instrument is not chosen, completed or scored against norms. If your interpretation is missing, that place stays open. [1] [2] [3]
In the session record, the haptotherapist separates the client's words, their own haptomimic observation, the conversation or contact offered and the response to it. Client reports are given in indirect speech, so that they do not read as your own finding. Posture, mobility, motility and expression are observation and not a statement by the client. If no answer is given, that is not a finding: silence is not recorded as absent, normal or safe. [1] [2] [3]
Before saving to the session record, the haptotherapist places the named experience, observation, what was offered, response and agreement next to the original note. Check that a client report is not presented as your own finding, that an action offered is not written down as carried out, and that a test performed without a result is also stated as such. Only then do you save the version whose origin and terminology are correct. [1] [2] [3]
For a session record, no automatic integration with every medical record system is suggested. You read the text through and transfer it yourself into the system your practice works with. Consider which life context, which client words and which observation really need to go with it, and who may view the transferred report. Format and version control follow the arrangements of your own practice. The handover remains human work. [1] [2] [5]
For the session record, the haptotherapist chooses between session report, intake and evaluation based on the actual purpose of contact. The intake contains the request for help, life context and introductory sessions, the session report distinguishes the client's input, haptomimic observation, what is offered and response, and the evaluation looks back at a previously recorded goal. If the encounter has no earlier goal, it is not an evaluation. If in doubt, choose the format that fits what you actually did. [1] [2] [3]
The haptotherapist records the request for help and the life context in the session record with origin and encounter moment. Reason for contact, referral, expectation and reason for haptotherapy belong to the intake; housing, relationships, work, leisure and meaningful events only insofar as they were discussed. Background that did not come up is not added. If a part is missing, that passage stays empty and you note that you will still ask about it. [2] [4]
The request for help and body experience stay in the client's own words in the session record as far as possible, rendered in indirect speech. What you observe haptomimically during the contact is given a different source role from what the client tells you. A haptotherapy conclusion you have stated and any score provided follow separately, with scale and time of measurement. In AI reporting too, an observation does not become a medical or psychological conclusion. [2] [4]
Record body experience in the session record with the encounter in which the client describes it, in the section on the purpose of contact and the client's input. Keep those words separate from the section with haptomimic observations, which states what you saw in posture, mobility, motility and expression. Do not turn it into a medical conclusion. Your interpretation comes separately and you approve the report. [2] [3]
Keep the client's experience in their own words and link your own observation to the moment and to what you were offering when you made it. Those two sources must not replace each other unnoticed. If they contradict each other and you do not resolve that on the spot, both stay visible with their source role and you note that verification is needed. The documentation format itself chooses no meaning and no effect. [2] [3]
In the session record, the haptotherapist gives observation during exercises the same certainty that the source itself supports. An impression remains an impression and is not written down as a finding, diagnosis or proven result. Haptomimics is a framework for observation, not a scale with cut-off values. If you are unsure about the wording, choose the most cautious one the source still supports. What the observation means is for you to decide. [2] [3]
In the session record, the haptotherapist only chooses context around the professional interpretation that clarifies the purpose of contact. That can be the dated main goal, the client's report, the observation you base it on and any score provided with scale and time of measurement. Personal life context stays limited to what is needed for that purpose. Background that clarifies nothing does not belong there; you decide what stays readable and necessary. [2] [3]
Record follow-up agreements in the session record separately from the described experience and the observation during the session. When an outcome measure was actually used, method, outcome and session moment stay linked to the original source. The documentation format does not derive an agreement from an impression or observation. The haptotherapist discusses the follow-up with the client and approves the wording. [2] [3]
In the session record, connect the client's perspective with a goal that was discussed in the encounter or that you have set professionally. Keep the request for help visibly separate and then note the follow-up action, who takes it on and when you will evaluate. Hysio Haptotherapy Session Report does not create a plan from loose data itself and does not invent an agreement. A follow-up that nobody has stated stays out. [2] [3]
For a look back at the process, Hysio Haptotherapy Evaluation fits. That format places the client's experience, your current observation, the previously recorded goal and the follow-up discussed side by side. It does not independently choose completion and does not invent a point of comparison. You assess whether this encounter really is an evaluation, keep uncertain changes clearly open and approve the source-bound text. [2] [5]
When a haptotherapy programme is completed, the session record keeps three perspectives apart: the client's experience, your observation and the follow-up discussed. Note what has changed in body experience, what remains uncertain and which agreement supports the closure. An episode only counts as closed when you have stated that yourself. Aftercare, referral or a follow-up appointment are only included when they were really discussed. [2] [5]
Haptotherapy has private professional and quality frameworks; the unprotected professional name does not fall under article 3 or article 34 of the Wet BIG. State in the record which framework you work under, with publisher, date and type of document. Keep a private professional standard visibly separate from statutory rules such as the WGBO and the Wkkgz. A reference without version or date is not enough for traceability. [6] [8]
The session record makes client words, body experience, observation, professional interpretation, what is offered, response and follow-up each traceable separately. Haptotherapy does not work with a fixed letter scheme such as SOEP or SOAP; that layout belongs to another profession. Use only the part of the professional competence profile that fits your encounter, and note the document version and scope. A local practice arrangement stays recognisably local. [6]
A haptotherapy title does not prove BIG registration; any other protected professional role is recorded separately from it. The haptotherapist is not among the professions of article 3 and not on the published list of article 34. If you are also, for example, a physiotherapist or nurse, note that registration separately and only when you can demonstrate it. [8]
Where the Wkkgz applies, the practice describes a complaint and its follow-up factually, alongside the complaints procedure and the dispute resolution body it is affiliated with. When something is reported, note the facts, the date and what was done, without legal judgement and without the question of blame. Whether the law applies to your form of practice is something you check yourself; a private registration does not answer that question. [11]
The retention period follows from the specific treatment relationship, legal basis and practice arrangement, not from a general assumption about complementary care. If your encounter falls under the WGBO, the statutory record retention period applies; if it falls outside it, the GDPR determines that you do not keep data longer than necessary. Record for each situation which basis you apply. An entry in a private register does not replace that consideration. [6] [10]
An entry in the Register Haptotherapie Nederland is private and should be mentioned by name and validity without suggesting government status. It says that you meet the requirements of that register, and not that there is a legally protected title or authority behind it. Note the full register name, the validity and the date on which you checked it. A second, legally regulated professional role stays separate from it. [7] [8]
In a haptotherapy session, record the question about support and the questions about touch, about recording and about sharing data each separately. Consent for one never follows from consent for another. Note each time what you asked, what the client answered and at what moment. If it concerned sharing, purpose and recipient are added separately. If no answer is given, that does not count as consent given. [4] [6]
Share only necessary information about experience, observation and follow-up with the agreed recipient and on a valid legal basis. Check in advance the care purpose, who the recipient is and how much you really send. Life context that is not needed for that purpose stays in the record and is not sent along. Consent for sharing is a separate question and does not follow from consent for the support. Check the destination before you send. [5] [6] [10]
The haptotherapist safeguards professional interpretation and boundaries; a report may not independently make a medical diagnosis or promise of effect. The session report therefore never formulates a working hypothesis itself. In the intake, at most a modest working hypothesis may be added when you have not stated a conclusion yourself, and your own conclusion always takes precedence. What you save and which follow-up you agree on remains your assessment. [2] [3]
Review sources when register conditions, association information or supervisory texts change, and state the uncertainty until the review is complete. Compare the RHN professional competence profile and the register information with the current IGJ page when a safety or supervision claim changes. The session record keeps the professional standard, private registration and supervisory information clearly separate. Uncertainty remains visible throughout. [6] [7] [9]
An outcome measure only has a place in the session record when it was actually used during the encounter. Note name, scale, outcome and time of measurement, and keep an experience score and a physical measurement in their own system. Nothing is calculated, and two moments are only compared when you state that comparison yourself. If a test was done but no result was mentioned, that is how it is recorded. [2] [3] [6]
In a session on body experience and contact, the haptotherapist only uses terminology they have said themselves. Mobility and motility remain different terms and are not corrected into each other because one sounds more likely. Haptomimics describes posture, appearance, expression, gestures, use of language, breath participation and presence, and remains observation. Physiotherapy or psychological terms are not added. Before saving, assess whether every term is necessary, precise and readable. [2] [3] [6]
The haptotherapist leaves the relevant part of the session record empty when the source says nothing about body experience. A missing item is not reconstructed and silence does not become a finding: so it does not say that there was nothing or that everything is normal. Note that the point is still open, or ask about it specifically at the next encounter. [2] [3]
Where there are contradictions about the contact and the experience, the haptotherapist records source, moment and wording separately. A correction you state on the spot replaces only that one item and not the rest of the passage. If the conflict remains, both readings stay visible with their source role and you note that verification is needed. Correction is made from the client's words or the recorded observation, never from interpretation afterwards. [2] [3]
The boundary around observation during exercises is that the haptotherapist leaves it as observation. It is not turned into a medical or psychological explanation, not into an effect and not into proof that what was offered has worked. What the client reports, what you see and what you think of it each keep their own origin. A convincing wording does not replace a missing source. Giving meaning to contact, what was offered and response remains your work, and you approve that interpretation. [2] [3]
When a client reports a care concern during a session, the session record keeps their words separate from what the haptotherapist observes. The encounter moment, the limits of what was offered and the agreed follow-up action are stated concretely. The documentation format gives no medical interpretation and does not determine urgency. The haptotherapist assesses for themselves what fits within their role and which support or referral turns out to be needed. [2] [3]
The session record keeps the client's words about body experience separate from your observation, from what you offered and from the response that followed. In addition, a statement by another care provider, a dated record source and an agreement or plan each stay recognisable in their own source role. A score provided keeps its scale and time of measurement. This way, you can see for each sentence where it comes from. [3]
The client's experience and an observation during an exercise are not converted into a code, classification or effect score. If you actually use an outcome measure, name, scale, outcome and date are carried over faithfully to the source. No value is calculated, nothing is scored against norms and no medical classification is chosen. If a code is missing in the source, that place stays empty. Your interpretation and the follow-up discussed remain separate components. [2] [3] [6]
Experienced change and professional observation stay distinct from a general efficacy claim. What the client says they notice is their experience, what you see is your observation, and whether one is related to the other is a judgement only you can make. No difference between two moments is calculated and no causal link is made. A change without a stated judgement therefore simply remains a reported change. [2] [5] [9]
Before saving to the session record, compare the named experience, context, outcome measure used and time of measurement with the source they come from. Does every term match what you said, are scale, unit, side or location still included, and has nothing been calculated that the source does not support? If an item is missing, that part stays open. Finally, remove any independent diagnosis, efficacy claim or treatment outcome. [2] [5]
For haptotherapy record keeping, the general subscription information is relevant first: Hysio Starter costs €20 per month, Pro €40 and Unlimited €80, each excluding VAT. If you pay yearly, you get two months free. There is no separate price for voice-driven reporting in haptotherapy. See the pricing page for the current terms. [12]
Sensitive information about life context and body experience is only included if it is needed for the support and the agreed purpose of the record. As a practice, record who has access, who reviews the text and when you disclose anything, and arrange the data processing agreement. Keep client words and your own observations separate and check the recipient in advance. That consideration stays with the practice and not with the documentation format. [13] [14]
For the session record, the haptotherapist compares writing by hand with dictation. Writing by hand gives full control over every sentence and over the layout; dictation produces running text that you still organise yourself afterwards. A general chatbot does not know the haptotherapy source roles and easily adds meaning that nobody has expressed. A profession-specific documentation format organises the source into fixed sections and leaves the interpretation with you. With every way of working, you read through what goes into the record. [2]
A practice offering experience-oriented care assesses Hysio, including the haptotherapy documentation formats, during a free trial of fourteen days: recording or dictating afterwards, the organisation into report sections, tracing the origin and the lead time to a final text. Apply the haptotherapy yardstick: do client words, observation, what is offered and response stay separate, and does the interpretation stay yours? Note which text you correct yourself before you save anything. [12]
No, there is no automatic link with every record system and Hysio does not promise one either. You read the haptotherapy report through and transfer it yourself into the system your practice works with. Agree who transfers it, which sensitive life context stays out of the handover and who manages the saved version. Without that agreement, it can no longer be traced later which text ended up where. [2]
The haptotherapist checks whether client words, body experience, session observation and professional interpretation are clearly separate, whether every term is correct and whether nothing has been added that nobody said. Agree within the practice who reviews, at what moment, and who saves and keeps the final version. The haptotherapist safeguards the boundary of what is offered and decides which follow-up is discussed with the client. The documentation format organises sources and does not establish an effect. [2] [3]
An error in a follow-up agreement is not fixed by also changing the described experience or session observation. The haptotherapist goes back to the correct encounter source, keeps the earlier wording and explains which part has been corrected. The session record states the review and only keeps the checked version for further record keeping. [2] [3]
If the client's perspective is not in the session note, the haptotherapist checks whether the experience was recorded verbatim elsewhere for the same encounter. An observation of your own during the exercise must not replace those words. Without an additional client source, the item stays open or is asked about again. The documentation format does not attribute an assumed experience or effect. [2] [3]
The haptotherapist measures time for the same type of session or process evaluation, and then across a series of encounters rather than just one. Time the start-up, dictating or recording, reviewing for separate source roles, correcting and saving separately. No time savings have been measured for this practice. Only your own comparison shows what admin with AI brings you. [2]
Start the rollout with one session type and a haptotherapist who reviews the client's words, observation and interpretation. Make agreements in advance about sensitive life context, about consent, about who reviews and about where the final text is kept. Only expand to a next type of encounter when the first reports are source-bound, readable and appropriately scoped. [2]
Hysio creates the report. You remain responsible for what you save in the record.
