Quick answer
The four formats fit public health consultation, case investigation, societal assessment and handover to partner organisations.
For every task level, from individual contact to population, there is a fitting format. Population, source, public mandate, the physician's assessment and follow-up stay separately recognisable.
Public Health Consultation organises task, context, facts from the source, the physician's assessment and follow-up for an individual or collective public health contact. The documentation format keeps the personal question of a client or institution separate from the wider public health duty within which the contact takes place. This makes it clear which part is a source fact and which part is the physician's own assessment. With voice-driven reporting, you dictate the source facts and your assessment after the contact; Hysio keeps them apart in this format. [2] [7] [3]
Case Investigation & Contact Report records investigation contact with symptoms, test sources, possible exposure, actions and the physician's assessment. The documentation format separates the timeline of the investigation from the actions taken, so that a test result, a measure taken and an administrative agreement do not merge into one sentence. For the public health physician, this keeps it visible which step rests on which source and at which moment. [2] [9] [4]
This documentation format is intended for an assessment with population definition, source provenance, exposure, limitations and stated options. It fits a question about a defined group, such as a school class, neighbourhood or work setting, where the source and the period of the investigation must remain named. The physician then explicitly links limitations and advice options to that definition. [2] [8] [5]
This documentation format is intended for a handover to partner organisations with purpose, recipient, case timeline, facts from the source, legal basis and status. Record recipient, data boundary and follow-up status separately for a traceable public health note. The physician assesses the scope and professional conclusion of the public health handover to partner organisations. The final professional review stays with the public health physician and is not handed over to Hysio. [2] [12] [6]
A contact for a public health duty calls for task level and context alongside the individual question. Think of a notification, a request for advice from an institution or an individual consultation with a public component: each variant calls for its own combination of assistant and source role. Who asks the question and the associated task level determine the appropriate report structure, rather than the subject alone. [2] [7] [3]
A population description needs a definition, period, location and source to be traceable. Name the group investigated concretely, for example by age, neighbourhood or period, and link each figure to the source it comes from. Without that link, a population description is no more than an impression. [2] [8]
A case describes a specific contact or record, while population data concern a defined group. A case fact belongs to one person or event; a population finding applies only to the group and period the investigation covered. If you mix the two, you end up with a conclusion that is correct for neither the case nor the population. [2] [7]
A public health question can concern risk, exposure, prevention, surveillance or the organisation of care. Record the task level of the question: whether it concerns an individual contact, a case, a group or a policy question. The physician then assesses the scope and draws the professional conclusion that fits that level. [2] [7]
A signal, test or notification is not the same as an infectious disease diagnosed by the physician. Hysio can organise the signal, the test result and the source side by side, but does not make a diagnosis itself and does not classify a clinical picture. That determination stays with the physician, based on the designated clinical and laboratory sources. [2] [9]
A public health measure needs a competent source, context of application and status. Note who pronounced or established the measure, for which situation and target group it applies and whether or not it has been confirmed. This prevents a provisional or local measure from being read as a general recommendation for every situation. [2] [10]
Case investigation can combine the physician's dictation, test source, notification, contact information and context documents. State the origin, the date and the role each source plays in the case timeline, so that a test result is not read separately from the moment the sample was taken. If that origin is missing, the data item stays visible as unconfirmed instead of being left out. [2] [9]
Possible exposure is a source data item with place, period, route or uncertainty and not a proven cause. Record what the source itself says about place and period, and keep separate what is still a suspicion. The physician then assesses whether the exposure is relevant to the further case or measure. [2] [10]
A named contact is a case data item that only gains meaning within the source and the applicable public health route. State name, relationship and period only as they were mentioned in the conversation or record, without filling in a missing detail yourself. The GGD or competent authority then determines whether and how that contact is approached. [2] [9]
A test result calls for test source, sampling date, method, result and any limitation. Always place the result next to the laboratory or source that issued it, so that a positive or negative value is not read as a stand-alone fact. A missing method or date then stays visible as an open point. [2] [10]
A notification signals information to the public health chain, while confirmation can have a separate source or physician status. So note separately when something was notified, when it was investigated and when a confirmation was established by the competent authority. This keeps a provisional signal clearly distinct from a definitive, confirmed outcome. [2] [9]
A physician's assessment is an explicit professional interpretation within the task, sources and limitations of the case. Attribute the interpretation to the physician and keep the underlying facts from the source separate from it, so that an observation does not tacitly become a conclusion. Only then does the assessment remain traceable to the person who made it. [2] [7]
A missing date, source or contact step is relevant uncertainty in a public health assessment. Name the missing part explicitly in the note, instead of tacitly closing the case timeline with an assumption. An open point then stays recognisable for whoever later assesses, takes over or adds to the case. [2] [8]
The GGD is part of the public health chain in which notifications, investigation and measures each have their own role. The GGD usually carries out source and contact tracing and outbreak control, while the public health physician keeps their own professional assessment separate. Do not confuse that chain role with an individual physician's conclusion. [2] [10]
Source provenance shows where a fact comes from, when it was obtained and how certain its status is. Put the source, the date and any limitation next to each statement, so that a suspicion does not look like an established fact. That makes the note verifiable later against the original source. [2] [12]
Contradictory population data call for comparing sources and a visible limitation of the conclusion. Place both sources with their origin side by side and name the difference, instead of choosing an average or preference yourself. The physician then assesses which source should take precedence for this specific case. [2] [8]
The Wet publieke gezondheid (Dutch Public Health Act) forms a legal framework for tasks around public healthcare and infectious disease control. Among other things, the act regulates the municipal task in youth healthcare and the notification route via the GGD, but it does not by itself determine what applies in an individual case. Applying that to the concrete situation remains up to the public health physician. [2] [8]
The RIVM information describes the notification requirement and the current route for designated infectious diseases. Because notification categories and deadlines can change, a notification should always state exactly which RIVM source and which date were consulted. A previously consulted version must therefore not be assumed to be current without a new check. [2] [9]
Osiris is a national source and registration route used for notifications in the public health context. Hysio can organise the facts from the source for that registration route, but does not write to Osiris itself and does not assign an automatic notification status. The actual registration and its assessment stay with the competent physician and authority. [2] [9]
Youth healthcare data have their own context, information standard and careful choice of source and access. Keep child, parent and healthcare professional as separate roles in the note, and do not fill in a missing data item based on an assumption about age or situation. The Informatiestandaard JGZ (youth healthcare information standard) remains the starting point for what is and is not recorded. [2] [11]
Relevance follows from the purpose, the task, the source and the chosen population or case definition. Include only data that actually serve the public health duty in question, and leave out other personal data from the source. This keeps the note limited to what is needed for this task. [2] [12]
A legal basis must be evident from the designated source or assessment and must not be filled in by Hysio. Note the legal basis only as the physician or the designated law names it, without adding your own legal reading. If that naming is missing, the legal basis stays an open point in the note. [2] [8]
A contact or sharing status can depend on task, legal basis and applicable agreements. Note the status as it actually appears from the conversation or record, without assuming consent where it has not been recorded. Whether that is sufficient for the route in question remains for the physician to assess. [2] [12]
Task level makes clear whether the text concerns individual contact, case investigation, population or policy. State that level for each part of the record, so that an individual observation is not accidentally read as a population statement. This keeps the note unambiguous to place for a later reader as well. [2] [7]
Guidelines, notification routes and public health measures can change and therefore call for a date and version. State for each cited source when it was consulted, so that a dated guideline is not read as a current instruction. For the public health physician, this remains a fixed part of citing sources. [2] [10]
A handover to partner organisations needs purpose, recipient, relevant facts, data boundary, a concrete question and status. Limit the handover to what the recipient needs for that specific question and name the data boundary explicitly. The status of the handover, such as sent or still open, should stay separate from it. [2] [12] [6]
Terms such as surveillance, notification, exposure, population, case and measure each have their own task context. Use surveillance for ongoing monitoring, notification for a targeted report and measure only for a step established by the competent authority, not as synonyms. This keeps the professional language in line with what the source actually describes. [2] [8]
An indicator is a measured or calculated value that needs a definition, numerator, denominator and source. State those four elements together, so that a percentage or number is not used outside the population on which it was calculated. The physician then assesses whether the indicator is relevant for this case. [2] [12]
A source limitation indicates which question the source is able to answer and which question it is not able to answer. Always state that limitation next to the statement itself, for example when a sample was small, a period short or a measurement one-off. This prevents a cautious source statement from being read as a firm, general conclusion. [2] [7]
An observation describes what has been observed, while interpretation is an additional professional step. First note what was actually seen or measured, and then attribute the interpretation separately to the physician who gave it. This keeps it visible which part of the text is observation and which part is assessment. [2] [9]
A risk assessment requires context, current sources and professional assessment and does not follow automatically from loose text. Hysio can place the facts from the source needed for such an assessment clearly side by side, but does not state a risk level itself. That qualification stays with the physician, based on the current guidelines. [2] [10]
A preventive action is a discussed or completed action with its own source, purpose and status. Distinguish a proposal from an action actually carried out and state the result only if the source confirms it. An action that is still open then stays recognisable as open, not as completed. [2] [7]
A route that is not confirmed remains a hypothesis or notification until a suitable source supports it. Therefore write the route as a suspicion and not as an established cause, even when it seems plausible at first sight. The physician then weighs whether additional investigation or source material is needed. [2] [9]
A physician's advice belongs in the handover recognisably as stated advice, with target group, date and status. Quote the advice as the physician worded it and do not extend it into a general Hysio recommendation for comparable situations. This keeps it clear that the advice belongs to the physician, not to the platform. [2] [7] [6]
Hysio can organise source material but does not take over a notification decision, diagnosis, measure or legal consideration. The physician and the competent public authority remain the parties that make such a decision; at most, Hysio supplies the organised facts from the source for it. That boundary does not change depending on how urgent the case feels. [2] [8]
An open action needs an owner, purpose, deadline or status when these are known from the source. State a missing completion as open instead of as dealt with, even when a next step seems likely. The physician then decides whether and when that action is followed up. Admin with AI helps you keep track of those open actions per owner. [2] [12]
In public health, Hysio Starter costs €20 per month excl. VAT. The rate belongs to the plan and not to one separate profession or documentation format for public health. The public health physician remains responsible for checking and always chooses what is saved in the record as final. [13]
For public health medicine, the four profession-specific documentation formats are available, and the central profile registration with code 174 works. Public Health Consultation, Case Investigation & Contact Report, Public Health Medicine Assessment and Public Health Chain Handover show how Hysio keeps population, case, public health duty and the physician's assessment separate. With each format, the public health physician checks source, professional content and record choice themselves. [16]
You can try Hysio free for fourteen days, including for case investigation. During that period you also use the four documentation formats for public health; this page describes how they are structured. The public health physician then checks the text against source, context, record purpose and professional agreements. [14]
In public health medicine, human review remains necessary for source origin, professional content, missing information and the final record choice. Place the text next to the original contact and the designated source, and check specifically that a suspicion has not been written down as an established fact. Only after that comparison does the physician transfer the text to the record. [16]
For public health medicine, Hysio does not write directly to an electronic record; the public health physician assesses the text and transfers it themselves. That transfer is done manually, in the place in the record or registration system that belongs to the task in question. This keeps the physician the last link before information ends up in the record. [17]
For public health medicine, time can be saved by organising a conversation or dictation, but checking and safe transfer remain part of the work. Consciously factor in that checking time, especially with a notification or handover to partner organisations where a source or contact error carries through further down the chain. The time saved then lies mainly in organising, not in skipping that check. [2] [7]
Start with one defined public health task, such as a single consultation or a case investigation. As soon as Hysio has created the report, compare the text with the original contact and the designated sources before you transfer it. Always keep population, individual source, analysis, advice and administrative agreement separate, and let the physician assess the scope and conclusion. [16]
For public health medicine, Hysio does not fill in any missing measurement, consent, assessment, action or agreement. An empty section stays visible as empty, instead of being filled in with a public health assumption. The physician decides whether and how that open point is completed in the record. [2] [9]
For each contact, record when and how you spoke to someone, what they told you and what advice you gave. Keep the data of the index client and of the contacts separate. Which measure follows is decided by you or the competent physician; Hysio only organises what was said. [16]
In public health medicine, the public health physician remains responsible for choice of sources, professional assessment, correction and definitive storage. Hysio organises the information provided, but does not take over the final assessment from the physician. That responsibility does not shift to the text, not even when that text looks convincing. [16]
Hysio creates the report. You remain responsible for what you save in the record.
