Quick answer
Hysio organises the request for help, psychological symptoms, protective factors, safety, evaluation and next steps, with room for professional nuance.
In every conversation, the report organises the request for help, protective factors, safety and agreements. Your interpretation stays separate from the client's words.
Hysio Primary Care Mental Health Intake is for a first conversation about the request for help, psychological symptoms, context, safety, goals and plan. The layout follows Summary, S. Subjective, O. Objective, A. Assessment and P. Plan, with Hysio Supplementary as a sixth part that only appears when there is genuinely something to add. You keep what the client tells you separate from your observation and professional assessment. Hysio does not make a diagnosis and does not fill in a safety assessment. [1] [2] [6] [7] [3]
Hysio Primary Care Mental Health Follow-up Consultation is for a follow-up conversation with a summary, the client's perspective, observations, evaluation and plan. Only describe the change since the previous encounter when it comes up in the conversation or in a reliable source. A feeling, score or remark about safety is not turned into a diagnosis or risk level without professional assessment. [1] [2] [6] [7] [4]
Hysio Primary Care Mental Health Evaluation & Handover is for an evaluation period with the starting situation, goals, progress, the client's perspective, safety, handover question and status. Consent, information for the recipient and the professional evaluation each stay recognisable in their own right. Hysio does not make a diagnosis, does not determine a crisis level and does not write a handover without your assessment and the agreed route. [1] [2] [6] [7] [5]
Intake is intended for the first conversation or one that is newly defined, Follow-up Consultation for an ongoing contact and Evaluation & Handover for a conversation in which goals, progress and a next link in the care chain are discussed. The choice depends on the care goal and the period you are assessing. Do not let a conversation about safety automatically become an evaluation or handover when that step has not actually been agreed. [1] [2] [6] [7] [3]
Start with what the client names as the reason for the conversation and only add context that was discussed during the intake or comes from a designated source. Clarifying the request for help belongs in the subjective part: record expectations, daily functioning, level of distress and any goals without making them weigh more than the client intends. You determine the professional evaluation, ask any follow-up questions and leave a diagnosis or risk assessment to the authorised healthcare professional. [1] [2] [6] [7] [3]
Safety information means what the client, a relative or a healthcare professional has actually said about safety, warning signs, agreements or sources of support. Record the source, moment and agreed action separately. A silence, emotion or single term is no evidence for a safety assessment. When there is reason to do so, you weigh the situation professionally and, in line with the local way of working, involve the GP or the crisis route. [1] [2] [6] [7] [5]
Write goals in the words in which they were discussed with the client and distinguish between the desired change, an agreed exercise and the professional assessment. A goal is not automatically an outcome. The intake format organises goal and plan. You discuss feasibility and timeframe, keep an eye on safety and record an adjusted goal only when that was actually agreed in the encounter. [1] [2] [6] [7] [3]
Hysio does not make a diagnosis, does not fill in suicidality or any other safety risk and does not make a treatment or handover decision. Hysio can organise the request for help, observations, evaluation and agreements. You check the source, assess the meaning and follow the local route for the GP, crisis service or other healthcare professional when the situation calls for it. [1] [2] [6] [7] [3]
Put the client's experience and words in the subjective part and record the behaviour or impression of contact you actually observed separately. Avoid letting an interpretation sound like an observation. You record the professional interpretation separately and check that the description stays proportionate, traceable and in keeping with the conversation. [1] [2] [6] [7] [4]
State what you are asking the GP, why that question is relevant now, which source information is relevant and which timeframe or route has been agreed. Keep the question to the GP separate from your provisional observation. You check consent, recipient, information needs and status and leave the GP to make their own medical assessment. [1] [2] [6] [7] [5]
The intake format organises the request for help, the subjective account, the objective impression of contact, evaluation, plan and supplementary information. This keeps the client's language, observation and professional interpretation separately readable. You determine which information is relevant, check the text against the conversation and only keep a diagnosis, safety assessment or referral when you have actually assessed or agreed it. [1] [2] [8] [11] [3]
A follow-up consultation can describe what has changed since the previous encounter, how the client experiences that, which observations were made, which goals were discussed and which plan follows. Do not use assumed progress as a source. You determine the professional evaluation and only record contact with the GP or another link in the care chain when that is evident from the conversation and the agreement. [1] [2] [8] [11] [4]
Only record a safety plan or early warning signs agreement when it has actually been discussed with the client or the team. Describe warning signs, actions, sources of support and responsibilities in the agreed wording and state the source. You assess whether the agreement is current, discuss changes and do not fill in missing safety information. [1] [2] [8] [11] [5]
Set out the goal, the starting position, relevant experiences and the period side by side. Write what the client experiences as change and only add your professional evaluation when it has been expressed. You decide whether the goal is continued, adjusted or closed and do not record a result because a score or conversation happens to sound favourable. [1] [2] [8] [11] [5]
Within the local collaboration, the GP can be consulted for medical assessment, safety, referral or coordination, and remains ultimately responsible for the care you provide as the POH-GGZ. Record the reason, question, answer and follow-up action separately. The Hysio formats do not take over a GP decision. You guard the boundary between POH-GGZ observation and medical diagnosis and leave an unanswered consultation point visible. [1] [2] [8] [11] [3]
A questionnaire such as the 4DKL or PHQ-9 is usually completed digitally by the client, outside the consultation. So discuss the outcome first and record what the client says about it. A score supports the conversation and is never decisive: it does not replace professional assessment and does not produce a severity category, cut-off value or risk level. If the result differs from what you hear, you describe both and leave the question open. [1] [2] [8] [11] [4]
Record that the question was asked and that no answer or no usable answer was obtained, insofar as this is relevant to care. Do not add reassurance or a risk level. The format does not fill in missing safety information. The local safety procedure is leading here: you decide whether immediate consultation is needed and record which action was taken and who was responsible for it. [1] [2] [8] [11] [3]
Record the purpose of the handover, which information the recipient needs, which question is being asked and which consent or route has been agreed. A handover is not a summary of everything that has ever been said. As the POH-GGZ, you prepare a referral; the referral decision and the referral letter stay with the GP. You check the handover question, source, recipient and status and leave the receiving healthcare professional to make their own assessment. [1] [2] [8] [11] [5]
Keep the previous report, the current conversation, any questionnaire, observation and agreement made recognisable as separate sources. Do not automatically record that an earlier goal still applies. You check date and context and decide which information returns in the new evaluation and which part is only needed as history. [1] [2] [8] [11] [4]
Record with whom information is shared, for what purpose, which consent was discussed and which restriction or refusal applies. Do not assume consent from the fact that someone accepts a referral. You follow the WGBO, the local privacy procedure and the agreed route and keep the status of consent visible next to the substantive question. [1] [2] [8] [11] [5]
The NHG guideline ADEPD provides a framework for relevant and structured recording within general practice. There is no separate documentation guideline for the POH-GGZ: you write in the same GP record as the GP, in the SOAP structure that ADEPD prescribes. The HIS Referentiemodel (HIS reference model) names the POH-GGZ explicitly under the S line, for a more extensive intake report. The guideline does not create a new diagnosis or safety route for Hysio. You follow the local agreements on record keeping and authorisation. Voice-driven reporting fits within that SOAP structure and does not turn it into a new route. [1] [2] [13] [14] [3]
Use the client's words, the context and the concrete consequences for daily functioning. Put observation and professional interpretation in a separate part and only use a diagnosis when it has been established by the authorised healthcare professional. The evaluation contains an expressed working hypothesis, not a classification. You assess the request for help and distinguish between a signal, working hypothesis, evaluation and formal diagnosis. [1] [2] [13] [14] [3]
The WGBO requires a record with the data needed for good care and gives rights regarding access and correction. For the POH-GGZ, this means recording with a clear purpose, keeping source and professional assessment separate and not adding unnecessary details. Hysio does not determine a retention policy. You follow the general practice procedure for access, corrections, consent and final saving. [1] [2] [13] [14] [4]
As a general rule, medical records are kept for at least twenty years from the last change, with exceptions and possible longer retention. The period has not been set specifically by Hysio. You follow the record policy of the care provider and check whether a relevant evaluation or safety agreement has been saved as record information. [1] [2] [13] [14] [5]
Limit the content to what is needed for the agreed care goal and check access, recipient and route. Do not keep safety information and personal context broader than necessary. You follow the WGBO, the privacy procedure and local agreements, record consent or restrictions and correct factual errors via the designated route. [1] [2] [13] [14] [5]
The POH-GGZ job profile describes support within general practice, with attention to conversation, guidance, collaboration and boundaries. The profile is not a general statement that every POH-GGZ may make a diagnosis or crisis decision independently. You work according to your training, competence, local agreements and the availability of the GP and record consultations clearly. [1] [2] [13] [14] [3]
Record who is making the complaint, which encounter it relates to, what the person reporting describes and which complaints route has been agreed. Do not pass judgement on the client or the healthcare professional and do not automatically call a complaint an incident. You follow the care provider's Wkkgz procedure and keep the complaint, internal assessment and any follow-up action apart. [1] [2] [13] [14] [4]
Record the reason, source information, question, the person consulted, the answer and follow-up action separately. Only record consultation that actually took place and mark a contact that is still outstanding. You check whether the safety route was followed according to the local agreement and do not present the answer of the GP or crisis service as your own observation. [1] [2] [13] [14] [5]
Write an option as an option, advice as advice and a decision as a decision that has actually been taken by a named healthcare professional. Add source and date. You check the goal, consent and follow-up appointment and do not let Hysio derive a treatment, referral or medication decision from a conversation. [1] [2] [13] [14] [4]
First record what the evaluation relates to and which starting situation was used as a reference. Then describe the progress, the client's perspective, current safety and the professional assessment. You check that an old situation is not read as the current state and that consent and handover status remain parts in their own right. [1] [2] [13] [14] [5]
Use words that describe the encounter, such as request for help, the client's perspective, observation, protective factor, early warning signs agreement, evaluation and follow-up. Clarifying the request for help, level of distress, functioning and working hypothesis are the POH-GGZ's own language; classifying, diagnosing and determining the indication belong to another profession. Only use a diagnostic or risk term when it has been professionally established. You check source, time and role for sensitive wording and do not let an uncertain meaning appear as fact in the GP record. In admin with AI too, you choose the words for sensitive topics. [1] [2] [10] [9] [12] [3]
Describe which person, activity, agreement or source of support the client or healthcare professional names as support, and who that information comes from. Do not draw the conclusion yourself that someone is safe because of it. You discuss the meaning in the professional assessment and only record a concrete safety or follow-up agreement when it has actually been made. [1] [2] [10] [9] [12] [5]
State that the agreement has not been discussed, is not known or still needs to be clarified. Do not fill in an action, contact number or assessment from a standard example. You follow the local safety procedure, discuss what is needed with the client and GP and record the actual next step with the responsible role. [1] [2] [10] [9] [12] [3]
Put both sources side by side and record which clarification was requested and what remains uncertain. Do not automatically choose the most serious or most reassuring version. You assess the contradiction within the professional safety procedure and only record consultation or follow-up when that actually took place. [1] [2] [10] [9] [12] [4]
No, Hysio does not derive suicidality, crisis risk or a safety level from words, silences, emotions or a questionnaire. The formats can organise explicitly stated safety information. You carry out the professional assessment and local escalation, record source and action and involve the GP or crisis service when the situation calls for it. [1] [2] [10] [9] [12] [5]
Record the questionnaire, the version when known, date, score and the source of the answers. Then describe whether the score was discussed in the conversation and which professional meaning was given to it. You do not treat the score as a diagnosis or automatic safety decision and let the client give their own explanation for a notable change. [1] [2] [10] [9] [12] [4]
Check the request for help, relevant context, goals, current safety information, professional evaluation, consent, recipient, handover question and status. Check whether each part comes from the encounter or a named source. You only authorise once the recipient can understand the core without Hysio having added a diagnosis or risk assessment. [1] [2] [10] [9] [12] [5]
Write what the client experiences, which observation was made and which measurement or questionnaire data are actually available. Avoid presenting improvement or deterioration as the effect of a single conversation. You assess the progress over the correct period and discuss with the client which next step is appropriate. [1] [2] [10] [9] [12] [5]
State whether the statement comes from the client, a relative, the POH-GGZ, the GP or another team member, and when it was made. Record the concrete content without changing the role of the source. You check the professional meaning and make action, consent and handover status visible separately. If in doubt about the severity, you record the source and the outstanding consultation point, without assigning an urgency yourself. [1] [2] [10] [9] [12] [3]
A final assessment is the assessment that you or another named healthcare professional actually makes about the progress, the request for help and the follow-up. It is not an automatic result of words or scores. You check source and period, discuss the conclusion with the client when appropriate and only record a handover or closure if it has been agreed. [1] [2] [10] [9] [12] [5]
Hysio has three plans: Starter €20, Pro €40 and Unlimited €80 per month, excl. VAT. With yearly billing, you get two months free. Which plan fits depends on the number of sessions you record per month. Also assess source checking, privacy, safety information and professional handover before you decide to introduce it. The POH-GGZ remains responsible for the substantive assessment and follows the local agreements on access to mental health information. [1] [2] [15] [3]
Agree which psychological and health data are needed, who has access, with whom you share them and how consent or restrictions are recorded. Keep safety information purposeful and verifiable. You follow the WGBO, the privacy procedure and local agreements for the GP, client and any handover recipient. Make changes in consent or recipient visible before you hand over the information, so that the GP and client can check the chosen route. [1] [2] [16] [17] [5]
You can type manually, use a local template in the HIS or assess another dictation solution that organises the conversation. Every route requires human review of the client's perspective, observation, safety, consent and plan. So do not only compare speed, but also whether a GP or recipient can find the source and current status. [1] [2] [4]
The Hysio free trial lasts 14 days and is not profession-specific. Use that time to test whether a report stays recognisable: choose a few clearly defined intake and follow-up encounters, agree who reads along and record which corrections were needed. Pay particular attention to the client's perspective, observation, safety information and consent. Then discuss whether the structure is safe, respectful and workable for the practice. [1] [2] [15] [3]
No. Hysio does not connect automatically with every GP information system (HIS). You get a substantive report structure, not an integration guarantee; without a connection, you enter the text into the GP record yourself. Check for each practice which route is permitted, who reviews the text, how consent and source stay visible and how a handover or correction is recorded. [1] [2] [5]
Let the POH-GGZ check whether the client's words, observations, goals, safety information and plan match the encounter. Let the GP or another healthcare professional review it when the local route requires that. Record who authorises, how corrections are made and when an open safety question needs to be discussed again. The substantive assessment stays with the healthcare professional who knows the encounter and the local responsibilities. [1] [2] [3]
Check the original conversation or record source and change the sentence according to the local correction procedure. Keep visible when the correction was made and who assessed it again. An error in safety information can have consequences for consultation or follow-up. You check the current status and inform the healthcare professionals involved when the practice agreement requires it. [1] [2] [5]
Record whether the goal, the timeframe, the starting situation or the client's perspective is missing. Ask targeted questions when that fits the encounter and otherwise leave it as an open point. You decide whether the evaluation can be completed responsibly and who holds the missing conversation. Do not describe a result that has not been discussed with the client and keep a missing timeframe recognisable in the record. [1] [2] [4]
Measure the time for recording, checking, correcting and retrieving goals and safety agreements separately. Also check whether the client recognises themselves in the report and whether the GP can quickly understand the handover question. No POH-GGZ-specific time saving has been established. Use anonymised examples and stop when the structure leads to extra interpretation or safety risk. [1] [2] [3]
Start with a clearly defined intake or follow-up appointment and agree how the client's perspective, observation, safety, consent and plan will be assessed. Also test an encounter with missing information and a handover question. Only expand once the POH-GGZ, GP and practice organisation have reviewed the content, privacy and escalation route. After each test, record what was checked, which correction was needed and who oversees the follow-up. [1] [2] [5]
Hysio creates the report. You remain responsible for what you save in the record.
