Quick answer Midwifery reports for every stage of care 50 questions with cited sources

Quick answer

Midwifery documentation for every stage of care

Hysio organises the pregnancy intake, antenatal check-up, birth plan, chronological birth report and postnatal handover separately.

Gestational age, maternal and fetal data, parents' reports, measurements and agreements stay linked to their own source and time point.

How does it work in midwifery practice?
After an antenatal check-up you dictate what you saw and discussed, and Hysio turns it into a report. For midwives, that is voice-driven reporting, also known as AI documentation. There are five report types: intake, check-up, birth plan, birth report and postnatal check with handover. You check the content. [7] [13] [19]
What does each report keep track of?
What is reported, observations, measurements, your assessment and the follow-up, each at its own time point. With AI reporting you dictate a check-up and the gestational age stays with the right measurement. [7] [13] [19]
How are the pregnant woman and the baby kept separate?
Details about the pregnant woman and the unborn baby each sit in their own part of the report. [7] [13] [19]
Who has the final say?
You decide what goes into the midwifery record. Hysio is built to be the most reliable choice for midwifery reports: every stage of care in your own words, with your check as the final step. [7] [13] [19]
Where does Hysio stop?
Hysio does not fill in missing midwifery details and makes no decision on risk, policy or referral. [7] [13] [19]

Voice-driven reporting in midwifery

Which reports do you make as a midwife with voice-driven reporting? #

For midwifery, Hysio aligns with five encounter purposes: pregnancy intake, antenatal check-up, birth plan, chronological birth report and postnatal check with handover. The format follows the moment of care and keeps information about the pregnant woman, the unborn baby, wishes, parents' reports and agreements clearly separate. In the birth plan, the stated wishes, the scenarios discussed in case labour takes a different course and the partner's role each have their own place. The midwife checks source, time and professional meaning before the text goes into the midwifery record. [3] [4] [5] [6] [13] [19] [20]

What does the midwifery pregnancy intake record? #

The pregnancy intake organises the reason for care, the current pregnancy, obstetric history, medical and psychosocial context, named measurements, professional assessment and agreements. Information about the pregnant woman stays distinct from information about the unborn baby and from general source notes. Pregnancy history, midwifery examination, analysis and care plan remain separate parts of the intake report. [3] [7] [13] [19]

What belongs in a report of an antenatal check-up? #

The midwifery check-up brings together the encounter purpose, the pregnant woman's report, maternal and fetal findings, recorded measurements, the stated assessment and the follow-up. The layout shows what was reported, what was examined and which agreement was actually made. Stage of pregnancy and encounter purpose appear at the top, so that it remains clear later at which gestation this encounter took place. [4] [13] [19]

How is a midwifery birth report structured? #

The birth report follows the source-bound timeline from admission and the course of labour to the birth, placenta, condition after the birth and follow-up. The client and the newborn each have their own place, so that a baby's data is not inadvertently read as a maternal finding. Times that are mentioned are copied as they were stated and are not reconstructed afterwards. [5] [13] [17]

What does the postnatal check with handover organise? #

The postnatal check organises the moment of the check, the client's current data, the newborn, feeding and parents' reports, the stated assessment and follow-up actions. The handover stays a separate part with its own recipient and status, not a summary that makes new care decisions. Hysio never writes that a handover has already been sent or received when the source does not say so. [6] [13] [17]

When is a pregnancy intake a better fit than a check-up report? #

Choose the pregnancy intake when the first conversation maps out the reason for care, pregnancy, history and context. Choose the check-up when an ongoing antenatal encounter with a current report, findings and follow-up is central. The choice follows the actual encounter, not the amount of text. If in doubt, look at whether the history is being taken again or whether only the current course is being followed. [3] [4] [7] [13]

When do you use the birth report format in midwifery? #

The birth report format is intended for a recording of the birth and dictation made afterwards about it, when sequence and times are the core. Birth, placenta, client and newborn are described separately. A separate question about the pregnancy or a later check-up does not automatically belong in this episode. What was recorded during the encounter and what was dictated afterwards stay recognisable as two sources side by side. [5] [13] [17]

When is the postnatal handover relevant in the record? #

The handover becomes relevant as soon as the postnatal period passes to someone else: kraamzorg (postnatal maternity care), youth healthcare, the GP or a colleague who does the next check. Use it only for information that was actually recorded for that recipient, with reason and status included. An open action stays open until the source carries a completed status. The check-up report and the handover remain two parts, so the postnatal check can also be read on its own. [6] [13] [17]

What happens to missing information in a midwifery report? #

Missing information stays empty or is kept recognisable as not discussed, not examined or not known when the source makes that distinction. Hysio does not calculate gestational age, does not fill in a measurement and does not turn an intention into a plausible agreement. A main section without source information keeps its heading and gets a fixed sentence saying that nothing has been recorded, so the gap stays visible. [3] [4] [7] [13]

Can Hysio determine a midwifery risk or care plan by itself? #

No. Hysio can organise recorded reports, observations, measurements and the stated assessment, but does not carry out maternal or fetal risk selection, triage, CTG interpretation, birth management or referral decisions. The midwife remains responsible for professional meaning and approval. An assessment only appears in the report with attribution, so it remains clear who stated it. [3] [4] [5] [6] [7] [13] [19]

Check-ups, birth and postnatal care

How does Hysio support a midwifery pregnancy intake? #

During an intake, Hysio gives the midwifery information a fixed order: reason for the encounter, pregnancy, history, context, examination, stated assessment and agreements. The source takes precedence for dates, gestational age and measurements. Parts not discussed are not filled in with general midwifery knowledge. You conduct the conversation as you are used to and dictate afterwards if needed; the structure is only applied after that. [3] [7] [13] [19]

What happens to a short antenatal check-up in the report? #

A short check-up leads to a short report. Hysio does not turn a sparse encounter into a full antenatal checklist and leaves a part without source information recognisably empty. The fixed parts do remain: the pregnant woman's report, maternal and fetal findings, measurements, the stated assessment and the follow-up. At most, a short note may appear at the bottom about a missing measurement condition or a verification point that the source itself already mentions. The more completely your check-up is recorded, the emptier that space stays. [4] [13] [19]

How do you keep maternal and fetal data apart? #

Record data about the pregnant woman under the maternal source role and data about the unborn baby under the fetal source role. For a measurement, state the subject examined and the moment. Hysio does not merge the two roles and does not infer a condition from a loose note. In the check-up report, maternal and fetal findings therefore each appear under their own heading. [3] [4] [13] [19]

Why is the timeline important in a birth report? #

During a birth, the timeline determines which event came first, which source belongs to it and what was recorded later. Hysio organises the named moments around admission, course, birth and placenta. The system does not reconstruct missing times and does not turn sequence into independent birth management. That way it remains clear afterwards what was known during labour at the moment you acted. [5] [13] [17]

What do you record during a postnatal check? #

During a postnatal check you note per person: how the client is doing, how the newborn is doing, how feeding is going and what the parents themselves report. Only note values and agreements that were actually mentioned. That way the handover stays traceable, while the format itself makes no judgement about the need for further action. Weight trend, postnatal day and recovery are not calculated; only what was measured or said goes into the report. [6] [13] [17]

How do you process a report from parents in midwifery documentation? #

A parent's report gets its own source role and is not rewritten without a source as an observation or professional conclusion. The record states who made the report, at what moment that happened and what the report related to. The midwife determines which meaning and which follow-up result from the encounter. In the postnatal report, feeding and parents' reports therefore appear together in a part of their own. [6] [13] [17]

How do you record a handover after the birth clearly? #

A handover after the birth names the relevant episode, current maternal and baby data, parent information, open points, recipient and agreed follow-up. Hysio keeps the action owner and status recognisable. A handover sentence is no proof that an examination, phone call or referral has already been carried out. The handover remains a part of the report that you read, adjust and pass on yourself. [6] [13] [17]

How do you handle source notes alongside the conversation? #

Use source notes as a separate source and keep the document date and origin when they have been supplied. A current statement by the midwife or pregnant woman does not silently overwrite an older note. Hysio can show the difference; you check which information applies to this encounter. What you contribute yourself carries the most weight, then a linked document, then what others have said. [3] [7] [13]

How do you make open follow-up actions visible? #

Write a follow-up action with the wording, responsible person and status as named in the encounter. Distinguish planned, discussed, carried out and still open. Hysio does not fill in a missing action owner and does not turn an intention into a completed handover. An action without a named owner stays visible without an owner, so you see what still needs to be assigned. [6] [13] [17]

Is Hysio intended for real-time decisions during a midwifery encounter? #

No. Hysio is intended to organise recorded information after or during the encounter, not to make real-time emergency decisions. In an abnormal or acute situation, the professional assessment remains decisive. The report format can record facts and agreements after you have acted or consulted. In an acute birth you record what you did and when, not what you should do. [4] [5] [7] [17] [19]

KNOV standards and the record

Which source roles belong in a midwifery record? #

Distinguish between what the pregnant woman, partner or parents report, the midwife's observation or assessment, information from a document or another healthcare professional, a plan or intention, and an action that was demonstrably carried out. The last two are the easiest to mix up: a planned check must never be written as done. Before the birth she is the pregnant woman, during and after the birth the client, and the baby is the newborn. [3] [6] [7] [13] [19]

How do you process information from a partner in a report? #

Information from a partner or another person present stays recognisable as that person's report and is only included when it is relevant to the encounter. Do not mix that information with the pregnant woman's own words. The midwife checks necessity, consent and the correct source before it is saved. What a partner says about themselves only belongs in this record when it affects the care for the pregnant woman. [3] [7] [13]

How do you record data sharing in a postnatal handover? #

For data sharing, note the purpose, the recipient and the agreements that were actually discussed. Keep maternal and baby data separate and do not include more than is needed for the stated follow-up. Hysio does not infer consent from silence or from the fact that a handover seems desirable. A referral letter or care chain letter itself does not belong in this report format; only the agreement that a handover will take place. [6] [13] [17]

What do you do when two midwifery sources contradict each other? #

Keep a difference between conversation, record note and external source visible and state which source and date belong to it. Hysio does not silently choose the most likely version. The midwife determines which verification is needed and which corrected text ultimately goes into the record. If the gestation or estimated due date differs, that difference remains in place until you have identified the correct source. [3] [4] [7] [13] [18]

What role can the Perinataal Woordenboek play? #

The Perinataal Woordenboek & Dataset (Perinatal Dictionary and Dataset) provide an information standard for data exchange in maternity care. Only use terms and fields when the source and the work process support them. Hysio does not turn a dataset field into missing clinical content and does not promise any technical exchange. The standard determines what data is called, not which care was provided or recorded. [3] [6] [13]

How do you handle prenatal screening information in the record? #

Prenatal screening information stays linked to the source, date and person involved. A reference to Peridos or a screening document is not a result in itself. Hysio only includes the finding or agreement that was actually supplied and does not carry out screening, interpretation or risk classification. Counselling about screening belongs in the intake or the check-up and does not get a report format of its own. [3] [4] [13] [14]

How do you correct an error in a midwifery report? #

Compare the sentence with the original report, measurement note or professional note and correct the text on the basis of that source. With an unresolved difference, keep both versions or the uncertainty visible. The midwife approves the change; Hysio does not determine which source takes legal or clinical precedence. Correct before saving; a report that is already in the record is changed according to the arrangements of your own practice. [3] [4] [7] [13] [18]

How do you limit sensitive information in maternity care documentation? #

Only include information that is needed for the midwifery encounter or the agreed follow-up. Keep psychosocial, medical, maternal and baby data with their own source role and leave out background that was not discussed. The midwife assesses relevance, purpose and the final record content. A sensitive detail that does not concern this encounter is better left out than summarised. [3] [6] [7] [13]

Who remains responsible for the content of the midwifery record? #

The midwife remains responsible for professional interpretation, risk assessment, care plan, consultation, referral and approval. Hysio only organises the source-bound information supplied for the encounter. Neat wording does not turn uncertain content or content not discussed into an established fact in the record. That responsibility follows from your own professional practice and registration, not from the chosen report format. [3] [4] [7] [8] [19]

What should a colleague be able to find in a postnatal handover? #

A colleague must be able to see which postnatal moment the handover relates to, which maternal and baby data are current, what the parents reported, which assessment was recorded and which actions are still open. The recipient and status belong with it. Hysio does not turn that into a new care assignment. That way a colleague can take over the postnatal care without first having to ask you what was agreed. [6] [13] [17]

Gestational age, CTG and referral

How do you handle gestational age in a report? #

Only copy gestational age, estimated due date or day of pregnancy verbatim when it is explicitly in the source. Hysio does not calculate any duration, date or interval; a stated notation such as 36+0 stays as it was said and is not derived from a date. If in doubt, note the source and the verification point, so the midwife assesses the meaning. The duration of amenorrhoea and the chosen dating method also stay as the source names them. [3] [4] [7] [13]

What context belongs with a midwifery measurement? #

A measurement can only be read back properly with the subject, the person examined, unit, method of measurement, date or time and source. Hysio puts that information together when it has been mentioned. The platform does not independently compare the value with a norm and does not turn it into a risk assessment. A value without a unit or measurement time stays incomplete in the report and is not filled in. [4] [7] [13] [19]

Can Hysio interpret a CTG or a fetal finding? #

No. Hysio can organise an explicitly described fetal finding or a CTG assessment recorded by the healthcare professional, but does not read a CTG and does not draw its own conclusion. The source must make clear who made the assessment, when that was and which follow-up was actually agreed. The same applies to fetal heart sounds, presentation and growth: only what was stated goes into the report. [4] [5] [7] [13] [19]

How do you use PWD terms without adding extra meaning? #

Use a term from the Perinataal Woordenboek in the meaning and context in which it was supplied. A code or field name does not replace professional text and does not prove a result. Hysio keeps the source term recognisable and lets the midwife check its meaning for the record. Do not translate a source term into smoother wording; the original words carry the meaning. [3] [6] [13]

What happens to a test that was mentioned but has no result? #

When a test is mentioned, write that it was carried out and that the result was not stated verbally, if that is what the source shows. Hysio does not add a result, classification or reassurance. The midwife decides whether another source or verification is needed. When you handle admin with AI, a requested test without a result still remains an open item. [3] [4] [7] [13]

How do you keep stages of labour and times tied to the source? #

Copy the named stage, event and time exactly and link them to the source in which they appear. Hysio does not correct a missing sequence, does not calculate a duration and does not infer birth management. The midwife checks the chronology before the report is approved. If a time is missing, that slot in the chronological sequence stays empty. [5] [13] [17]

How do you describe placental and postpartum data carefully? #

Describe placental and postpartum data as separate source-bound parts alongside the event of the birth. Only state what was recorded about the client, placenta or follow-up and keep time and unit with values. Hysio does not explain a cause and does not independently assess whether the status is abnormal. Blood loss, condition and scores come from the source and are not inferred from the further course. [5] [6] [13] [17]

Why are units and measurement times important in the postnatal period? #

Unit and measurement time show what a value refers to and how it came about. That matters when maternal or baby data stand side by side. Hysio only uses the supplied value, without rounding or comparing. The midwife checks source, person and professional meaning. A newborn's weight and a value of the client therefore never end up on the same line, and weight loss or postnatal day is not calculated. [4] [6] [13] [17]

May Hysio translate a midwifery term into a diagnosis? #

No. A midwifery term, observation or parent's report stays distinct from a diagnosis or risk classification. Hysio can literally organise a stated professional assessment, but does not invent a diagnosis and does not make a VIL (Verloskundige Indicatielijst) interpretation. The midwife determines which term is fit for the record and correct. If a term stays unclear, it appears in the report as it was said. [3] [4] [7] [19]

What do you do with an abnormal or unclear source value? #

Keep an abnormal or unclear value visible with source, unit and uncertainty, and do not add an explanation. Hysio does not derive urgency, advice or referral from that value. The midwife assesses the situation, consults a source if needed and records the follow-up themselves. A value you already doubted yourself also stays, with the doubt you expressed about it. [4] [6] [7] [13] [17]

Practical questions about Hysio

How do you start voice-driven reporting in a midwifery practice? #

Start with the encounter purpose: intake, antenatal check-up, birth or postnatal handover. Then only record the source that belongs to that moment and read the proposal back in full. That way you test the structure against your own way of working without leaving a clinical decision to the platform in advance. Go through the first reports alongside your own record text, so you see where the layout differs from your habit. [3] [4] [5] [6] [13] [19]

Does Hysio automatically create a technical link with my record? #

No, not by itself. Hysio provides checked, copyable report text; there is no automatic link with every record or information system. Whether a link is possible in your environment depends on your own system and supplier and requires a separate check. As long as there is none, you place the text in the record you work in yourself. You remain responsible for the correct place, source and approval. [3] [6] [13] [19]

How do you check a proposed midwifery report? #

First read back the pregnant woman, the baby and any parents' reports per source role. Then check dates, times, units, negations, measurements, professional assessment and follow-up agreements. Remove everything that does not follow from the source and only approve when the text correctly reflects the actual encounter. Pay particular attention to negations: an undiscussed topic must not appear in the report as rejected or ruled out. [3] [4] [5] [6] [7] [13]

Can Hysio fill in missing midwifery content? #

No. Hysio can leave a missing part recognisable and support source verification, but does not fill in a risk factor, gestational age, measurement, diagnosis, care plan or agreement. Use an empty space as a signal for professional review, not as an invitation to accept a likely answer. Especially with fixed, familiar topics the temptation to fill them in anyway is strong; Hysio does not take that step. [3] [4] [7] [13]

How do you use Hysio for a handover to a colleague? #

Choose the postnatal handover when that is the actual encounter purpose. Then weigh, part by part, whether the recipient really needs it; a handover is not a copy of the entire postnatal record. Hysio organises the information, but you decide whether sharing is appropriate and whether the text forms a correct professional handover. Only pass on the text after you have compared it with your own record note. [6] [13] [17]

Can Hysio determine when a pregnant woman should be referred? #

No. A possible referral, consultation or escalation remains a professional decision that can only be recorded when it was discussed or carried out in the source. Hysio can organise what was reported and the agreed follow-up, but does not determine urgency, indication or recipient. A stated doubt about referring stays a doubt in the report, not an indication. [4] [6] [7] [17] [19]

How do parent and baby data stay separate in Hysio? #

The report structure has separate parts for the pregnant woman or client, the newborn and the parents' reports. Check names of people, measurement times and source roles. Hysio does not add baby data to the maternal assessment and does not create a combined health picture. If a value is next to the wrong person, that is the first error you notice when reading back. [5] [6] [13] [17]

What is the midwife's role with Hysio? #

The midwife chooses the appropriate encounter purpose, assesses source and context, corrects incorrect wording and approves the final text. Hysio helps with organising and wording, but does not take over any professional assessment, birth management, risk selection or decision on data sharing. You are therefore both the source of the report and its last reader before it goes into the record. [3] [4] [5] [6] [7] [8] [19]

How do you keep the documentation usable for the maternity care chain? #

Use recognisable source roles, a clear time point and a concrete distinction between report, observation, measurement, assessment and follow-up. Only include information relevant to the recipient. Hysio supports that readability, while the midwife keeps the content of and responsibility for the handover. Further along the care chain, the maternity care assistant, GP or gynaecologist also read the report; what is not traceable for them lacks context. [6] [9] [13] [17]

What does Hysio help with in midwifery reports, and what doesn't it do? #

Hysio organises a recorded intake conversation, a check-up, a birth plan conversation, a birth or a postnatal encounter in the matching report format. The platform does not select risk, does not interpret a CTG, does not decide on birth management or referral and does not fill in missing facts. You check and decide what is saved in the record. Organising is therefore something other than assessing, and only the organising lies with Hysio. In voice-driven reporting, the conversation with the pregnant woman stays yours and Hysio only provides the structure. [3] [4] [5] [6] [7] [13] [19]
